One copy and proof of recording must be submitted with the building application Doc:Building Permit Revised 2014 • lam, _ FROM HOMES BY JEFF AND SONS 7 POWHATAN RD PEPPERELL MA 01463 TO; Michael Smolak 315 South Bradford St North Andover Ma 1) We will remove the existing greenhouse that measures 30 x 50'. 2) We will excavate a whole for a foundation which will be a 4 foot deep frost wall. The whole will measure 30 feet wide by 50 feet long. 3) We will install a foundation which will have a 20 inch footing 10 inches thick and a 10 inch thick wall 4 feet tall. We will then backfill the entire foundation grade and level. 4) We will excavate a trench from the two half baths continue through the front of the foundation and connect to the sewer system in the front of the old building. We will also excavate a trench for the water lines into the bathrooms. We will install all the piping for the sewer and for the water and backfill level and compact for a concrete floor. We will install 4 inch PVC pipe around the interior pp perimeter of the foundation for ventilation. We will then put 4 inches of crushed stone for a base for the concrete floor. 5) We will pour a 4 inch thick steel reinforced concrete floor within the foundation. 6) We will frame a structure 30' x 50' on the foundation. The walls will be 2 x 6 construction with a pressure-treated sill plate and double top plate and all walls will be framed with window and door openings as shown on plan provided by Giattino architect. The roof and ceiling will be trusses and they will be mechanically tied to the walls with hurricane ties. The walls will have half inch zip plywood installed and ring nails and taped. The roof will have one half inch zipplywood also ring nails and taped. The roof will have 6 feet of water and ice P Yw g p protection 8 inch white drip edge and 30 year architectural shingles with a vented ridge. 7) The exterior walls will be shipped lap barn boarded. Doors and windows will be trimmed by roughs on one by four Pine. 8) The doors will be Anderson French would double French doors. In the windows will be Harvey vinyl double hung windows. • FROM HOMES BY JEFF AND SONS 7 POWHATAN RD PEPPERELL MA 01463 9) On the interior we will frame for two half bathrooms. There is already an existing handicap bath in the old facility. In the half baths we will install toilet, pedestal sink, and electric hand dryer. The baths will also have a ventilation fan light combination with a motion sensor switch which will turn the light and fan on when anyone enters the room. 10)The interior walls will also be barn board. The ceiling will be v-groove pine. There will be four imitation beams from side to side on the ceiling constructed of hollow roughs sawn Pine. There will also be beams running up the ceiling at the same points and meeting at the top of the cathedral ceiling. Then we will have diagonal beans coming from the crossbeams up to the ceiling beam to form and imitation truss look. 11)The room will have a forced air heating system and air-conditioning system which will be installed by Morris heating and air. All propane connections will be done by a licensed plumber and pipefitter. 12)The electric will be performed by a licensed electrician. We will have recessed cans in the ceiling, wall sconces, and plug elements along the walls to code. The electrician will also wire the heating and air-conditioning units to code. 13)The concrete floor will be polished concrete or painted with polyurethane floor paint. 14)The front of the addition will have a 8 foot trellis extending from the building towards the street. 15)We will install a set of granite steps leading from the side of the building down towards the animal petting area. 16)We will build a small room off the back of the existing office on to the loading dock which we will enlarge to accommodate trucks entering from the back not the side.
