Please find attached the report I mentioned (Received: Monday, June 20, 2011 10:11 AM.) When I spoke to Mr. Larkin on Friday, he said he would forward it to me. All of the work that appears in this report occurred prior to Shawmut becoming agent for Meadowview. I have also included a copy of an email my maintenance supervisor Paul Letourneau sent to Mr. Larkin. It appears he had a prescheduled appointment for today at 11:00 that I was unaware of. I spoke to Paul and he says the tenant said he wanted to have the Health Department attend the meeting. Hi John Per our conversation about the moister inside your unit at Meadow View building 4 unit 2. You explained to me that you smelled mildew and seen evidence of moister seeping in from baseboard. I explained that I would like to see this unit again because back in April when I was it there was no evidence of this. I explained to you that I can set up an air quality test and if there is no mold spoor that are a health issue that the home owner will have to pay for this test. And if there is the association will pay and then the board will advise us to the next step. North Andover MA This email is to confirm that the above address was treated for mold on 6/8/07. Please see attached Certificate of Guarantee, on site photographs and general observations/comments below. Mold Remediation Treatment. Condo Unit 2 (Observations). • I tested the external wall surface with a calibrated Moisture Meter and on the front wall in the living room area; there was moisture present. A dehumidifier was in operation in the rear bedroom, by placing the dehumidifier in the front area this should address the wall moisture. • The water intrusion leading to the mold development is a result of negative grading and inadequate draining externally. Heavy rain water pouring from the roof will almost certainly result in similar flooding of Unit 2 if the problem is not rectified. • Looking at the building externally, rain from this roof deposits all its run-off directly onto the ground. As you can see from the photographs, a large percentage of this water runs back towards the corner of the building causing constant moisture on the outside of the building which can transfer to the inner walls causing mold. • It would seem, from my visual assessment the building externally that on the far comer of the building a similar situation may have occurred, however the condition has been addressed and a drain has been installed; it looks like a French drain ( I would have to see the drawings to be sure. • I have addressed the current mold problem, but until the external causative factor is rectified you and the Association will be dealing with further mold problems should a heavy rain storm occur. • I have taken a number photographs outside which clearly illustrate the problem. �_ � i �.. .__ ... ..._ _ �j, _..... .,7. .��. � ; i . �� � , �x� � �� F��p!� ; A � �7 �7 ` M�;'L � t R����`` �r ^iii' . . (L) Failure to install electrical, plumbing, heating and gas -burning facilities in accordance with accepted plumbing, heating, gas -fitting and electrical wiring standards or failure to maintain such facilities as are required by 105 CMR 410.351 and 410.352 so as to expose the occupant or anyone else to fire, burns, shock, accident or other danger or impairment to health or safety. (M) Any of the following conditions which remain uncorrected for a period of five or more days following the notice to or knowledge of the owner of said condition or conditions: (1) lack of a kitchen sink of sufficient size and capacity for washing dishes and kitchen utensils or lack of a stove and oven or any defect that renders either operable. (2) failure to provide a washbasin and a shower or bathtub as required in 105 CMR 410.150(A)(2) and 410.150(A)(3) and any defect which renders them inoperable. (3) any defect in the electrical, plumbing, or heating system which makes such system or any part thereof in violation of generally accepted plumbing heating, gas -fitting, or electrical wiring standards that do not create an immediate hazard. (w) failure to maintain a safe handrail or protective railing for every stairway, porch balcony, roof or similar place as required by 105 CMR 410.503(A) and 410.503(B). (5) failure to eliminate rodents, cockroaches, insect infestations and other pests as required by 105 CMR 410.550. (N) Amy other violation of Chapter II not enumerated in 105 CMR 410.750(A) through (M) shall be deemed to be a condition which may endanger or materially impair the health or safety and well-being of an occupant upon the failure of the owner to remedy said condition within the time so ordered by the board of health.
