I understand and agree to the following obligations for management of this project:
1 . As the installer I am obligated to call for any and all inspections. If homeowner, contractor,
project manger, or any other person not associated with my company schedules an insp'ection
and the system is not ready then item two shall be applicable .
2. As the installer I am required to have the necessary work completed prior to the applicable
inspections as indicated below. I understand that requesting an inspection, without completion
of the items in accordance with Title 5 and the Board of Health Regulations may result in a
$50.00 fine being levied against my company.
a) Bottom of Bed — generally first inspection unless there is a retaining wall which should be done first. Installer
must request the inspection but does not have to be present
b) Final Inspection — Engineer must first do their inspection for elevations, ties, etc. As -built or verbal OK from
engineer must be submitted to BOK after which installer calls for inspection time. Installer must be present
for this inspection. With pump system all eiectrical work must be ready and able to cause pump to work and
alarm to ftmction.
c) Final Grade — Installer must request inspection when all grading is complete. Does not have to be on site.
3. As the installer I underst that persons or companies not associated with my company may
not perform the work required by my company to complete the installation of the system
identified in the attached application for installation. I further understand that work by others
unlicensed to install septic systems in North Andover can constitute reasons for denial of the
system, and/or revocation- or suspension of my license in the Town of North Andover plus
significant fines to all persons involved.
4. As the Installer I understand that I must be on site during the performance of the following
construction steps:
a) Determination that the proper elevation of the excavation has been reached.
b) Inspection of the sand and stone to be used.
c) Final inspection by Board of Health staff.
d) Installation of tank, D -box, pipes, stone, vent, pump chamber, retaining wall and other components.
5. As the installer I understand that I am solely responsible for the installation of the system as per
the approved plans. No instructions by the homeowner, general contractor, or any other persons
shall absolve me of this obligation.

Dear Sandra,
Because the proposed leaching beds for both lots are so close together, and the design
for both lots is shown on the plan and on the profile on both sets of designs, I have
chosen to address both lots in one letter.
Enclosed find the "Checklist for North Andover Septic System Plani" for the above-
mentioned site. The following is a list of all the 'Problem' areas and deficiencies Port
Engineering has found.
• The use of the reserve arm will require both raising the septic tank and building
sewer, or will require a pump.
The bottom of the proposed leaching fields are at elevation 148.5' for Lot A, and
144.50' for Lot B. The highest existing grade for the two reserve areas is about
elevation 150'. If we assume the ESHW is down 36" below the ground surface,
then the bottom of the field to be built in the reserve area would be 15 1'. Almost
certainly, the use of the reserve would require a pump for both Lots.
• As noted in General Note 10 on Sheet 3, the use of the reserve area would require
the construction. of a concrete retaining wall.
(In my opinion the above two issues can be resolved if trenches are used in place of
fields. The use of a trench design will allow the reserve areas to be between the
trenches. Thus the reserve and the trenches to be built would be right next to each
other, and the elevations would therefore be the same for both.)
There is more than 3' of fill over southerly half of the field on Lot B. 3 10 CMR
15.221(7).
As correctly shown on Section A -A, the fill required for the field on Lot A will fill over
the field on Lot B by more than 3'.
PoifT (This can be partially addressed by the following observation. The elevation of the

[Eflffi highest existing grade of the field on Lot A is about 147. TP 98-12-1 is closest to this
GIN high point, and if ESHW of 4' below the ground surface is used (as determined by TP
98-12-1) then the elevation of ESIM for the highest grade for Lot A is 143'. Thus the
Civil Engineers & bottom of field for Lot A would be 4' higher or elevation 147'. The proposed design
Land Surveyors has the bottom of field for Lot A to be 148.5'. Thus in my opinion it is possible to

A condition of approval should be that the benchmark would be set as per said Note 11.
• The pipe connecting the ends of the distribution lines must be solid pipe. NA 15.01
• On Section A -A the stone to be placed under and around the distribution lines of the
field is incorrectly shown as " r' of IX' — 3/8" washed stone ". It should be" 10"
of%" — 1 1/2" washed stone ". (Also the design calls for 4" of 1/8" to 3/9" washed
stone to be placed over the larger stone. The design regulations call for a thickness
of T" rather than 4". However I see no problem with putting down 4" instead of 2".)
The following are problems that are for only one of the two lots.
LOT A
• The proposed leaching field for Lot A is to be built on Lot B, with a "Grading and
Maintenance Easemenf' to be created. I could not find in the regulations any reason
why this cannot be done (the regulations only deal with shared systems). However
3 10 CMR 15.290(d) states in part a concern that "...each user ... has the legal
ability to accomplish any necessary maintenance, repair, or upgrade of the
component." Therefore I would suggest the following:
1. A copy of the proposed Grading and Maintenance Easement be forwarded to the
Board of Health for review that 3 10 CMR 15, 290(d) has been complied with.
2. A copy of the recorded easement once it has been recorded at the Registry of
Deeds (to be placed in the file).
3. A metes and bounds description should be created and a plan of the easement
with dimensioning should be recorded at the Registry of Deeds along with the
easement instrurnent.
• The grading shown on the plan around the septic tank for Lot A should be changed
to show that the final grade contour 152 should wrap around the tank (to have proper
cover as shown on the profile).
* For Lot A, the invert of the building sewer at the foundation should be raised to
elevation 151.25' (to maintain the minimum slope of 0.01 to the septic tank).
* A minor drafting error in the profile. The slope shown for the pipe running from the
foundation to the tank should be 0.01 and not 0.001 -

We are in receipt of your letter dated April 16, 1999. In response to the issues
raised in said letter:
1. Trenches were considered but rejected due to area limitations for the
following reasons:
a.) The break out requirements would require the trenches to start
approximately 301 from the southern lot line. The width of system including
reserve would be an additional 32'. With the required 101 separation for the
system on lot A, the total distance from the lot line would be approx. 72' to the
first trench on lot A. This would not leave enough area to the 100' wetland
setback for the system on lot B.
b.) Nearly twice as much of the lot on the south side of the proposed
house on lot B would need to be cut and filled.
c.) The higher water table in test pit 98-12-2 would require a pump
to be used in the primary system initially rather than added later should the
primary system fail.
2. The leach area on Lot B has been raised .881 to have maximum 3' of cover
3. Access covers are brought to within 6" of finish grade.
4. Benchmark is located in ridge on southeast side of leach areas.
5. Pipe connecting end of leach pipes has been specified.
6. Section A -A note has been changed.
Lot A Only:
7. Easement agreement will be filed before request for certificate of
compliance.
8. Slope of sewer line from building is 0.01
9. Profile is revised.

Lot A still has the following technical deficiencies:
• The leach field is missing a vent (needed because of the proposed pump system).
• Pump specifications are missing.
• Volume of effluent to be pumped and the number of cycles per day area missing (3 10 CMR
15.22(04)R.
• The "on" switch for the pump is shown to be located only 3" above the "off switch which
appears to be incorrect if 440 gallons are to be pumped in one dose.
The plans dated February 17, 2000 are approved for a dwelling with a maximum of nine
rooms.

We are in receipt of your letter dated May 18, 2000 in which you quote 15.241(d) stating the
vent is located incorrectly. The purpose of this vent is the venting of the pump not the leach field; it
has therefore been located at the highest point. Because the field does not exceed 50', it does not
require venting 15.251 (11). Locating the vent as designed relieves the pressure from the pump at the
d -box allowing for gravity feed of the leach field from the d -box. Placing the vent at the end of the
leach field will cause the system to be pressurized all the way to the end of the distribution line. Flow
through the d -box will thus be pressurized. If you still feel we should relocate the vent, we will do so.
Please advise.

Dear Sandra,
I previously had concerns about the proposed vent for Lot A Forest Street. My concern
was that if the vent is directly attached to the dbox then any jostling of the vent pipe
might torque the dbox out of level, affecting the distribution of effluent in the
distribution lines.
Phil Christiansen is the Professional Engineer who has stamped and signed the design
plans. He and I talked on the phone today. Phil stated that it is his professional opinion
that the vent can be constructed such that it will not affect the dbox integ6ty. Based on
Phil's professional experience and opinion, I do not have any further concerns about the
vent pipe being connected directly to the dbox.

I find that the revisions to the design dated March 20, 2000 adequately address the
regulations except for the following:

0 The proposed vent for the leaching bed is shown attached to the dbox, 3 10 CMR
15.241(d)states: "wheretrenches ... are used, the end of each distribution lateral
shall be connected to one or more vent(s)." Therefore I would suggest that the vent
be located at the end of the trench laterals. (I do not have to review this change
when it is made).
