Type .Campgrounds,, State Sanitary Code: Chapter VI, I performed my annual
inspection of your facility on May 24, 2006.
Enclosed is a copy of .the inspection report for your review. This office
would appreciate knowing your plans for correction of the conditions noted on
the report. The Plan of Correction should be submitted to my attention at the
address listed above, within 30 days of receiving this letter. The Plan of
Correction should detail the specific steps that will be taken and .the date
of expected compliance.

"Facilities, equipment, and firebreaks for fire
prevention and fire fighting shall be provided as may be
recommended by the local fire department or fire warden .

of debris and growth. "

"The toilets shall be so located that they are not more than
500 feet from the camp sites of those people who are expected
to use them. Self-contained units are exempt from this 500-
foot requirement . The operator shall provide an adequate
supply of water and toilet paper . The operator shall provide
for the ventilation of each toilet room to the out doors and
shall screen each window or other exterior opening with
screens containing not less than 16 meshes per inch. A self-
closing solid door shall be considered as satisfying this
requirement . "

individual or group renting the space enforces the
prohibition against smoking."

* The swing seats are 27 inches apart; they are 31 inches
from swing supports.
The U. S . Consumer Product Safety Commission recommends
that the distance between the seats and the swing
supports be a minimum of 30 inches apart . They recommend
that seats be a minimum of 30 inches apart.
Green Slide
It is recommended that either rubber mats be installed,
or more wood chips be raked into the landing zone of the
slide .

That is all Sate owned land made up of many parcels we have no listing for 1951 turnpike, which is just a
mailing address for the the ranger station out on Middleton Street in the state forest. But the entrance from
Turnpike to Harold Parker is Map: 108.0 Parcel: 44 on the North Andover(right)side and Map :108.0
Parcel:43 On the Middleton (left)side.
The Harold Parker State Forest, lies in North Andover, Andover, North Reading and Middleton, and comprises
just over 3000 acres with about a 1/3 of it in North Andover.

Dear Mr. Scott:
Enclosed is a Report of Inspection of the Harold Parker State Forest conducted on June 11,2002.
The inspection was conducted by Joel Hollis,RS in accordance with 105 CMR 440.000,Minimum
Standards for Developed Family Type Campgrounds, State Sanitary Code: Chapter VI.
The electric hand dryers in several comfort stations are inoperative.They should be repaired of
removed. Other concerns include the unsafe swing seats at the playground,the improper dumpsters and
the clarity of the swimming water.The Commonwealth should not be paying the dumpster vendor for
substandard equipment.
These issues were called to the attention of the Department of Environmental Management last
year along with the request that a plan of correction be submitted to this office.No plan was received.
Please provide this office with a plan of correction, addressing each violation noted on the
enclosed inspection report within 21 days of receipt of this correspondence. Should you have any
questions,please do not hesitate to contact our office.

State Forest on May 16, 2000. The inspection was conducted by Joel Hollis, R.S., a
representative of this Department, accompanied by Toni Mendes.
Work needs to be done in Comfort Stations One and Two. We also have some concern
relative to the area used for swimming. Prior to use, it is essential that the clarity and bacterial
quality be assessed to ensure that it meets the minimum standards. Please provide this office
with the results of the bacterial testing.

Campgrounds, State Sanitary Code, chapter VI, an inspection of the facilities at Harold Parker
State Forest was made on May 18, 1999. The inspection was conducted by Joel Hollis, R.S.
accompanied by Bruce Colburn.
This hot water heaters in Comfort Station 2 is still not operating. This hot water tank was
replaced last year, but it has never worked. You are hereby ordered to provide hot water,with a
range of 110°F— 130°F,to all handwash sinks and showers by May 28, 1999. It is also required
that the electric hand dryers be repaired/replaced or dispensers with paper towels in Comfort
Stations 2 and 4 by that same date.
Other work that needs to be accomplished in the Comfort Stations include repair of
ripped screens and shower stalls.
The inspection also raised concern about the clarity of the water at the bathing beach.
Unless the clarity meets the standards of 105 CMR 440.000, swimming is not permitted. Please
inform this office of the findings of your investigation in this regard.

Dear Mr. Frederick:
In accordance with 105 CMR 440.000: Minimum Standards for Developed Family Type
Campgrounds, State Sanitary Code: Chapter VI, an inspection was made of the Harold Parker
State Forest Park by Joel Hollis, R.S., on May 30, 1997. The conditions at this campground
have not significantly approved over the past several inspections.
Evidence of tobacco use was noted in campground buildings. These buildings must be
posted with signs noting that smoking is prohibited. The comfort stations exhibit mold, contain
broken tiles and several contain conditions which could lead to physical injury.
Extensive algae growth was noted at the bathing beach. Bathing is not allowed at any
beach where a black disk six inches in diameter, on a white background placed at a depth of at
least four feet of water, is not readily visible from the surface. It is also necessary that the water
be biologically tested in compliance with the requirements of 105 CMR 445.000.
Please provide this office with a plan of correction within 21 days of receipt of this letter.
The plan of correction should outline what action will be taken to correct the noted violations.
We would also appreciate receiving a copy of the results of any water testing done this season.

The conditions in the comfort stations are deteriorating. Many of the problems in these facilities
have been noted since 1995.
Please provide this office with a Plan of Correction within 7 days of this receipt of this letter. Our
records do not reveal that Plans of Correction have been filed for the previous three years.

be obtained only from a source approved by the
Department . Any campground restricted to tent camping
and backpacking shall be exempt from this regulation if
an approved water supply is available within 1000
feet .

440 . 102A The inlet surround was damaged during snow plowing. It
must be repaired.
Holding tank dumping stations shall be connected to a
municipal sewerage system or to subsurface facilities
designed in accordance with Title 5 of the State
Environmental Code, and shall consist of a 4" diameter
inlet with a hinged or chained cover. The inlet shall be
surrounded by a concrete apron pitched to drain into the
inlet and curbed to exclude adjacent surface water. "

All pipes and pumps delivering drinking water shall be
. installed and maintained in good working order and in
accordance with the state plumbing Code, 248 CMR 2 . 00 . The
area surrounding a pump or hydrant used for water supply
shall be maintained in a properly drained and sanitary

Comprehensive Compliance Inspection at Harold Parker State Forest,
1951 Turnpike Road, North Andover, Massachusetts . The purpose of
this inspection was to determine the compliance status of Harold
Parker State Forest with regard to the Massachusetts environmental
regulations referenced above .
On the inspection date, regulatory violations were observed.
On August 29, 1997, a Notice of Noncompliance (File No. NON-NE-97-
9125-2E) was issued to Harold Parker State Forest . On September
12, 1997, Department personnel conducted a follow-up review. Based
upon a review of the required submittal, it has been determined
that Harold Parker State Forest had complied with the requirements
of the Notice of Noncompliance as of the date of the follow-up
review.

In accordance with 105 CMR 440 . 000 : Minimum, Standards for
Developed Family Type Campgrounds, (State Sanitary Code, Chapter
VI) , Joel Hollis, RS of this Department conducted inspections of
the above-cited camp grounds between September 7 and 23 , 1993 . 105
CMR 440 . 21 (1) provides that a family type campground shall not be
operated by the Commonwealth without the approval of this
Department .
Many of the facilities inspected have common problems
including the lack of backflow preventers on the hose bibs, toilet
facilities in disrepair, and improper curbing and lack of clean up
hoses at the dumping stations . Additionally, there are no plans on
file with this Department showing the buildings . structures,
fixtures and facilities including the sources of water supply and
the works for the disposal of sewage and waste water.

Of particular concern are the leaching facilities at the Lake
Dennison Camping Area. The leaching facilities at this camp are
flooded approximately every 3 - 5 years as the campground is built
on the flood plain of a dam. Continued use of these facilities
will be based upon approval by the Department of Environmental
Protection.
The beaches at the Myles Standish Forest exhibited extensive
algal growth. Bathing and swimming shall not be permitted in water
when a black disc, 6 inches in diameter, on a white field placed at
a depth of at least 4 feet of water is not readily visible from the
surface of the water; or when under normal usage, such disc is not
visible from the surface of the water when placed on the bottom
where the water depth is less than 4 feet .
In addition to an opinion from the Department of Environmental
Protection relative to the continued use of the leaching facilities
at the Lake Dennison Camp, please provide a plan of correction

NOTICE OF COMPLIANCE
On May 3, 1995 a Notice of Noncompliance citing your system for
failing to monitor for NITRATES was issued. As a result of further
investigation by the Department, it was determined that Harold
Parker State Forest was not in receipt of the monitoring sample
schedule from the Department of Environmental Protection.
As a result of the above stated reason, the Department' s Notice of
Noncompliance, Monitoring Violation for NITRATES program dated May
3 , 1995 .has been rescinded. Please note however that this does not
relieve you of your responsibility for continued compliance in the
future. Your monitoring sample schedule is being sent to you under
separate cover.
