RAM activities are being initiated to address a historical release of mineral oil dielectric fluid (MODF) at the above -referenced location. The release is a result of a slow leak from a pad -mounted electrical transformer located approximately 20 feet northeast of the Merrimack College Athletics Complex building. This letter follows notification to the Massachusetts Department of Environmental Protection (DEP) on August 10, 2016. Release Tracking Number 3-33745 was assigned to this release. Please note that this notice is being provided to inform you of these events. No action is being asked of you in response to this notice. The focus of the proposed RAM is the removal and proper off -Site recycling/disposal of MODF- impacted media. In order to accomplish this goal, Coneco will direct the performance of limited soil removal activities and the assessment of post -excavation conditions in the release area, including the collection and laboratory analysis of soil samples. Field activities associated with the RAM are scheduled to commence on June 30, 2017, and are expected to take approximately one day to complete. This submittal is being made on behalf of the Massachusetts Electric Company d/b/a National Grid, the party conducting response actions relating to this release. Auuie's Pub This letter serves as your approval to construct the Auggie's Pub located in the Sakowich Building at Merrimack College. Please note that any changes made to the plan during the construction phase, must be approved through the North Andover Health Department. There are two final inspections that are required by the Health Department prior to the opening of the kitchen. The first is a "Final Construction". This inspection is performed after the bulk of construction is done. Equipment is in place, etc. This inspection is usually performed approximately 48 - 72 hours prior to the final food inspection. When ready for the final construction inspection, please call our office at the phone number listed below to schedule the first of the 2 inspections. The second inspection is a "Final Food". The final food inspection will take place 24 to 48 hours prior to opening. At the final food inspection, it is expected that the premises will be ready for business as follows. The establishment will be clean of all construction materials. 1) All hand sinks and bathrooms will be stocked with a wall mounted paper towel and soap dispensers 2) The ladies room will have a covered trash can for feminine item disposal 3) Bathrooms must have "employee must wash hands before returning to work" signage 4) Hand sinks should be labeled "hand wash only" 5) If potentially hazardous foods are being offered undercooked, such as hamburgers to order, a disclaimer regarding foodborne illness must be posted, as the code requires. 6) There must be test strips for the Chlorine sanitizer on site 7) There must be chlorine on site. 8) Gloves must be on site. Please note that the state does not recommend the use of latex gloves due to some person's sensitivity to latex that may cause them illness. 9) You must obtain copies of the state and federal food codes and keep them on premises 10) At minimum, employees should be trained on the sick policy and sanitation basics. In general, you must meet the state code requirements to be allowed to be open for business to the public including, but not limited to the list above. This is a Health Department plan approval only. Please be advised that other departments may have specific requirements. This approval does not supersede any other department's request regarding other town or state regulations. The language in bold is specific; please do not change it in any way. If you have one or more interior grease traps please note the plumbing code 248 CMR 10.09 (m): 1. A laminated sign shall be stenciled on or in the immediate area of the grease trap or interceptor in letters one -inch high. The sign shall state the following in exact language: IMPORTANT This grease trap/interceptor shall be inspected and thoroughly cleaned on a regular and frequent basis.
