Fine, M.D. Internal Medicine Services, Inc. 236 Highland Avenue Somerville, MA 02143 � (617) 628-9330 February 23, 1995 RE: Margo Riley 49 Dover Street apt 47 Somerville, MA 02144 DOB: 04-19-21 This very obese lady enjoyed fairly good health until January 1992 when she presented to a Florida doctor with shortness of breath and was found to be in Atrial Fibrillation with a rapid ventricular response. She was placed on digoxin and anticoagulated with coumadin. She did well until December of 1994 when she had a stroke. Head CT scan confirmed a new infarct in the left parietal area extending to the occipital region. Clinically she presented with aphasia and a visual field defect. Echocardiograms showed no valvular lesions and electrical cardioversion attempted in April of 1992 was unsuccessful. She also is know to have hypercholesterolemia and glaucoma. Medications include: Coumadin, Lanoxin and Timoptic eye drops. Her stroke has rendered her incapable of handling her own affairs. She requires continuous supervision and assistance. She was placed on digoxin and anticoagulated with coumadin. She did well until December of 1994 when she had a stroke. Head CT scan confirmed a new infarct in the left parietal. area extending to the occipital region. Clinically she presented with aphasia and a visual field defect. Echocardiograms, showed no valvular lesions and electrical cardioversion attempted in April of 1992 was unsuccessful. She also is know to have hypercholesterolemia and glaucoma. Medications include: Cournadin, Lanoxin and Timoptic eye drops. Her stroke has rendered her incapable of handling her own affairs. She requires continuous supervision and assistance.
