On 4/15/99 a re-inspection of a rental property was conducted at the address
above. Present were the renter, Dan Dowd, the property owner, Carrie
Pszybysz and myself.
Initial discussion was held with the owner concerning the definition of a two
family dwelling and its relation to exterior snow removal. I identified that
disagreed with her definition which would exclude her from being a two family
because she is a home owner, however, I stated that I would contact the MA
State Health Department for clarification.
All parties were present when we entered the premises, 55 Park Street. My goal
of the inspection was to observe the corrected items and review the outstanding
ones. Concerning the discussion of some of the less hazardous items the owner
was combative and disruptive with her language. Stating that the renter
basically didn't deserve these items. For example, " then I will just take out the
refrigerator". As the tension between the parties rose I requested their
cooperation and suggested that I could leave and return with a police person at
a later time if that was needed. I then, on request by the owner, looked at an
exterior door that the renter had damaged and requested that he repair the door.
We continued upstairs to the bathroom and observed the temporary solution to
the shower stall problem. The owner became more agitated and the renter
stated that it may be better if the owner would just not speak to him so they
wouldn't get into an argument. The renter's actions somewhat instigated the
backlash from the owner. For example Mr. Dowd didn't like the two tone paint
repair job on the wall and stated so. Soon after, in anger, the owner said, "bite
me". Words and profanity began to be exchanged between the two parties. At
this time I stated that this inspection was over and that they would both be
receiving notices concerning the next Board of Health meeting.
As I left the premises the Mrs. Pszybysz wanted to know why I was leaving. I
stated that I was unable to do my job appropriately and turned to leave. In a
final gesture she stated, "fuck you". I said, " That's nice, have a nice day" and
left the site.

Dear Mr. &Mrs. Pszybysz:
On April 26, 1999 a follow-up inspection was done on the property at 55 Park Street,
rear. I noted that the screens on the second floor had been repaired and that the safety switch for
the heating unit had been repaired. There were still problems with the dishwasher not working
and the refrigerator light not lighting. In addition,the storm door in the kitchen will need some
weather-stripping or an extension at the bottom of the door since there is more than 1/8"of space
between the door and the sill. This is a violation of 105 CMR 410.501B4c. I spoke to your
tenant concerning directing the spray of the showerhead to avoid the problem of water on the
floor in the upstairs bathroom. I would suggest that you look into replacing the showerhead with
a water saving one. This type should decrease the amount of water passing through the head at
any one time and eliminate the problem.
Please call me at your earliest convenience before May 17, 1999, at 688-9540 to discuss
the remediation of the remaining violations.

Dear Mr. &Mrs. Pszybysz:
On May 17, 1999 a fallow-up inspection was done on the prope-Tty at 55 Park Street,rear.
All previously cited violations of the State Sanitary Code, Chapter II, Minimum Standards of
Fitness for Human Habitation have been corrected; except for 410.452—the safety hazard of
icing on the stairway of the exit. The design of the addition has resulted in a lack of appropriate
downspouts to collect and divert water away from the stairway. This probl�cm. must be addresscd
before November 15, 1999. Please call the Health office at 688-9540 to set up an appointment
for an inspection when the work is complete.
I also closely inspected the rear door for structural damage at the request of Mrs.
I szybysz. The wooden frame around the door was not damaged. The door itself is steel and has
only a primer coat of paint on it. Because of the caulking that the tenant used in order to keep out
winter drafts, some of the primer was removed. Mr. Dowd agreed to scrape and paint the door.
There was no structural damage to the door.

to correct these violations within the time allotted on the enclosed form. Failure
to comply within the allotted time period may result in a criminal complaint

1.What is the Blackboard Connect service?
The Blackboard Connect service allows authorized town leaders to create and rapidly disseminate time-sensitive
messages to every telephone number stored in the notification database. With the Blackboard Connect service,
authorized users can send thousands of messages in minutes. Only authorized officials are allowed access to the
system.
2. How does the service work?
Authorized officials compose and record a voice and'text message.That message is then delivered to individual
telephones, cell phones, and text-receiving devices in the notification database.
3.What types of messages will be sent using the service?
Any message regarding the safety or welfare of our community would be disseminated using the Blackboard
Connect service. Examples include severe weather warnings and updates, hazardous traffic or road conditions inside
the city or affecting local routes, and any other situation that could impact the safety, property,or welfare of our
citizens.

4. Is my telephone number included in the notification database?
It is our intention and hope that every residence and commercial facility in our community be included in the
notification database. For businesses,we only store one main phone number. For residents,we may have more
than one number that belongs to you in the database. Additionally,you may request to have a secondary number be
entered into our database for priority calling. Please click on the Blackboard Connect logo on this page to
register/sign up. Click"Add,my contact info"if it is your first time to the website. After that,you may return to the
website and edit your information as you see fit.
5. May I use a cell phone as my notification database listing?
Yes,we can accept cell phones as secondary phone numbers in the database and encourage you to request that
your number be included.
6.What precautions are being taken to protect personal information?
Blackboard Connect takes security and privacy concerns very seriously and does not sell,trade, lease or loan any
data about our clients to any third party. From a technical perspective,we utilize multiple physical and virtual layers
of firewalls to maintain data security. Blackboard Connect only utilizes secure transmissions with its customers. No
confidential information is ever transmitted between Blackboard Connect and its customers using e-mail or FTP, but
rather always utilizes either a VPN tunnel or SSL. Data is hosted in state-of-the-art facilities which require photo
identification,thumb-print recognition, keyed access, and are manned 24/7 with full-security personnel. All data is
encrypted prior to being placed on tape for offsite storage. Blackboard Connect also retains an external, independent
security firm to perform annual security audits.
7.Will the Blackboard Connect service work if I have a call screening system on my phone?
There are several varieties of call screening devices that use differing protocols for screening. In general,the system
has been found to work with these devices; but some may require some type of pre-programming to allow our town's
telephone number to pass through. We may conduct periodic test to assure that messages are being delivered to
numbers in the notification database.
8. If I am a non-resident home or condominium owner,what phone should be listed in the notification
database?
For non-residents or owners who reside out of our town,you may provide an additional phone number to be included
in our database to contact during certain situations. In general,calls are sent to the primary number only, but we
also have the ability to call multiple numbers for each resident or business when requested by the resident or
business.

9. If I have provided more than onehone number,when will the be called?

Should a situation arise that requires us to contact you at multiple phone numbers,we can activate the system to
place a simultaneous call to all of your numbers. In most cases,we will be sending calls only to one phone number.
10. My primary phone or my second listing is a cell phone with a non-local area code. Will the Blackboard
Connect service call numbers outside the area?
Yes. The area code does not impact whether or not a call is made.
11. How does the Blackboard Connect system respond to busy signals or no-answer situations?
For busy signals,the call will be repeated several times in an attempt to reach you. The same is true for No-answer
and Call-waiting. If a message recorder answers the phone,the message will be left on the answering device. If,
after several attempts the call does not successfully go through,the system will stop attempting to call.
12. 1 was not able to listen to the entire call. Is there a way to repeat the message?
Yes,at the end of the message playback, simply press the star(") key on your telephone to have it repeated in its
entirety.
13. 1 answer the phone but the"Hello"message repeats. It then hangs up,
calls back,and the same thing happens again. How can I hear the entire message?
Repeating or looping of messages happens when the system detects excessive noise in the background. Loud
radio/television volumes, people talking, or busy traffic noise,can cause this. When you receive the next call, say
"hello"once and turn down the volume of your radio/television or press the mute button on your telephone to allow
full message delivery.
