The work that needs to be done should not take up to much time. The reason for the extension is to
ensure there is time enough to get the work done. Any time after that, the Health Dept. can appear in court to
file a complaint. After the 14th call me so as I can do a re-inspection. If at that time, all goes well, I will then
issue you an LOC - Letter of Compliance. j

house on 51 Maple Ave, but from our discussion, I did glean the
following issues that need remediation:

future rodent and insect infestation, but I was glad to hear that
you didn't find any evidence of current infestation
. The bathroom and living room radiators don't work
. There is debris blocking the back egress

. Several electrical plates are missing
Given that I haven't received the letter and that I am flying to
California tomorrow morning for the school vacation week with my
family, I am asking that you start the two week clock upon my
return, which is April 28.
I am copying my cousin on this note, and perhaps he can get a start
on some of the problems in my absence.
I am taking our conversation very seriously, and will do all I can to
have all of these issues remediated as expeditiously as possible.

ORDER LETTER
An authorized inspection of 51 Maple Street was performed by Board of Health staff on
April 2,2008 at which violations of 105 CMR 410.000 Chapter II of the State Sanitary Code,
Minimum Standards of Fitness for Human Habitation were found. Failure to respond within
the allotted time period may result in a Board of Health finding that the dwelling is unfit for
human habitation.
All violations must be corrected within seven (14) days of receipt of this Order Letter or a plan
for completion must be approved by this office if a professional contractor must be hired to do

April 2,2008 at which violations of 105 CMR 410.000 Chapter II of the State Sanitary Code,
Minimum Standards of Fitness for Human Habitation were found. Failure to respond within
the allotted time period may result in a Board of Health finding that the dwelling is unfit for
human habitation.
All violations must be corrected within seven (14) days of receipt of this Order Letter or a plan
for completion must be approved by this office if a professional contractor must be hired to do
the work. A confirmation from the contractor must be obtained at the receipt of this order
