Construction was completed by F.P.Reilly Inc of Andover, MA. This inspector was on sit twice during the two days where most of the construction took place. Once installation of the new sewer was complete and the driveway was re -paved I allowed the removal of the haybales, but not the silt fence, so grading and stabilization could be completed. As of today the work is complete and the site is stabile. The area along_ the driveway adjacent to the culvert crossing was stabilized v,� th ripz� p while the balance of the site was loamed and seeded. The areas that were loamed and seeded have an established grass cover, however the silt fence needs to remain in place until the grass is fully stabilized in the spring. The sewer shall be installed over the culvert through the center of the driveway only. If this is not possible, the applicant must file a Notice of Intent with the Conservation Department. 2. Work shall occur during the drier months between July 15 and September 15 only. Pre -construction 3. Prior to commencement of work, the centerline of the sewer line shall be staked in the field for review and approval by the Conservation Administrator or the Associate. 4. Prior to commencement of work, the applicant shall submit to the Conservation Department for approval, a detailed sequence of construction, including the installation of sedimentation/erosion control devices. 5. Prior to the commencement of work, a de -watering basin shall be set up in a location approved by the Conservation Administrator or the Associate. 6. Prior to commencement of work, staked hay bales backed by trenched siltation fence shall be placed along both sides of the driveway along a distance to be determined and approved by the Conservation Administrator or the Associate during an on site inspection. X1,7. A minimum of 15 hay bales and sufficient stakes for the staking of said bales shall be on site for emergency purposes. 8. A wetland Scientist shall be designated as an "Erosion Control Monitor" to oversee the sewer crossing work. 9. The Conservation Department shall be contacted at least 72 hours in advance of the start of work for an on-site meeting to ensure that: the erosion controls were placed in the approved location and properly installed, all other pre -construction conditions have been complied with, and all conditions are understood. Durine Construction 10. The Erosion Control Monitor must be present on site during the work duration of the sewer line installation over the culvert. 11. The Conservation Department shall be notified immediately if de -watering activities are necessary. 12. The De -watering activities shall be supervised and witnessed by the Erosion Control Monitor and the designee must be on-site while work specific to dewatering and the culvert crossing is occurring and this section is complete. 13. Water from any de -watering shall be properly discharged into an approved settling basin and shall not be discharged directly into any wetland resource area. There shall be no stockpiling of soil or other materials within twenty-five feet of any resource area for a time period of more than 10 hours. After Construction 15. Upon completion of the work, all disturbed areas shall be properly stabilized against erosion. 16. CONSERVATION COMMISSION Environmentat & Transportation Engineers April 22, 1999 Mr. & Mrs. Kevin Merli 172 Bridle Path North Andover, MA 01845 Re: Sewer Contract 3C File No. 58034.00 Dear Mr. & Mrs. Merli: We, as consultants to the Town of North Andover, have been contracted to complete the design of sewers for Bridle Path, Winter St. and So.°Bradford Road. Because of the low houses along Bridle Path we have spent some time coming up with the best solution for the Town and the property owners. The form that we previously sent was to initiate our lines of communication. We have met with many of your neighbors; we have met with a representative of the Conservation Commission and we have taken elevations on your driveway. Based on the information we have collected, we have determined that it will be possible to serve your house by gravity. The sewer connection will be about 10 feet deep at the street line. It_ will be possible to construct the sewer over the culvert, however, because of the flat grade in your drive theer_vvill_ofznecessi be flatter than the standard house connection. We attach a profile of your driveway for your information. Before installing the pipe you should verify all elevations. Prior to proceeding with the work you must get the approval of the Conservation Commission. It will be necessary to stay in the drive to minimize the environmental impacts.
