Complaint- Dan Bonnell,978.891.0454,tenant at 34 Johnson St.calling to complain about rodents,black mold,roof leaks,water in basement.He has spoken to Ben Osgood and
the other trustees regarding these issues many times.He has also sent him numberous emails regarding the issues and still to this day nothing has been done.He is
currently having a hug ant infestation.He had an extermination come out to the house and they stated that there is an extreme issue regarding the pests and ants.He
has 2 kids ages 5 and 9.Would like someone from the Health Department to come out and look at the issues.
Complaintant withdrew complaint.The landlord is working on the issues.Case closed.

ORDER LETTER
An authorized inspection of 32 Johnson Street,was performed by Board of Health staff
on December 14, 2009 at which violations of 105 CMR 410.000 Chapter II of the State
Sanitary Code, Minimum Standards of Fitness for Human Habitation were found.
All violations must be corrected within seven (7) days of receipt of this Order Letter
or a professional contractor must be hired to evaluate the conditions noted below and
a signed contract for work must be submitted. If a contractor is hired all compliance
work must be completed within 30 days. A plan of corrective action should be
submitted to the BOH. Requests for extensions must be in writing and approved in
writing or the time table will remain as listed above.

.482 Owners hall provide, install, and maintain in operable condition ...carbon monoxide alarms
in every dwelling that is required in accordance with any provision of the Mass. General Laws
Please contact the North Andover Safety officer or a professional fire
safety consultant to be sure the unit is in compliance. Report findings to
the Health Office and add detectors as needed.
8) Electrical light fixture in basement hallway 410.256
utilizing a plug found in a basement ceiling light.
"No wiring shall ...extend through a doorway or other opening in a structural element....that
extension cords which connect portable electric appliances or fixtures to convenience outlets shall
not be considered temporary wiring."
Ensure this unit is compliant and install proper fixture and electrical
source as needed. The hallway must be lit, but the fixture must comply
with the electrical code.

Dear Ms Sawyer:
In response to your Order Letter dated December 16,2009,Center Realty Trust would like to offer the
attached pertinent information,and to make the following request regarding timing of repairs to 32
Johnson Street.
Attachments:
1. Summary Process Agreement for Judgment: The agreement for judgment entered into by
both the Trust and Caroline Cole, requires that she vacate 32 Johnson Street no later than
January 15, 2010
2. Invoice from Emergency Alarm Response System: This invoice dated October 14,2006 was
for the installation of smoke and carbon monoxide detectors in all units owned by Center
Realty Trust including 32 Johnson Street.
3. Three Invoices and Punch List from TGR Carpentry totaling$ 10,164.50: These invoices
were as a direct result of preparing 32 Johnson Street for the occupancy of Caroline Cole on
October 16,2007.
Center Realty Trust has contacted the firm of Doug Small Electrical Contractor,of North Andover to
correct the electric light fixture in the basement hallway at 32 Johnson Street as outlined in Paragraph 8
of the Order Letter(section 410.256). This work will be completed no later than December 22, 2009.
Additionally pursuant to Paragraph 7 of the Order Letter,(410.482)the Trust has contacted Emergency
Alarm Response System of Windham, NH to inspect 32 Johnson Street to insure that the proper number
of carbon monoxide and smoke alarms are installed and in proper working order. This work will also be
done no later than December 22,2009 provided that Ms Cole allows entry.
Center Realty Trust request that the other items listed as violations to 105 CMR 410.000 be allowed to
stand as is until Ms Cole vacates on or before January 15, 2010. The reason for this request is, none of
the other items listed are of life safety concerns, and significant work was done to 32 Johnson Street
prior to and during Ms Cole's occupancy. Center Realty Trust would propose that after vacancy,the unit
will be painted and all items listed will be corrected and subject to your inspection prior to any
occupancy.
