RE: 301 Johnson Street - Repair
Dear Charlie:
I am corresponding as a follow-up to the repair of the
septic system at 301 Johnson Street. The construction of the new
septic system was conducted to this department's satisfaction as
of my last inspection, however, the sinks in the basements were
not connected to the new system as discussed prior to
construction of the new system. I do not know if these sinks
have been connected to the new system. If the sinks have not
been connected, they should be connected as soon as possible.
I would appreciate it if you could inform me relative to the
status of the connections. Thank you for your anticipated
cooperation in this matter.

RE: Septic Failure Charles Salisbury
#301 Johnson Street - North Andover
Dear Mr. Rosati :
On August 13 , 1991 , in your presence we performed a site
inspection at the subject location. This inspection was in
response to complaints by Mr. Salisbury , the owner of the
property , regarding the functional condition of the existing
subsurface disposal system on the site.
Our inspection involved consultation with the owner regarding
specifics , such as age , location , type , failure patterns , and
pumping requirements of the existing system. Also a backhoe was
present to excavate portions of the existing facility for our
inspection as well as to excavate deep observation holes and
percolation test holes so that we could evaluate the on- site soil
conditions for their suitability in supporting a replacement
subsurface disposal system.
Our observations of the existing facility indicate that it is
comprised of a concrete septic tank (approximately 750 gallon
capacity ) which appears to be deficient of inlet and outlet tees
due to corrosion of supporting steel supports . The effluent
exiting the septic tank then passes into a leaching trench or bed
running parallel to the existing tree line on the down hill
( northwesterly ) side of the site . The relatively low elevation
of this area could be indicative of a rather high water table
elevation in this area as approximately 60 feet farther down
slope wetland vegetation and a water course tributary to Lake
Cochichewick exist. The combination of a high water table and
the existing leaching facility being, of course , below existing
grade indicate that a lack of a suitable soil receiving layer may
have contributed to failure of the existing system.
Due primarily to the existing wetlands off site and related 100
feet buffer zone it was determined that a total replacement
subsurface disposal system should be constructed in the higher
ground to the southeast of the existing facility . The replace-
ment system can be located over 100 feet distant from the
existing wetland to the northwest of the site .

Deep observation hole tests and percolation tests were conducted
in the proposed leaching site and favorable results were
obtained. The specific data is documented on the accompanying
sketch .
Based on the obtained soil data , existing topography , existing
house layout, and discussion with the owner, health agent, septic
installer and designer we recommend the following :
* Leaching trenches could be constructed to the rear and
roughly perpendicular and beginning approximately 50 feet
away from the existing building. On the southeasteerly side
of the top of slope of the ridge running southerly away from
the building.
* Size and capacity based on a four bedroom flow ( providing
for a future one bedroom addition ) and percolation rates
obtained are indicated on the accompaning sketch .
* The elevation of the trenches should be based on keeping the
top of the proposed peastone (1 /8 "-3/8 " ) approximately two
feet below grade ( at the lowest point of the existing grade
along the proposed trenches ) . Using trenches with an
effective invert depth of twelve inches at the end of the
distribution line would in turn , mean that the bottom of
trench would be 3 . 5 feet below grade .
* A new 1500 gallon concrete septic tank should be installed
approximately 30 feet away from the existing foundation to
allow same for a possible future building addition.
The elevation of the septic tank should be kept at a minimum
elevation as possible to provide adequate slope for down
stream piping , while keeping the building sewer invert low
enough to allow for internal plumbing connections .

or if you feel it proper for us to pursue in this direction with
further plans and specifications .

Last pumped out May 16, 1977 Pumped out once every three years.

Original tank was metal and was put in when the house was built in
1951 . This disintegrated about ten years later and was replaced
by a concrete larger tank. Because of detergents which were not
biodegradable there had been some trouble with the distribution box
and and leaching field. These were relocated abg t fifteen ars
