Dear Ms. Starr:
We have revised the Sewage Disposal System design plan for 1423 Salem Street based on the written
comments of John Noonan in a letter dated March 28, 2002. Specifically, we have changed the plans as
listed below.

1. We propose to convert the existing D-Box to a DMH. We propose the installation of a manhole
riser to the surface with a water tight connection.
2. The breakout requirements of 310 CMR 15.211(1)[4], specifically 15' to a 3:1 slope, can be
achieved on this site. However, this would require an additional 800+ square feet of disturbance
in the buffer zone to nearby wetlands. The conservation commission preferred and recommends
that we reduce the amount of this disturbance, therefore we proposed the impervious barrier. We
are proposing a plastic membrane barrier. This barrier material meets DEP's Guidelines for
Design and Installation of Impervious Barriers and Slope Stabilization for Title 5 Systems dated
March 1, 2002, attached.
3. A note has been added to the plan view as well as the profile stating that the outlet pipes from the
distribution box shall be set level for the first two feet. Additionally, the profile has been
graphically changed to demonstrate this.
4. Buoyancy calculations were provided with the orig
inal submittal on 1/15/02. We are resubmitting
them along with this revision.set.
5. The vent detail has been revised to show an elbow up at the end of the distribution pipes.
6. Spot grades of 102.4 have been added to the downhill side over the leaching area.
7. Minimum pipe slopes have been provided on the profile.
