1 visited 111 Pembrook Road with Donna Delaney on July 24th, 2009 at Approx. 10:30 A.M.to perform a
wellness check on Louise Conte at the request of her stepdaughter Karen Conte. Chief Martineau had
notified me that the EMT"s had removed Louise's daughter on a pink slip the day before and he
personally had been there and heard the daughter verbally threaten to kill her for letting people in the
house.
On that day we found Louise to have bruises all over her left arm and one small one on her right
arm.She stated that she was not taking aspirin or Coumadin,and did not know how she got the
bruises.Also she had what appeared to be cigarette ashes on the front of her dress.She was not willing
to engage in conversation,and stated she was fine and did not need anything.We told her to feel free
to call either one of us if she had any concerns.
I notified Judy Wetmore of protective services of my findings,who turned the case over to Diane Powell
and I spoke to her several times that day on the phone.She was going to try to send someone out that
day.She was very familiar with the case. Diane Powell's#978-946-1484.
Chief Martineau and Donna Delaney were also filing a report.

Hello Scott,
Just an update on Louise Conte and the 111 Pembrook problem, here in N.Andover. Yesterday I met a general
contractor who is working on the home. He states that so far he has removed or thrown out all the rugs and
furniture in the home that were unsalvageable. The walls and ceilings have been cleaned or stripped of wall paper
and painted. He is going to tile the kitchen and bath floors. There are bathroom fixtures and wall rot that need to
be addressed too.A licensed electrician will be brought in for electrical issues. He is a family member,who feels
to be very conscientious about the issues.
I believe the family still feels that Louise and her daughter should not be allowed back in their home for their own
safety, but the Health Departments jurisdiction will end when the sanitary code violations are corrected. The
mental health or personal danger issues are unfortunately not within my purview. I will let you know when the
Health Dept. gives the property a certificate of compliance. Thank you.

An authorized inspection of 111 Pembrook Ave was performed by Board of Health staff
on June 12, 2008 at which violations of 105 CMR 410.000 Chapter II of the State Sanitary
Code, Minimum Standards of Fitness for Human Habitation were found. Failure to
respond within the allotted time period may result in a Board of Health finding that the
dwelling is unfit for human habitation.
This premise is currently unfit for Human Habitation. According to the State
Sanitary Code 105 CMR 410.831 (D), "If at any time the board of health determines in
writing that the danger to the life or health of the occupant is so immediate that no
delay may be permitted,then the board of health may immediately issue a finding
that an occupied dwelling or portion thereof is unfit for human habitation without
providing the notification or hearing specified in 105 CMR 410.831."
No portion of this dwelling shall be occupied without the prior written permission of
the board of health based upon the board's written finding that the dwelling or
portion thereof to be occupied is fit for human habitation. All violations listed below
must be corrected.

The above mentioned items all contribute to an odor problem. Throughout the dwelling there
was a strong stench of old food,tobacco,burnt materials, urine etc. Once all damaged materials
are removed, all surfaces should be cleaned and sanitized.
Please submit a plan of action for the correction of these violations along with signed contracts
of for any work that is to be conducted as well as the completion report. It is recommended that a
professional cleaning company be contracted as this dwelling is in a highly unsanitary condition.

smoke detectors and carbon monoxide alarms in every dwelling
that is required to be equipped with them
Consult with the Fire Dept and install detectors and alarms as recommended
6) Bathroom

7) Throughout home piles of soiled clothing,trash and debris were found 410.602(B)
- The occupant of any dwelling unit shall be responsible for maintaining it a clean and sanitary condition
and free of garbage,rubbish,other filth or casuse of sickness
Dispose of all trash and garbage and debris. Sanitize.
The above mentioned items all contribute to an odor problem. Throughout the dwelling there
was a strong stench of old food, tobacco, burnt materials, urine etc. Once all damaged materials
are removed, all surfaces should be cleaned and sanitized.
Please submit a plan of action for the correction of these violations along with signed contracts
of for any work that is to be conducted as well as the completion report. It is recommended that a
professional cleaning company be contracted as this dwelling is in a highly unsanitary condition.

Just an update on Louise Conte and the 111 Pembrook problem, here in N.Andover.Yesterday I
met a general contractor who is working on the home. He states that so far he has removed or
thrown out all the rugs and furniture in the home that were unsalvageable. The walls and ceilings
have been cleaned or stripped of wall paper and painted. He is going to tile the kitchen and bath
floors. There are bathroom fixtures and wall rot that need to be addressed too. A licensed
electrician will be brought in for electrical issues. He is a family member,who feels to be very
conscientious about the issues.

I believe the family still feels that Louise and her daughter should not be allowed back in their
home for their own safety, but the Health Departments jurisdiction will end when the sanitary code
violations are corrected. The mental health or personal danger issues are unfortunately not within
my purview. I will let you know when the Health Dept. gives the property a certificate of
compliance. Thank you.
