MEMORANDUM OF DATES
THIS MEMORANDUM is by and between NAOP, LLC ("Landlord") and Dr. Douglas
A. Wine ("Tenant") with respect to that certain Lease between Landlord and Tenant dated as of
June 16, 2016 for the Premises known and numbered as 451 Andover Street, Suite 190, North
Andover,MA 01845,(the"Lease").
Landlord and Tenant hereby confirm that the Delivery Date is July 10,2016 and that this
dates shall be conclusive for all purposes of the Lease.
Landlord and Tenant hereby confirm that the Lease Commencement Date is July 10,
2016 and that the Term shall expire on July 9, 2019, and that these dates shall be conclusive for
all purposes of the Lease.
Landlord and Tenant hereby confirm that the Rent Commencement Date is July 10,
2016 and that the Term shall expire on.Junee-3t,20T6,and that these dates shall be conclusive for

Tenant has prepaid Three Thousand Six Hundred and Fifty Five Dollars ($3,655.00) as
First Month's Rent at the time of Lease execution. Per this Agreement, Landlord shall issue a
one-time credit for the pro-rated First Month of the Lease in the amount of One Thousand Ninety
Six Dollars and Fifty Cents($1,096.50)which shall be applied to August 2016 rent payment.
It is further provided, that all other terms of the Lease and Extensions thereof, shall
remain as contained during this Extension Period.
This Agreement is contingent on Tenant maintaining complete confidentiality with
respect to all the terms and rates of this Agreement from all other parties including but not
limited to Employees, Tenants, Retailers and Brokers in the community. Landlord and Tenant
agreed that neither party is in default of the Lease as of the date of this writing.
IN WITNESS WHEREOF, Landlord and Tenant have executed this document as
of the first date set forth in the first paragraph above.
