I, Patrick J. Slattery, Registration No. 4552 being a registered
professional architect, representing Patrick J. Slattery Architect,
hereby certify that I have reviewed, annotated and directly supervised
the preparation of all design plans, computations and specifications
concerning:

for the above named project and that, to the best of my knowledge, such
plans, computations and specifications shall the applicable provisions
of the Massachusetts State Bulding Code, all acceptable engineering
practices and all applicable laws and ordinances for the proposed use
and occupancy.
I further certify that I shall perform the necessary professional
services and be present on the construction site on a regular and
periodic basis to determine that the work is proceeding in accordance
with the documents approved for the building permit and shall be
responsible for the following as specified in Section 127 . 2 .2 :
1 . Review of shop drawings, samples and other materials of the
contractor as required by the construction contract documents as
submitted for building permit, and approval for conformance to the
design concept .
2 . Review and approval of the quality control procedures for all
code-required controlled materials .
3 . Special architectural or engineering professional inspection of
critical construction components requiring controlled materials or
construction specified in the accepted engineering practice standards .
Pursuant to Section 127 . 2 .3, I shall submit periodically, a progress
report together with pertinent comments to the local Building Official .
Upon completion of the work, I shall submit a final report as to the
satisfactory completion and readiness of the project- for occupancy.

I, Patric J. Slattery, being registered professional architect
certif that I have supervised the construction of a project
identified a "W.E.Aubuchon Co.In , 577 Chickering Road, North Andover,
MA in accord nce with the require ents of Chapter 127 of tyle Comm. of
Massachusett Building Code.
I have visited the project on five occasions throughoutithe
construction, to inspect progress and compliance and appro'e the work in
place.
Znspectior visits were made at various intervals commencing with
Project Start-up on June and at b -weekly intervals throug4out Julx
andAugust. I made a Final Walkt rough on August 29th at site with
W.E.Aubuchon Co. representative Ciristopher Slattery and cgnferred with
representatives of the w.E.Aubuch n Contractor, throughoutithe project,
to discuss d tails and progress. As of this date, I have Vund that all
new work and systems are in opera le condition and the cometed work is
able to sero the purposes intend d.
This cert' ficate is intended t cover those portions of ,the project
now complete . In accordance wit Chapter 127 of the Comms of
Massachusett Building Code I am ertifying the occupancy f the above
