An exploratory excavation was done near the north foundations to locate an
electrical ductbank not shown on available drawings.

Concrete sampling and rebar inspections were performed for concrete pours in
metals bunker areas.
Daigle Engineers, Inc., of Methuen, the Independent Structural Engineer,
provided its approval of structural design packages prepared in support of

Weekly inspection of Erosion Control for the MRI -APC Retrofit Project in North Andover, MA- Haybales and Siltfence
are inspected on a weekly basis for cuts, rips, tears and basic deterioration. Haybales must be new, twine must be
wrapped tight and each haybale must be butted together tight and have two stakes in each one. Silt fence must be erect
and properly fastened to each stake. Row of stone in front of silt fence must be kept in good condition as to not let any
silt beneath the haybale or silt fence.
Erosion Control inspected on 5/14/99. All silt fence and haybales are in excellent condition. Row of stone is also in good
Condition.

Daigle Engineers Inc. of Methuen has nearly completed its first phase of work as
the Independent Structural Engineer. Additional design packages are being
prepared on MRI's behalf for Daigle's review.
V. Permit Compliance
All subcontractors receive site specific permit compliance orientation, including

requirements were developed as part of the local Project approval process.
In accordance with No. Andover Planning Board Site Plan Approval -Special
Condition 1.g., a truck turn warning sign was installed on Route 125 northbound.
In accordance with No. Andover Planning Board Site Plan Approval- Special
Condition Li., a Yield sign was installed for right turns from Holt Road onto
Route 125.

Building Commissioner (Mr. Nicetta) was onsite 4/27/99 for observations.
Construction trailer installations were reviewed with W -T.
Conservation Commission representative (Ms. Martin) was onsite to 4/29/99
review Resource Area compliance. She stated that she was satisfied with the
Project's compliance and noted that more than the required amount of erosion
control was installed. She provided helpful suggestions to further improve the silt
fence installations. A copy of MRI's document with guidelines for use of the
Resource Area was given to her for information/comment.

Robert Heitz of Water Safety Services, Inc., was on site 4/7/99 under contract to
the Town of North Andover to inspect the proposed modifications to the tie-in to
the City water system.

placing and compaction of fill material.
V. Permit Compliance
All subcontractors receive site specific permit compliance orientation, including
the Dust Control Plan and MRI Site Resource Area restrictions. These
requirements were developed as part of the local Project approval process.
Silt fences and hay bales were installed as one of the initial site activities.
Construction permits were requested from the Town of North Andover including:
building permit for trailers on site; installation of backflow preventors; project
construction signs; and relocation of 10" fire line.
