The project is located in Industrial -2 Zoning District, Map 78, Parcel 01, and consists of approximately 103,200 s.f. of a manufacturing facility with frontage on Holt Road, North Andover, MA. The Site Plan Review Special Permit was requested by Crusader Paper Company, Inc., 350 Holt Road, North Andover, MA. The property is the main manufacturing location for the Crusader Paper Company. The purpose of the proposed addition is to add 18,000 s.f. of additional storage space for the raw materials used at the plant. This application was filed with the Planning Board on March 2, 2007. The applicant submitted a complete application which was noticed and reviewed in accordance with Sections 8.3, 10.3, and 10.31 of the Town of North Andover Zoning Bylaw and MGL c.40A, sec. 9. The public hearing was opened on April 17, 2007 and closed on May 1, 2007. The Planning Board makes the following findings as required by the North Andover Zoning Bylaws Section 8.3 and 10.3: FINDINGS OF FACT: 1) The specific site is an appropriate location for the project as it is located in the Industrial 2 Zoning District and involves the construction of permitted uses within those zones. 2) Specifically, a Special Permit allows in the Industrial 2 District the following North Andover Zoning Bylaw, Section 4.133(11). Light manufacturing, including manufacturing, fabrication, processing, finishing, assembly, packing or treatment of articles or merchandise provided such uses are conducted solely within a building and further provided that such uses are not offensive noxious, detrimental, or dangerous to surrounding areas of the Town by reason of dust, smoke, fumes, odor, noise, vibration, light or other adverse environmental effect. 3) Further, the new addition will be located so as to obstruct access around the building. To compensate for this lack of access, the design includes a new access onto Holt Road connecting to the cul-de-sac. This new entry will provide for access around the entire building as required by the Fire Department. The construction of the addition will match the existing exterior style and will not require any additional signage or additional landscaping. The project calls for no additional parking to be constructed. The applicant has sought a Variance described in a separate petition currently pending before the Zoning Board of Appeals from Section 8, Paragraphs 8.1.2 and 8.1.6 for relief from the requirements of off-street parking. 4) There will be no nuisance or serious hazard to vehicles or pedestrians. 5) The site drainage system is designed in accordance with the Town Bylaw requirements and has been reviewed by the Town's outside consultant, Vanasse Hangen Brustlin, Inc. No significant changes are proposed for the site regarding drainage facilities or impervious areas. 6) The applicant has met the requirements of the Town for Site Plan Review as stated in Section 8.3 of the Zoning Bylaw. 7) Adequate and appropriate facilities will be provided for the proper operation of the proposed use. 2 350 Holt Road Site Plan Review Special Permit May 1, 2007 8) The project will provide for municipal water, sewer and utilities on the property to adequately address the needs of the project. 9) Final approval of this Site Plan Special Permit by the Planning Board is conditional upon a Notice of Intent to be issued by the Conservation Commission. 10) This Site Plan Review Special Permit is conditional upon final approval from the Zoning Board of Appeals for a dimensional Variance from the following Section 8, Paragraphs 8.1.2 and 8.1.6 of the Zoning Bylaw for relief from the requirements of off-street parking. Finally the Planning Board finds that this project generally complies with the Town of North Andover Zoning Bylaw requirements as listed in Section 8.3 but requires conditions in order to be fully in compliance. Written confirmation of said review and approval by the Department of Public Works must be submitted to the Planning Department. c) A bond in the amount of three thousand ($3,000) dollars must be posted for the purpose of insuring that a final as -built plan showing the location of all on-site utilities, structures, newly constructed roadways/access ways, topography, and drainage facilities is submitted. The bond is also in place to insure that the site is constructed in accordance with the approved plan. Plans and hydraulic calculations for each commercial system must also be supplied to the Building Department. 7) DURING CONSTRUCTION: a) During construction, the site must be kept clean and swept regularly throughout the construction process: Dust mitigation, dewatering and roadway cleaning must be performed weekly, or more frequently as dictated by site and weather conditions and as directed by the Town Planner. b) The developer will make every effort to locate stockpiles on the site away from roadways, access ways, and wetland resource areas. Any approved piles must remain covered at all times to minimize potential dust and be required to install appropriate erosion control measures to mitigate potential impacts to Lake Cochichewick. Certification that the systems have been installed properly in accordance with the above referenced regulations must be provided from both the North Andover Fire Department and the North Andover Building Department to the applicant. The applicant must then provide this certification to the North Andover Planning Department. d) Any and all signage plans for the site, which have been provided to the Planning Board during the site plan review process, were presented for the materials of the signage only. Any signage plans must conform to Section 6, Signage and Outdoor Lighting Regulations of the Town of North Andover Zoning Bylaw and must be approved by the Building Department. In no way is the applicant to construe that the Planning Board has reviewed and approved the signage plans for Zoning Compliance as that is the jurisdiction of the Building Department. All additional permits must be obtained by the applicant prior to construction. 9) PRIOR TO THE FINAL RELEASE OF ALL SECURITY AND ESCROWED FUNDS a) The Planning Staff shall review the site. The applicant requested and was granted a waiver from Section 8.3.5(e)(xv) Landscaping Plan. 11. All site work and external construction activity shall be limited to between 7:00 a.m. and 7:00 p.m. Monday through Friday and between 8:00 a.m. and 5:00 p.m. on Saturday. 12. (GST) is pleased to submit this report on the subject project. It consists of a, brief description of the project as: we understand it, subsurface data obtained at the project site, evaluation of the data, and foundation design recommendations developed based on this evaluation relative to the geotechnical requirements of the proposed addition to the existing Crusader raper Company facility in North Andover, .Massachusetts. In addition, we discuss those items, which have potentialsignificance, relative to the foundation construction. The work has been undertaken in accordance with our proposal letter dated October 29, 2007, and your subsequent, authorization. The content of this report is subject .to .the Limitations stated: in Appendix A. PROJECT UNDERSTANDING We understand that the proposed project cotsists of a siixgle-story addition, 80 ft by 225 ft (18;00.0 sq. ft) in footprint, to the north end of the existing single -story concrete -block building (100,000 sq., ft in footprint). We understand that the finished floor grade of the existing building is at. El. 45.42. The existing site topography (Ref. Drawing prepared by New England Engineering Services, Inc., ; ,M N. AndoverAS "Proposed Site Plan, 350 Holt Road, N. Andover, MA", dated March .2, 2007) at the site of the proposed addition gently slopes down from the north e of the existing building (El, 41, approximately) to further north. (EL 36; south sideof the existing Detention Pond). This observation is significant., because; if the proposed addition will also have its finished floor grade at El. 45.42, this would require site filling up to 6 ft to accommodate an earth -supported slab on=grade. This matter will. be discussed after .presentation of the subsurface data collected. at the site. We understand that. the proposed addition would be composed of a steel frame with metal facade. The test borings are designated as B=1: through B-4, and their loeation;s are marked in Figure 2. Borings were, advanced to depths ranging from 7 ft (B-2) to 22 ft (,134, B-3) utilizing a 2.75 -inch diameter (inside) continuous hollow -stem flight auger powered by an ATV (All Terrain Vehicle) rig. A Standard. Penetration Test (SPT) was performed (ASTM 'D 1586) and :a 1 -3/8 -inch diameter split -spoon soil sample was retrieved at the ground surface (0 to 2 ft),, and subsequently at 5 -ft vertical intervals at each test boring. Each soil sample retrieved was examined by the 0S1 engineer in the field and classified in accordance with the Burmister System. Characteristic portions of each soil sample were preserved in 4 -inch glass jars., and delivered to the GST Soils Testing Laboratory for confirmation of field classifications, possible laboratory index testing (if and as necessary), and 'temporary storage, Soil samples will be shipped. to .your designated address at your request. SPT data (N=Blow Counts < per 6-416h successive penetrations of the spoon) and the classifications of the soil samples are presented in a test boring log for each hole ( drilled and tested. These logs are presented in Appendix B. SUBSURFACE CONDITIONS Test boring data have disclosed that the site for the proposed addition is underlain by the following characteristic natural or man-made subsoil units from the ground surface. (existing grade) down: Asphalt Pavement exists along the driveway at the north end of the existin9.. building, which, will be removed for the proposed constriction. The existing driveway is approximately 15 ft wide, and the asphalt pavement is 4 -inches in thickness: Fill. soils were encountered within the upper zone of the soil. overburden within the area of the proposed addition in all four borings. Apparently the Fill soils had been placed in site grading associated with the, construction of the existing building, and also Fill soils inay have been used in developing perimeter containment: of the existing Detention Basin beyond the.. north end of the existing building. Based on available data, the Fill soils are about 7 ft in thickness with anticipated variations. The Fill soils consist of very loose to rrediuin dense Coarse to Fine Sand; some to little Gravel, little to. trace Silt. The Fill soils were encountered to be ranging from "moist" to. "wet", However; groundwater freely entering the drilled .hole was not encountered: in. this soil unit. Based on the SPT data, we understand that the Fill soi.Is were not placed in lifts with systematic compaction, thus they would be characterized as an "un -engineered fill", As such the Fill soils are not suitable to support. neither foundation support elements (footings) nor an on -grade floor slab. Glacial Till: Natural Glacial Till deposits underlie the Fill soils at the site of the proposed addition. Glacial Till is composed of unstratifed drift, including dense to very dense mix of Fine to Medium Sand, some to little Gravel, some to trace Silt. The natural moisture content of the Glacial Till deposits were reported to range from "moist" to "wet". Groundwater freely entering into :the drilled hole was not encountered within this soil unit. It appears that the Fill soils are essentially excavated, then re -worked (placed as fill) duringplacement as Fill soils, presumably to develop the existing site grading, and construction of the existing detention basin. Crusader Paper — North Andover, MA GSI Project No. 207339 December 26, 2007 Page 3 All borings were terminated in the Glacial. Tillunit, Frequent to occasional Cobbles and Boulders should be anticipated in the Glacial Till deposits. • Weathered Bedrock is anticipated to underlie the Glacial Till within proximity of the termination depths of the test borings. Since Bedrock was not cored during the explorations, this could not be confirmed, Groundwater freely entering into any of the drilled holes was not encountered. The soil overburden, however, including both the Fill soils and. the natural Glacial Till deposits exhibit a variable moisture content, which appears to be increasing with the increasing fines (Silt) content. We should also note that groundwater regime ata site a fluctuate seasonally with precipitation, snow -melt, temperature, and major environmental as well as roan -made effects, Thus, groundwater conditions during actual construction ri?ay not be similar to diose encountered .during the explorations. FOUNDATION DESIGN IMCOli�,NDATIUNS General Massachusetts State Building Code is the applicable "Code" for foundation design and construction of the proposed addition (structure). Foundation Support The -Fill soils are not suitable for foundation support, and they should be excavated and removed iri the footing areas within the "zone of influence" defined by the imaginary lines extending one foot (minimum) laterally beyond the perimeter of a footing, thenbreaking down at a slope of 1HAV and extending until it reaches to the underlying natural Glacial Till deposits. The zonae inside the imaginary lines is the "zone of influence" where it shall be constructed in compacted (95 percent compaction per ASTM D .1.557) in 94n.ch. thick lifts (loose lift thickness) of Structural Fill (see below). Actually, to accomplish. the construction. of the Structural Fill within the "zone of influence" a volume of fill.soils greater than the "zone of influence We recommend that the proposed structure be. supported on spread and/or wall (strip) footings bearing on the compacted Structural Fill. The spread footings, may be proportioned for an allowable design bearing stress of 2.5 tsf (tons per square foot), however, no footing should be smaller than 3 ft, by 3 ft.in plan area. Wall (strip) footings may be proportioned. for an allowable . design, bearing stress of 2.25 tsf, .however, the width of the wall footing should be at least 18 inches, If 2=f1: wide (minirriuin) wall footings had been adopted for the exisfng Building, we recommend that the wall footings for the proposed addition should also be 2 ft wide (minimum). The concept of "done of influence" discussed above does equally apply both to spread footings and the wall footings (see Figure 3), Exterior footings exposed 'to. ambient temperatures should be protected against potential frost action (i.e., initial heave:during freezing and subsequent settlement due to thawing of the ground). The .Code requires that exterior footings shall have a 4 -ft (minimum) embedment measured from the bottom of the footing to the finished (landscaped) grade. In the permanently heated interior areas, the embedment may be :reduced to 18 inches measured from the bottom of the footing to the top of the: floor slab. Crusader Paper North Andover, MA. GSI Project No. 207339 December 26, 2007 _ .-- -- Page 4 We understand that the proposed addition will abut to the existing building, and the same Finished Floor Elevation (FFE) for the slab will be maintained. We recommend that along the. contact with the existing building, the new footings of the proposed addition should be constructed at the same elevation with the existingfootings.: This would avoid any underpinning r- of the existing foundations. Floor Slab j We recommend that the ground floor slab be designed as an earth -supported slab -on -grade {{ bearing on compacted Structural: Fill. The Fill soils -Overlying the project :site are not support for direct support of thefloor slab and they should be excavated, removed and replaced with compacted. Structural :Fill as described below. Support requirements for the ground floor slab will depend on the "Design five ,Load" for the slab, This information is not available to us at the time this report has been prepared. Since the proposed facility will be. used by Crusader Paper Co., Inc., relatively high design live loads (e.g., paper storage) may be operational. The Finished Floor Elevation (FFE) for the floor slab is also not available. at this tinie.. If we assume that the proposed addition will have the same FFE (131. 45.42) as of the existing building,. this would .require significant filling (approximately 3 to over 8 ft) of the site to attain the design. grades. in the slab areas, we recommend that the top 2 ft (minimum) be excavated and removed, and the exposed subgrade in the granular, medium dense Fill soils be subjected to an aggressive subgrade preparation (proof -rolling). Subgrade preparation should be implemented by a large (10,000 lbs) self-propelled vibratory roller having four passes (minimum) iu two perpendicular • directions. Subgrade preparation for the slab areas should be completed prior to the construction of the footings under the supervision of a Geotechnical Engineer, who may require excavation and removal of the Fill soils to exceed 2 f3 in some areas(i.e., over -excavation may be recommended). Upon completion of subgrade preparation, Structural Fill shall be placed in compacted .(95 percent minimum.; ASTM .D 1557) lifts again under the supervision of a ( Geotechnical Engineer. Li structural design of the floor slab, a Modulus of Subgrade Reaction of 125 pci (pounds per square incl per inch) may be used, We anticipate that the thicknessof the compacted. Structural. Fill placedshould-be at least 2 ft in the floor slab area.. The above recommendations for the floor slab support should be reviewed by GSI upon availability of the. FFE and the Design Live Load for the slab. Post -Construction Settlements A rigorous settlement analysis could not be performed at this early phase of design since we do not have: the estimated design loads. The nature of the subsurfaceconditions underlying the project site would be considered "competent" associated with relatively small post -construction settlements. The relative magnitude would be controlled by the degree. of high-class earthwork accomplished., Where the recommended site work and subgrade preparation are achieved satisfactorily, and considering the recommended design bearing stresses above, we estimate that post -construction settlement at a particular footing location would not exceed one inch. We assume this would be an acceptable performance for the proposed addition. Crusader Paper North Andover, MA GSI Project No. 207339 December 26, 2007 Page 5 Foundation Drainage Based on the available subsurface data we do not. recommend any special foundation drainage elements for the proposed addition, If the existing (original building) has some foundation - - drainage. elements, these should be:implemented (extended) for the proposed addition, Requirements; of the Code for water -proofing and damp -proofing should be implemented, as a minimum. Seismic. (Geotechnical) Design Input In accordance with the Code (Section 1612,4:2), we have: established that the project site would be classed as Soil Profile Type S land a Site Coefficient 1.0. Site Coefficient is an indicator the degree of potential amplification of the seismically ,induced stress waves as they travel upward through the geological profile underlying the project site. Also, in accordance with the Code (Section 18053), we haveestablished that the project site as riot underlain by -subsurface conditions that are susceptible to earthquake -induced liquefaction. CONSTRUCTION CONSIDERATIONS Earthwork As described above; the proposed project will involve a relatively extensive earthwork, including full excavation and. removal of the Fill soils at the footing locations; partial excavation and removal of the Fill soils in the slab area; subgrad.e preparation; and filling to raise the existing grades. We anticipate 'that excavation for site preparation and footing excavation. could be accomplished by conventional equipment. We also anticipate that the required excavation can be performed as an open -cut with slope laybacks. The soils to be excavated would be classified as Type C per OSHA regulations, and they may stay stable at a cut (or fill) slope of 1.5 H;l V for a temporary period. Requirements for sub rade q g preparation havebeen described above. Special earthwork requirements may be necessary around: the proposed Detention. fond area. Structural. Fill shall consist of well -graded clean. Sand -Gravel ,mixes free of ices snow, organic matters and other deleterious substances. In addition the Sand -Gravel mix .shall meet the following particle size distribution (ASTM, 0 442): Sieve: Size Percent Passing by Weight 4 inch 100 No.. 4 30-80 No. 40 10-50 No. 200 040 Suitability of the excavated on=site soils (i.e., re -worked or natural Glacial Till) to be used as Structural Fill may be investigated systematically by the .Geotechnical Engineer during the early phase of construction. Due to site filling required, we anticipate that Structural Fill from borrow sources will be required. Crusader Paper = North Andover, MA GSI Project No. 207339 December 26: 2007 Page 6 Construction .Dewatering Eased on available subsurface (groundwater) data, we do not anticipate a major groundwater control activity during the planned construction: Surface run-off should not be allowed to enter the excavation. We anticipate that measures needed for groundwater control during foundation. construction, which should be conducted "in -the -dry", would be achieved by providing collection sumps, and sump pumping approach.. Construction Monitoring _ We have indicated that the proposed project will require a relatively substantial earthwork; and we recommend, that the earthwork operations be monitored by a Geotechnical Engineer for optimizing (economy) and expediting the work. Thi"s is also a Code requirement. Specifically, We recommend that.you retain GSI toprovide the constructionmonitoring (earthwork QA/QC) so that our specific recommendations could be implemented as intended. Program for Structural Tests and Inspections (780 CMR 1705.3) 1. Structural tests and inspections of soils and foundation work arefor the purpose of providing assurance to the Owner, Building Official, and Structural Engineer of Record, that the construction complieswith the structural design components associated with .the work performed by the Geotechnical Engineer of Record (GE). These tests and inspections are for quality assurance audits and their implementation does not relieve the Contractor or Sub -Contractors of their responsibility, for quality control. of.the work and any design for which they are responsible. 2. The GE will direct the implementation of .this programand select any structural program.. inspectors required to undertake the 3.
