This condition list shall be filed with the North Andover Conservation Department as NACC#

re-file with the required supporting documents and information.
OTA SMALL PROJECT-The North Andover Conservation Commission has determined that the
proposed project as submitted does not meet the small project criteria and hereby denies the application.
As such, a Request for Determination of Applicability(RDA)or a Notice of Intent(NOI)must be filed.

Dear Ms.Merrill,
This letter is written as a request for the conservation commission to make a determination as to a
small project exemption.I am in the process of obtaining the necessary approvals for a proposed second
floor and garage addition to my home located at 58 Appleton St.North Andover. I have received approval
from the zoning board of appeals with regard to a right side set back variance for the proposed garage,
which would be situated approximately 103' from any wetland. Wetland delineation and measurements
were performed by Richelle Denney of Arrow Environmental,LLC.
The area of the project that is the subject of a small project exemption request is an approximately
150 S.F. area of the home that is presently supported by concrete piers instead of foundation.This area lies
in the North West corner of the house and is part of the existing structure's footprint.There is no desire to
expand this area, it would be removed and reconstructed on a proper foundation.This area of the house was
measured to be approximately 67' from the delineated wetland to the West of the house. I would be willing
to incorporate erosion control,such as double staked hay bales, into the project near the site of the proposed
