RE: Title V review for 429 Salem Street,Lot C
Dear Sandra,
Enclosed find the"Checklist for North Andover Septic System Plans"for the above-
mentioned site. The following is a list of all the `Problem' areas and deficiencies Port
Engineering has found.
j 1. The reserve area must be located 4'off the primary leaching system. NA 9.04
2. Existing contours must be shown extending into the pavement of Salem Street to be
able to determine whether the 3:1 slope is in compliance. (The stone wall shown is
a retaining wall, and thus a 3:1 slope may encroach on the pavement of Salem
Street)
3. If the reserve area is used,an impervious barrier must be installed to keep the fill
required for the 3:1 slope from encroaching over Salem Street. North Andover
regulations require that impervious barriers be constructed of reinforced concrete,
which must be designed b professional engineer. At a minimum this

4. I believe there is a drafting error in the"Profile SepticSy
stern"S stem"detail. The bottom
of stone elevation is given as 196.10', 1 believe it should in fact be 199.10 .
5. The regulations state: "Trenches shall be used wherever possible."310 CMR
15.240(6). Reasons why a leaching field was used instead of trenches should be
given.
6. A note that the distribution lines of the field shall be connected with solid pipe
should appear on the profile. NA 15.01.
7. A note that the slope of the pipes in the leaching field shall be 0.005 should appear
on the profile. 310 CMR 15.251(9)(The system inverts were correctly designed
using this slope).
8. The vent detail is for a monitoring well and should be redesigned as just a vent(i.e.,
no perforated pipe running vertically below the leaching field). (Note that a vent is
not required for this design, however a vent may be advantageous to the proper

5. The"Profile Septic System" appears to have drafting errors. The inverts of the pipe
exiting the foundation, and entering and exiting the septic tank appear to be 1 foot
too high. I believe the invert at the building should be 208.05' (rather than 209.05),
the inlet of the septic tank should be 207.85 (rather than 208.85)and the outlet of
the septic tank should be 207.60(rather than 208.60).

A minor point is that on the site detail plan,the dimension between the foundation and
septic tank of 10' is missing.
