P.0.Box01971-8560 T978-745.6817 I F978.745.6067 www.structures-north.com 27 January 2017 Town of North Andover, MA 120 Main Street North Andover, MA 01850 Attention: Donald Belanger Reference: East Mill Penstock Stabilization North Andover, MA Dear Mr. Belanger: On Tuesday 17 January and Friday, 20 January 2017, we visited the East Mills in North Andover to do an investigation of sink hole that had formed below the first floor slab. Sinkhole Discovery- • Two 48 inch metal pipes ran under the building from the retained mill pond on its east side toward Osgood Pond to west. These connected with the mill pond in a large concrete chamber behind a trash screen and movable gates, forming a penstock. • The pipes met the chamber in two 48" diameter holes in the concrete foundation under the east wall of the mill. • It had been determined that the pipes had partially rusted and caved in as their flow had become restricted and a plume of soil had been discovered at their downstream outlet. Preparations were being made to replace the crushed sections of pipe. • When the pipes were drained and inspected directly, a large, 15 foot by 20 foot by 50 foot cavern was discovered above the rusted holes. • We were promptly contacted by the owner to investigate and help stabilize the situation. We reviewed videography that had been done from below and did a quick inspection of the building, observing the following: - The floor slab dipped by a small amount over the cavern, being higher along the column lines. From the still images within the cavern one could see rectilinear concrete foundation walls and wide arches that spanned over the pipe alignment. t Structures North East Mill Penstock Stabilization 27 January 2017 North Andover, MA - We could also see the underside of the slab at the top of the cavern. We were told that the building had been constructed directly over a river that had followed the route of the penstock. - No sign of global movement had been observed in the building structure and the east wall appeared sound and plumb. • Out of concern that the floor slab could collapse into the cavern we immediately advised the owner to evacuate the office spaces that were over the subgrade disturbance. Based upon all of the above, it appeared that the cavern had been initially created by soil sifting into the pipes when the rusted holes were created. This sifting increased to scouring as the holes enlarged. The flow was restricted after the pipes began to collapse and additional soil started to clog the flow. Because of the building foundations would have been constructed more deeply than the former river, or the pipes that replaced it, it was not plausible that the foundations would have been undermined since the disturbance was above the inverts of the pipes. The slab was assumed to partially span over the top of cavern as a catenary, and partially rely on the wedges of soil that remained arched around the sides of the cavern. The building's east foundation wall between the gate chamber and the penstock had once acted like a dam holding back pond water that had pressed against the soil below the floor slab, while at the same time holding the soil back over a vertical height of approximately 18 feet. Stabilization - It was decided that the optimal solution would be to fill the cavern with about 8 feet of concrete over the pipes and soil base and to fill the rest of the cavern with flowable fill up to the bottom of the slab. We compared the lateral outward pressure that was exerted against the east wall by the displace soil with the more concentrated fluid pressure of the concrete and flowable fill and determined that each placement would need to be limited to five feet of height would need to stiffen before the next successive lift. The concrete and the flowable fill were provided by "J.G. MacLellan Concrete Co., Inc.". The mix design of the products is attached at the end of this report. In order for the concrete mix to be poured, two openings were cut through the slab. The openings were approximately one square foot each in area and 18 feet apart, 2 Structures North 27 January 2017 East Mill Penstock Stabilization North Andover, MA one of which is close to the east exterior wall adjacent to the pond and the other one is located in. the storage room closer to the west elevation exterior wall. For the purposes of this report we will refer to them as opening 1 and opening 2, respectively. It is noteworthy that there is a wall beneath the slab, parallel to and 12 feet away from the west exterior wall. The concrete mix was poured on Monday, January 23. The volume of the poured concrete was 209 cubic yards. The flowable fill pour was designed to take place at the maximum height of 5 feet at a time. After the concrete pour, the distance from the top of the concrete to the top of the slab was 105". Deducting the average thickness of the slab (6") would suggest two pours of 48". The first pour started at 7:30 am and ended at 10:30 am. 5 feet of flow fill was poured in opening 1 and 5.5 feet in opening 2. A few hours had to pass for the flow fill to reach a certain strength at which it could hold its own weight and not transfer it to the surrounding walls. The second pour started at 2:00 pm and ended at 3:30 pm. 3 feet of flow fill was poured in opening 1 and 2.5 feet in opening 2. The total of 182 yrd3 of flowable fill was used. On Wednesday, January 25, 2017, we visited the building to inspect the result of the job done on the previous day. The condition of the building was satisfactory and no deformation in the walls, cracks in the flow fill and irregularities in the floor was observed and the condition was suitable for the openings to be covered with reinforced concrete. No work may be authorized beyond 30 days from the date of this certification without written approval of the Department. D. Special Conditions Erosion controls will be installed between all work and the pond including a siltation boom at the pipe outlet and prior to construciton of the new pipe inlet. A wetland scientist, approved by the Conservation Department, shall assess the impacts to resource areas that may have occurred due to the pipe collapse and the subsequent emergency measures implemented before the issuance of this permit. During emergency operations the designated wetland scientist shall inspect the site to assess any impacts to the resource areas and provide a report to the Conservation Department summarizing the weeks activities, the condition and functionality of the required erosion controls, and any mitigation that may be required as the result of the emergency activities. Pumps should be kept on site for emergency E. Appeals The Department may, on its own motion or at the request of any person, review: an emergency certification issued by a conservation commission and any work permitted thereunder; a denial by a conservation commission of a request for emergency certification; or the failure by a conservation commission to act within 24 hours of a request for emergency certification. Such review shall not operate to stay the work permitted by the emergency certification unless the Department specifically so orders. The Departments review shall be conducted within seven days of: issuance by a conservation commission of the emergency certification; denial by a conservation commission of the emergency certification; or failure by a conservation commission to act within 24 hours of a request for emergency certification. If certification was improperly granted, or the work allowed thereunder is excessive or not required to protect the health and safety of citizens of the Commonwealth, the Department may revoke the emergency certification, condition the work permitted thereunder, or take such other action as it deems appropriate. Several test holes were drilled in the slab which revealed the extent that the structure was undermined. On January 23, 2017, Building 11, was declared an unsafe structure by Donald Belanger, Inspector of Buildings/Building Commissioner and was posted as such. The building was declared, unsafe in accordance with the Eighth Edition of the Massachusetts State Building Code 780 CMR Section 116. David Steinbergh through Mr. Wathne P.E. is making the structure safe, and will be submitting a letter along with a full engineering report per our conversation. Mr. Steinbergh has been told to submit a building permit along with all associated documentation to the Building Department. North Andover Conservation Commission. This letter servesas the document you requested during your site visit on Monday, January 23, to provide details about the emergency activities taken thus far and remaining to be performed. During a recent inspection of the culvert system that runs from the edge of the Mill Pond at East Mill to the outlet structure at Osgood Pond, at West Mill, we discovered that two 48" steel 'pipes located under Building 11 at East Mill, had failed and needed immediate repair. These pipes are the only path for water coming down the Cochichewick Brook into the Mill Pond to flow through the Property and to Osgood Pond. This water ultimately flows into the Merrimack River. When we discovered the pipe failure, we engaged National Water Main Cleaning Company (NWMCC) to immediately repair the pipes. This would entail temporarily pumping down the pond into a catch basin in the culvert system downstream from the damage so they could enter the pipe, make repairs, and remove any excess debris from the system. When NWMCC entered the pipe on the morning of Friday, January 20, they found the conditions under Building 11 to be unsafe due to the undermining of the structure and the instability of the earth that remained. NWMCC demobilized and recommended we keep the pumping operation in place until a solution could be designed (as flow under the building could cause further undermining). At that time, RCG NAM engaged a structural engineer, John Wathne of Structures North. He supervised a test drilling of the slab in Building 11 to determine whether the structure, which was occupied at the time by office 1 waddles; booms and turbidity curtains in each pond, and slit sacks at the catch basins. Prior to removal of the sedimentation controls, we will enter the pipes and sluiceways to remove material that accumulated as a result of the collapse. In the future, a new inlet control structure will be designed and permitted through the local conservation commission. We trust that the details articulated in this.letter clearly support the need for an emergency work permit. Thank you for your consideration and assistance.
