Duplex receptacle and data/telephone outlets should be installed on exterior wall. Receptacle at base of the installed stair will not be usable. ♦ Storage/Mail Room: These rooms were combined to make one room. Plan shows switches with pilot lights. The three way switches installed do not have pilot lights. The closet opposite to this room is shown to have a 4' strip and a 2' strip light was installed. The switch is shown with a pilot light. The switch installed does not have a pilot light. ♦ Storage Closet: The storage closet located at the stair leading to the second floor is shown with switch having a pilot light. The switch installed does not have a pilot light. ♦ Copy Room: The receptacles installed in this room are installed in EMT. This room is a finished space and the drawing require that this room be installed in wiremold if the work can not be installed recessed. ♦ The panelboards shall all have typed index cards installed reflecting the actual installation. (Note while I was at the site the electrician was present and indicted that he was there to trace out the circuits and complete the panel board indexes.) ♦ As-built drawings should be submitted for the church's records. There are many Items of work to be completed. 45.2 DPA will notify Mr.Nicene when and if all the major items are done then a minor copy of the Puneb Ust can be obtained and sent forward to Mr.Nicetta so he can get a final occupancy permit. 45.3 DiDio gave DPA a copy of the building card with all the signatures ou lt.It Wasn't until today that it was finally complete for that portion of the job.There is stlil a number of items to be completed both on the new addition and en the existing basement. 45.4 DPA was given the change order by Mr.Silva which DPA will review,Sign,and give to Fr. Keyes for his signature and then forward it to the Chancellor. 45.5 An application for payment was submitted and will be reviewed by DPA and Mr.Silva.lif it seems in order,DPA will sign it and have Fr.Keyes sign it and forward it to the Chancellor. There were a number of items on the fist from Field Visit#44,some ef,which'is already covered in No.44.2,stating that the paving contractor has been nu ied that he is getting cracking in the pavement that was just jnstalled and he must tome back to address that. Fr. Keyes is unhappy with the grooves that are disedoring at the pavement bat they will probably come back and repaint after they re-patch all the cracIring,wherever it occurs. 45.6 • The issues of item 44.3 still exist. Kr.'DiDio was given until next Wednesday to complete the punch list created today and all other hem of the punch list by%W Wednesday,October 24,2001,at 2:00 p.un. Contractor will provide materials. There are a number of pieces of hardware that need to be readjusted and relocated due to the improper locking situation. In general we hope to have all the outstanding issues resolved by'Wednesday,October 24.2001. 44.X Mr. DIDio did not love his inspections completed as required by all parties. Be had misplaced the Building Inspectors'sign-off card. The date was extended to Friday,October 12,2001. Mr.DIDio said that the Electrical Inspector made inspection on the building but according to Mr.Ippolito the Electrical Inspector did not inspect anything. He just walked through the space and gave b1m his approval. MnIppelito has a list of items that the electrical subcoutraetor should be doing and has not done yet so even the inspection was made it is not acceptable. 44.2 The subcontractor doing the pavement was there this afternoon to try to complete his work, doing the detail requested by Fr.Keyes and DPA. 44.3 Mr.Nicetta was present at the site and had concerns about sow of the workmanship. DPA recognized this as an inane. He explained to Mr.DiDio that he should do his own punch list to reexamine his subcontractors who have done work before we do our review. He can do his own corrective work rather than as giving a long punch iia#. DPA gave Mr.INDio until Friday,10/12101 to eowplete this work Mr.DiDio insisted that DPA do a punch list but DPA felt Mr.DiDio is not ready for that yet,especially since the punch list done a month ago on the basement still has not been completed. Once he feels be is at a reasonable level then DPA will advise Mr.Tom Ippolito to please start doing the punch list and DPA will coordinate with Mr.Ippolito,but only after Mr:DiDio completes his work 44.4 The electrical work has not been completed in the basement or In the addition. 'There are still a uumber of issues to be resolved. DPA will net do.a punch list until those items Are resolved. 44.5 The Fire Inspector is scheduled to make Its inspection on Thursday..DPA warned the Contractor to make sure everything is in operation,emergency battery units,etc...,and that all those items are working because if they are rejected they may not be able to obtain another inspection for another month,so DPA gave fair warning for this work,to be done. 44.6 In the meantime paving was continuing on the exterior of the building. There is a number of issues on the outside that have not been resolved. We cannot emphasize enough that the work must be dome and that Mr.DiDio must complete It. 44.7 DPA noted that they put the chair rail in those area but did not put the panel moulding on the wails,so it looks unprofesslonaL DPA is emphasizing that that should be completed too. 44.8 It was requested that Mr.DiDio provide 430 wall door amps. 44.9 Completed. 44.10 Completed. 44.11 The thermostat is missing from the new storage room. I am stating that this is untrue. During my inspection I walked around with the electrician and required the following: 1. That all panels be marked with typed directories 2. All lighting fixtures trims and plates be installed 3. All items on the electrical engineer's punch list be completed 4. A final report by the electrical engineer is due with his stamped approval of this project as stated in 780 CMR 116.0 and 116.4 Mr. Ippolito is not the assigned electrical engineer on this project,nor is he a certified electrical inspector and should not be evaluating what I am observing during my inspections by further questioning my duties. Mr.Anthony DiDio Design Partnership Arch,- Mr. Angelo Petrozzelli Item# The purpose of this meeting was to disc=the following: 45.1 A Punch List was done today on the new addition portion of the building.There are many Items of work to be completed. 4S..2 DPA.will notify Mr.Nicettx when and if all the major items are dome then a minor copy of the Puneb fast can be obtained and sent forward to Mr.Nicetta 90 he can get a final occupancy permit. 45.3 MDio gave DPA a copy of the building card with all the signatures on-it.It wasn't until today that it was finally complete for that portion of the job.'There 4 still a number of items to be completed both on the new addition and en the existing basement. 45.4 DPA was given the change order by Mr.Silva which DPA will review,sign,and give to Fr, f Kcycs for bis signatare and then forward it to the Chancellor. 45.5 An application for payment was submitted and will be reviewed by DPA-and Mr.Silva.V it seems in order,DPA will sign it and have Fr.Keyes sips it and forward it to the Chancellor. There were a,number of items on the So from Field Visit#44,some of which'is already covered in No.44.2,stating that the paving contractor has been notified that he is getting cracking in the pavement that was just installed and he mast come back to address that. Fr. Keyes is unhappy with the grooves that are discoloring at the pavement bort they will probably come back and repaint after they re-patch 0the cracking,wherever it occurs. 45.6 The issues of Iters 44.3 still east. Mr.DiDio was given until next Wednesday t0 complete the punch list created today and all other items of the punch list by uW Wednesday,October 24,2001,at 2:00 p.ur. There an a number of pieces of hardware that need to be readjusted and relocated due to the improper locking situation. In general we hope to have all the outstanding 'issues resolved by Wednesday,October 24,2001. 44,1 Mr. DiDio did not have his inspections completed as required by all parties. He had misplaced the Building Inspectors'sign-off card. The date was extended to Diday,October 12,2001. Mr.DlDio said that the Electrical Inspector made inspection on the building but according to Mr.Ippolito the Electrical Inspector did not inspect anything. He just walked through the space and gave tuna his approval Mr..Ippolito has a list of items that the electrical subcontractor should be doing and has not done yet so even the inspection was made it is not acceptable. 44.2 The subcontractor doing the pavement was there this afternoon to try to complete his work, doing the detail requested by Fr.Keyes and DPA. 44.3 Mr.Nicetta was present at the site and had concerns about som of the workmanship. DPA recognized this as an issue. He explained to Mr.DiDio that be should do his own punch list to reexamine his subcontractors who have done work before we do our review. He can do his own corrective work rather than us Shi ag a long punch list. DPA gave Mr.DiDio until Friday,10/M1 to complete this work Mr.DiDio insisted that DPA do a punch fist but DPA felt Mr.DiDio is not ready for that yet,especially since the patch list done a month ago on the basement still has sot been completed. Once he feels be is at a reasonable level then DPA will advise Mr. Tom Ippolito to please start doing the punch list and DPA will coordinate with Mr.IppWto,but only after Mr:DiDio completes bis wont. 44.4 The electrical work has not been completed in the basement or in the addition. 'There arc still a cumber of issues to be resolved. DPA wig no do.a punch list until those items are resolved. 44.5 The Fire Inspector is scheduled to matte its inspection on Thursday..DPA warned the Contractor to make sure everything is in operation,emergency battery units,etc...,and that ala those items are working because if they are rejected they may not be able to obtain another inspection for another month,so DPA gave fair warning for this work to be done. 44.6 In the meantime paving was continuing on the exterior of the building, There is a number of 1 issues on the outside that have not been resolved We cannot emphasize enough that the work must be dome and that Mr.DiDio must complete it. 44.7 DPA noted that they put the ehair call in those ureas but did not part the panel moulduog on the wails,so it looks unprofessional. DPA is emphasizing that that should be completed too. 44.8 It was requested that Mr.DiDio provide 430 wall door stops. 44.9 Completed 44.10 Completed 44.11 The thermostat is missing from the new storage room. Please provide. 44.12 DPA forwarded Mr.DiDio's information regarding the change orders to Mr.Peter Silva for his re-view and.preparation of a Change Order. I am stating that this is untrue. During my inspection I walked around with the electrician and required the following: 1. That all panels be marked with typed directories 2. All lighting fixtures trims and plates be installed 3. All items on the electrical engineer's punch list be completed 4. A final report by the electrical engineer is due with his stamped approval of this project as stated in 780 CMR 116.0 and 116.4 Mr.Ippolito is not the assigned electrical engineer on this project,nor is he a certified electrical inspector and should not be evaluating what I am observing during my inspections by further questioning my duties.
