You are hereby required, in the name of The Commonwealth
of Massachusetts, to provide to the Civil Clerk's Office
Lawrence District Court on FEBRUARY 22, 1996 AT 9 A.M., any
and all Health Department records regarding inspections,
results and responses of the property addtess of 25-27
Clarendon Street, North Andover, Massachusetts for the period
February 1992 to February 1995. Specifically, it is requsted
that the Keeper of the Records provide any housing inspections
reports for this time period, any responses from the landlord,
Peter Lorentz or his agents, any documented action completed
as the result of Health Inspections and/or any actions taken
by the Department for noncompliance. This information is
requested for the trial in the case of Lorentz v. Cary, et al
Civil Action No. 9418CV1129 to be held on Thursday, February
22, 1996 in the Lawrence District Court at 9 A.M.
Hereof fail not, as you will answer your default under
the pains and penalties in the law in that behalf made and
provided.
