This very obese lady enjoyed fairly good health until January 1992 when she presented to a
Florida doctor with shortness of breath and was found to be in Atrial Fibrillation with a rapid
ventricular response. She was placed on digoxin and anticoagulated with coumadin. She did well
until December of 1994 when she had a stroke. Head CT scan confirmed a new infarct in the left
parietal area extending to the occipital region. Clinically she presented with aphasia and a visual field
defect.
Echocardiograms showed no valvular lesions and electrical cardioversion attempted in April
of 1992 was unsuccessful. She also is know to have hypercholesterolemia and glaucoma.
Medications include: Coumadin, Lanoxin and Timoptic eye drops.
Her stroke has rendered her incapable of handling her own affairs. She requires continuous
supervision and assistance.
Sincerely,

RE: Margo Riley
49 Dover Street apt 47
Somerville, MA 02144
DOB: 04-19-21
This very obese lady enjoyed fairly good health until January 1992 when she presented to a
Florida doctor with shortness of breath and was found to be in Atrial Fibrillation with a rapid
ventricular response. She was placed on digoxin and anticoagulated with coumadin. She did well
until December of 1994 when she had a stroke. Head CT scan confirmed a new infarct in the left
parietal. area extending to the occipital region. Clinically she presented with aphasia and a visual field
defect.
Echocardiograms, showed no valvular lesions and electrical cardioversion attempted in April
of 1992 was unsuccessful. She also is know to have hypercholesterolemia and glaucoma.
Medications include: Cournadin, Lanoxin and Timoptic eye drops.
Her stroke has rendered her incapable of handling her own affairs. She requires continuous
supervision and assistance.
Sincerely,
Albert H. Fine, M.D.
