As the installer I am obligated to call for any and all inspections. If homeowner, contractor, project manger, or any other person not associated with my company schedules an insp'ection and the system is not ready then item two shall be applicable . 2. As the installer I am required to have the necessary work completed prior to the applicable inspections as indicated below. I understand that requesting an inspection, without completion of the items in accordance with Title 5 and the Board of Health Regulations may result in a $50.00 fine being levied against my company. a) Bottom of Bed — generally first inspection unless there is a retaining wall which should be done first. Installer must request the inspection but does not have to be present b) Final Inspection — Engineer must first do their inspection for elevations, ties, etc. As -built or verbal OK from engineer must be submitted to BOK after which installer calls for inspection time. Installer must be present for this inspection. With pump system all eiectrical work must be ready and able to cause pump to work and alarm to ftmction. c) Final Grade — Installer must request inspection when all grading is complete. Does not have to be on site. 3. As the installer I underst that persons or companies not associated with my company may not perform the work required by my company to complete the installation of the system identified in the attached application for installation. I further understand that work by others unlicensed to install septic systems in North Andover can constitute reasons for denial of the system, and/or revocation- or suspension of my license in the Town of North Andover plus significant fines to all persons involved. 4. Enclosed find the "Checklist for North Andover Septic System Plani" for the above- mentioned site. The following is a list of all the 'Problem' areas and deficiencies Port Engineering has found. • The use of the reserve arm will require both raising the septic tank and building sewer, or will require a pump. The bottom of the proposed leaching fields are at elevation 148.5' for Lot A, and 144.50' for Lot B. The highest existing grade for the two reserve areas is about elevation 150'. If we assume the ESHW is down 36" below the ground surface, then the bottom of the field to be built in the reserve area would be 15 1'. Almost certainly, the use of the reserve would require a pump for both Lots. • As noted in General Note 10 on Sheet 3, the use of the reserve area would require the construction. of a concrete retaining wall. (In my opinion the above two issues can be resolved if trenches are used in place of fields. The use of a trench design will allow the reserve areas to be between the trenches. Thus the reserve and the trenches to be built would be right next to each other, and the elevations would therefore be the same for both.) There is more than 3' of fill over southerly half of the field on Lot B. 3 10 CMR 15.221(7). As correctly shown on Section A -A, the fill required for the field on Lot A will fill over the field on Lot B by more than 3'. PoifT (This can be partially addressed by the following observation. The elevation of the N[ED [Eflffi highest existing grade of the field on Lot A is about 147. TP 98-12-1 is closest to this GIN high point, and if ESHW of 4' below the ground surface is used (as determined by TP 98-12-1) then the elevation of ESIM for the highest grade for Lot A is 143'. Thus the Civil Engineers & bottom of field for Lot A would be 4' higher or elevation 147'. The proposed design Land Surveyors has the bottom of field for Lot A to be 148.5'. Thus in my opinion it is possible to One Harris S tree L lower the design elevations of the field on Lot A by 1. 5'. A condition of approval should be that the benchmark would be set as per said Note 11. • The pipe connecting the ends of the distribution lines must be solid pipe. NA 15.01 • On Section A -A the stone to be placed under and around the distribution lines of the field is incorrectly shown as " r' of IX' — 3/8" washed stone ". It should be" 10" of%" — 1 1/2" washed stone ". (Also the design calls for 4" of 1/8" to 3/9" washed stone to be placed over the larger stone. The design regulations call for a thickness of T" rather than 4". However I see no problem with putting down 4" instead of 2".) The following are problems that are for only one of the two lots. LOT A • The proposed leaching field for Lot A is to be built on Lot B, with a "Grading and Maintenance Easemenf' to be created. I could not find in the regulations any reason why this cannot be done (the regulations only deal with shared systems). However 3 10 CMR 15.290(d) states in part a concern that "...each user ... has the legal ability to accomplish any necessary maintenance, repair, or upgrade of the component." Therefore I would suggest the following: 1. A copy of the proposed Grading and Maintenance Easement be forwarded to the Board of Health for review that 3 10 CMR 15, 290(d) has been complied with. 2. A copy of the recorded easement once it has been recorded at the Registry of Deeds (to be placed in the file). 3.
