(She was there with NA Public Works to look at the future replacement of their culvert.) She looked quickly at our work area and did not voice any issues. I told her I was not going to replace the hay bales (adjacent to the area used for temporary roadway) that floated away, as the silt fence was still intact and there was a good growth of grass started. She agreed. Alison wanted to the restoration after we made these repairs (as she did not have her file with her). After the work is completed, I will set up a new inspection date. The work will most likely involve a little less then one day of work. The (4) four Pussy Willow plants were washed out and new ones need to be purchased and planted. The same size and type that was planted. before should be purchased. (GLSD will supply the mulch for the top dressing over the plants.) The crew should bring a wheel barrow (along with their regular small tools) as a small wash out at the end of the pipe repair needs to be filled. Also a 10 by 10 foot loam and seed area alongside the road (where the bypass road was) needs to be regarded and loam and seed installed. White should purchase a small bag_of grass seed and also bring the straw blanket that Fernando recently purchased for this job. GLSD will have a front end loader deliver loam for the crew to use. No special equipment, except for the rack truck (with its small tools), is required. Please invoice this repair work, including the purchase of the seed and straw blanket, on separate invoices marked "Repairs due to Flood Damage". The request is considered an emergency since the condition of the weakened section of the sewer force main pipe would present an imminent threat to public and environmental health should the pipe fail and discharge raw sewage to the nearby intermittent stream, which is a tributary to the Merrimack River. Also submitted with that letter was a memorandum from Isabel Tourkantonis of Camp, Dresser & McKee (CDM) dated November 3, 2005 and a plan entitled "Water Pollution Abatement Facilities' dated January 1972, revised June 1976, which details the location of the work, the scope of work and environmental protection measures to be implemented for the work. On Tuesday, November 8, 2005, I conducted a site inspection with yourself and Ms. Tourkantonis of CDM to review the details of the proposed work and to determine further additional conditions necessary for work to proceed. The work will consist of excavating a 20 to 30 foot section of the PCP force main, cleaning of the exterior pipe barrel for inspection, installation of reinforcing tendons around the pipe exterior, backfilling the trench excavation, and clean up of the work area. Upon this site review and review of the submitted documentation as referenced above, this Department has determined to allow this emergency pipe repair work to be conducted as specified in the referenced documentation due to the emergency nature of the work. In addition, the following conditions must be adhered to as required under this Enforcement Order: ❖ Prior to the commencement of work activities, erosion controls consisting of staked haybales and trenched silt fence must be properly installed as shown on the plan submitted. In addition, trenched silt fence shall be installed along top of bank to protect the downgradient resource area and the bank itself and at the culvert outlet downgradient to the work. Upon installation of said erosion controls, the Conservation Department shall be contacted for a site inspection. ❖ Machinery or equipment shall be used along the banks at a minimum to limit disturbance. Bank Restoration Plan for Emergency Force Main Repair Work Based on today's site inspection and our recent discussion, it is our understanding that the Greater Lawrence Sanitary District (GLSD) has completed the emergency work which consisted of excavating a 20 to 30 foot section of the 72 -inch pre -stressed concrete cylinder pipe (PCC.P) sewer force main, cleaning of the exterior pipe barrel for inspection, installing reinforcing tendons around the pipe exterior, backfilling the trench excavation, and dean up of the work area. The GLSD and the contractor are in the process of restoring the temporarily disturbed channel bank and the temporary vehicle access way from the paved road. Please note, the following summary provides general guidelines for the emergency restoration work and are not the standard restoration details that are typically included in CDM's construction specifications. The emergency bank restoration plan was discussed with Ms. Alison McKay, North Andover Conservation Commission Administrator, on December 5 and 6, 2005 and was verbally approved by Ms. McKay during a telephone conversation on December 7, 2005 with CDM and GLSD. Upon completion of the site work, the temporary culvert will be removed and approximately 25 linear feet of bank will be restored to original contours to the extent possible. Bank stabilization will be accomplished by rip -rapping the bank from the toe of slope up to approximately 9/+ up the slope with placing 4- to 8 -inch stone because the slope is in places steeper than 2 -HAV. The remainder of the bank will be finish graded with soil and seeded with a MassHighway Department Mix for Slopes and Shoulders conforming with M6.03.0. This will improve the condition of the steep bank which previously consisted of exposed soil eroding into the waterway. Restoring the bank by installing riprap will provide the highest degree of protection against future scouring that typically occurs along this bank. Prior to seeding the slope, loam shall be placed along the slope and easement followed with applications of limestone and starter fertilizer. A bio -degradable erosion control blanket will be installed on the slope and adjacent GLSD easement after placing the grass seed mix and soil amendments to temporarily stabilize the soil until the seed germinates and develops sufficiently durable roots to hold the soil. The remaining bank will be planted with container - grown pussy willow (Salix discolor) (approximately 4 shrubs total) in a linear row. The proposed shrub layer is anticipated to provide shade and cover for wildlife and overhanging vegetation for wildlife perching and feeding. The shrubs will be planted approximately 5'on center to the extent possible. Container -grown stock shall have been grown in a container long enough for the root system to have developed sufficiently to hold its soil together, firm, and whole. The plantings shall be installed by cutting out a hole from the erosion control blanket to place the shrub root mass into the ground. All plants shall be dug and no heeled -in plants or plants from cold storage shall be used. When shrubs are set, loam shall be tamped lightly. Loam shall not be packed so firmly as to drive out all the fine air needed for a well aerated soil. Refer to the attached suggested documents including: a conceptual section for shrub planting locations; plant list, details for erosion control blanket, shrub planting; and a loaming and seeding specification. Due to the late season and time of year for planting, the recommended shrubs are not locally available at this time. We confirmed this with several local nurseries, who all recommended waiting until spring to obtain the recommended species. The request is considered an emergency since the condition of the weakened section of the sewer force main pipe would present an imminent threat to public and environmental health should the pipe fail and discharge raw sewage to the nearby intermittent stream, which is a tributary to the Merrimack River. Also submitted with that letter was a memorandum from Isabel Tourkantonis of Camp, Dresser & McKee (CDM) dated November 3, 2005 and a plan entitled "Water Pollution Abatement Facilities" dated January 1972, revised June 1976, which details the location of the work, the scope of work and environmental protection measures to be implemented for the work. On Tuesday, November 8, 2005, I conducted a site inspection with yourself and Ms. Tourkantonis of CDM to review the details of the proposed work and to determine further additional conditions necessary for work to proceed. The work will consist of excavating a 20 to 30 foot section of the PCP force main, cleaning of the exterior pipe barrel for inspection, installation of reinforcing tendons around the pipe exterior, backfilling the trench excavation, and clean up of the work area. Upon this site review and review of the submitted documentation as referenced above, this Department has determined to allow this emergency pipe repair work to be conducted as specified in the referenced documentation due to the emergency nature of the work. Tourkantonis Date: November 3, 2005 Subject: Environmental Protection for the Emergency Force Main Repair Work The Greater Lawrence Sanitary District (GLSD) is in the process of soliciting bids from contractors to immediately conduct an emergency repair to a portion of the 72 -inch pre- stressed concrete cylinder pipe (PCCP),sewer force main located within and adjacent to Riverview Street. The condition of the weakened section of the sewer force main would present an imminent public health and environmental hazard to the public should the pipeline fail and discharge raw sewage to th nearby�rote__-rmittent_stre._am�and ultimately to the Merrimack River. The emergency work will consist of excavating a 20 to 30 foot section of the PCCP force main, cleaning of the exterior pipe barrel for inspection, installation of reinforcing tendons around the pipe exterior, backfilling the trench excavation, and clean up of the work areDaThere is no other pipeline available to convey wastewater to the GLSD plant and thus this work needs to be completed expeditiously. Background Information PCCP is concrete pipe which uses a steel reinforcing wire (tendons) wrapped around the pipe to provide much of its strength. In early 2005, the GLSD installed an acoustic monitoring system along this section of the PCCP sewage force main. The acoustic monitoring system is capable of detecting wire breaks that may occur and the location of those breaks. To date, 14 wire breaks have been detected on this. section .of -the GLSD force main. On September 13, 2005 the GLSD conducted internal inspections of the 72 -inch force main. One of these inspections was conducted by Openaka Corporation, a consultant that specializes in the evaluation and rehabilitation of PCCP. On a letter dated September 20, 2005 the consultant stated the following concerns: "...the GLSD should be moving forward zvith repair of the pipe hollozv as soon as possible due to the potential for failure, especially during surge events. We are of the opinion that the pipe should be repaired as soon as possible to mitigate the potential for rupture. It is our recommendation that GLSD raise the issue zvith their Board this month so that they are azoare of the incipient failure risk and potentially design of a repair can be set in motion as quickly as Greater Lawrence Sanitary District November 3, 2005 Page 2 possible. We are of the opinion that repair of the pipe as an emergency measure is not unreasonable due to the potential impact of a pipe burst." The Massachusetts Division of Capital Asset Management issued an emergency waiver on October 31, 2005 from normal bidding procedures so that the proposed emergency repair can proceed in an expeditious manner. Existing Conditions On November 1, 2005, a CDM environmental scientist conducted a site inspection of the work area. The proposed work will take place off the southeast side of Riverview Street between How use No.49 and the existing 30 uicla_corugatedrnetal pipe (CMP) cum rt ­(refer to attached Sheet No. FM>3.9). The CMP culvert conveys flow to an intermittent stream that is tributary to the Merrimack River. There was low flow in the channel at the time of the field inspection. he banks along the channel on the eastern side of the road, between the railroad tracks and the road (where work is proposed) is primarily dominated by Japanese knotweed (Polygonum cuspidatum) and are eroded in various areas. A force main access manhole is visible on the southern bank near the railroad tracks. The banks of the intermittent stream on the downstream end of the culvert support red maple (Acer rubrum), American elm (Ulmus Americana), red oak Quercus rubra), highbush blueberry (Vaccinium conjmbosum), and Japanese knotweed. Flags 1-1 through 1-5 were placed to demarcate the To of Inland Bank defined by the mean annual high-water line. No work is proposed within the downstream west of Riverview Street and is 200 feet away from the work Proposed Work The force main and water main are located immediately adjacent to the southern bank of the channel. For the contractor to excavate the pipe within the narrow work area without compromising the structural stability of the force main and adjacent water main the culvert will need to be extended.anvroximately 3Q.deet to�the,east_within the limits ofthe exisfiii � � k channel banks and toward the railroad tracks. The entire 20 foot length of the force main will be excavated in sections to one foot below the bottom of the pipe for inspection and cleaning. Reinforcing tendons will be installed on the exterior of the pipe. Once the reinforcing tendons are installed and the structural stability of the force main has been confirmed, the trench will be backfilled. After repair work is completed, all fill will be removed from the top of the temporary culvert and from within the channel. The temporary culvert will be removed and the banks will be restored to original Greater Lawrence Sanitary District November 3, 2005 Page 3 contours. The banks will be seeded with-a.seed 'miirand stabilized with erosion control fabric. Thiswill improve the condition of the steep banks which are currently eroding. Additionally a tempos gray _L access way is proposed off the western side of Riverview Street to allow vehicle traffic to pass through the work area. The temporary access way will be approximately mately 2,250 square feet and will be located on an upland area between the two overhead utility poles and the edge of pavement. The gravel access way will be located southeast of the CMP culvert and will not extend into any wetland areas. Silt fence/haybales will be installed along the western edge of the road to prevent the transport of sediment to the intermittent stream. Sedimentation controls will also enclose the access way to demarcate the limit of work area. Sedimentation controls and the limits of work are sketched on the attached plan. The emergency work is anticipated to be completed within 7 to 10 days. The proposed work is limited to temporary disturbances and areas will be. restored to p er construction conditions. Disturbed area's as will be seeded and stabilized upon completion of wor a d sedimentate- ond erosion controls will not be removed until the exposed soils are stabilized. Mitigation Measures The following mitigation measures will be implemented during construction to avoid and minimize impacts to wetland areas. ■ Prior to commencement of construction, staked silt fence and/or hay bales will be installed at the limits of work along the pipeline route adjacent to resource areas to prevent the transport of sediment and debris to downgradient wetland resource areas during construction. The silt fence/hay bale barrier will be inspected weekly and after all storm events of a 1/2 -inch or more of rain and repaired as needed. The barrier will be left in place until the area is permanently stabilized. Hay bales will be replaced as necessary due to sediment build-up and degradation. I> ■ Worktinnand adjacent to wetland resource areas will proceed as rapidly as possible. fj�Limiting the exposure time of disturbed soils to wind and precipitation will minimize the soil erosion and subsequent sedimentation. ■Eb ill containment equipment (e.g., oil absorbent pads, oil absorbent materials, containment oms, shovels, etc.) will be stored onsite in an easily accessible manner for use in the anup of accidental releases of fuel or other hazardous substances. Post Construction Measures The following summary of mitigation measures will be implemented to restore any short- term impacts that occur in or adjacent to wetland resource areas. Greater Lawrence Sanitary District November 3, 2005 Page 4 ■ The disturbed areas will be restored to pre -construction conditions. Vegetated areas will be seeded with a perennial grass seed mix and maintained and reseeded as necessary until 80 percent cover is achieved, and in paved areas, the pavement will be repaired. ■ All disturbed soils in Inland Bank, Riverfront Area and the 100 -Foot Buffer Zone will be permanently stabilized to match pre -construction conditions to the extent possible. Seeded areas will be maintained and re -seeded to ensure that grass cover is adequate to stabilize the exposed soil. ■ The silt fence/hay bale barrier will not be removed until a vegetative cover dense enough to prevent erosion is established in the work area. Per our telephone conversation with Ms. Alison McKay, North Andover Conservation Commission Administrator, on November 1, 2005, we are scheduled to meet at 9AM on November 8, 2005 to conduct a site walk. We recommend that the GLSD request the North Andover Conservation Commission allow this important work to proceed in accordance with the Emergency Certification procedures found in 310 CMR 10.06. As noted previously, the weakened condition of the sewer force main presents an imminent public health and environmental hazard to the public. An Emergency Certification Form may be issued by a conservation commission under 310 CMR 10.06 for an emergency project relating to the protection of public health or safety. Attached for your information is a WPA Emergency Certification Form completed with applicant and project information pursuant to the Massachusetts Wetlands Protection Act M.G.L c. 131, § 40. Appeals The Department may, on its own motion or at the request of any person, review: an emergency certification issued by a conservation commission and any work permitted thereunder; a denial by a conservation commission of a request for emergency certification; or the failure by a conservation commission to act within 24 hours of a request for emergency certification. Such review shall not operate to stay the work permitted by the emergency certification unless the Department specifically so orders. The Department's review shall be conducted within seven days of: issuance by a conservation commission of the emergency certification; denial by a conservation commission of the emergency certification; or failure by a conservation commission to act within 24 hours of a request for emergency certification. If certification was improperly granted, or the work allowed thereunder is excessive or not required to protect the health and safety of citizens of the Commonwealth, the Department may revoke the emergency certification, condition the work permitted thereunder, or take such other action as it deems appropriate. Attached is a drawing that shows the location of the force main repair and environmental protection measures that we plan to implement during the pipe repair. The Contractor will be mobilizing to the site, installing the environmental protection measures and preparing the work area for the pipe repair during the week of November 21, 2005. Excavation of the force main pipe is scheduled to begin on November 28, 2005, weather permitting. We anticipate that the pipe work and clean-up will be completed by the end of the week. The Contractor will be working 24 hour days starting November 28 until the pipe has been repaired and backfilled. Final clean-up will then be completed during 8 hour day shifts. We request that the North Andover Conservation Commission allow this critical repair work to proceed in accordance with the Emergency Certification procedures found in 310 CMR 10.06.
