After a permit application has been accepted by an Inspector of Wires appointed pursuant to M. G.L c. 166, § 32, an electrical permit shall be issued to the person, firm or corporation stated on the permit application. Such entity shall be responsible for the notification of completion of the work as required in M.G.L. c. 143, § 3L. Permits shall -be limited as to the time of.ongoing construction activity, and may be.deemed.by the7nspector_of-Wires abandoned-and-invalidif-he—_.. _ or she has determined that the authorized work has not commenced or has not progressed during the preceding 12 -month period. Upon written application, an extension of time for completion of work shall be permitted for reasonable cause. A permit shall be terminated upon the written request of either the owner or the installing entity stated on the permit application. C1 The Permit Extension Act was created by Section 173 of Chapter 240 of the Acts of 2010 and extended by Sections 74 and 75 of Chapter 238 of the Acts of 2012. The purpose of this act is to promote job growth and long-term economic recovery and the Permit Extension Act furthers this purpose by establishing an automatic four-year extension to certain permits and licenses concerning the use or development of real property. Landlord is responsible for maintaining the area free from chronic wetness, however the tenant must be responsible for organizing their belongings so the contractors can effect repairs. b. Many items wet and moldy. These items must be washed and dried or removed from the basement. Tenant must clean, remove and/ or organize all excessive items for basement floor. 3) Bathrooms a. Outer doorway — wood rotted, hole through floor b. Kitchen cabinets — old and in disrepair; i. Drawers broken ii. Undersink base water damaged and broken c. Pantry doors off tracks in disrepair d. Many painted areas chipping throughout kitchen. Doorways etc. e. Leaky garbage disposal fixed with duct tape Owner must have kitchen areas evaluated and at minimum remove all rotted wood and damaged cabinets. Paint all damaged #7 Elm Court 7/21/10 surfaces. Toilet seat broken - Owner must maintain structural elements so that all areas are in good repair for its intended use Owner must repair bathroom issues to comply with the code. All surfaces must be In good condition, paint in good condition, toilet in good condition, etc. c. Ventilation inadequate. 410.280 Owner must provide adequate ventilation to the outside with an air exchange rate of 5 x per hour as found in the code Install or repair proper ventilation of fan to the outside 5) Living Room 410.500 Owner must maintain structural elements so that all areas are in good repair for its intended use a. Holes in walls; living room and hall going up b. Front doorway damaged, gap around door c. Windows inoperable d. Screens in disrepair e. Air conditioner inappropriate for size of hole. The hole is not required, but if present must meet its purpose. f. Paint in disrepair Owner must repair/replace all windows, screens, door, walls etc. and ensure all structural elements free from defect, must be cleanable and weather tight. 6) Upstairs Front bedroom a. windows in disrepair and 410.501 not weather tight; frame pulls out easy, very loose, no screens - Owner responsible for maintain windows weather tight and safe Replace windows and screens in bedroom b. Wet floor — mold evident, insulation in ceiling wet, Piles of wet clothing tenant states was from past flooding during storm Conditions confirmed by observations listed above. This is a chronic problem - Owner must maintain basement free from chronic wetness Water source must be investigated by an expert in basement water problems. A report identifying the problems must be submitted to the health office and the infiltration problem must be addressed completely. Repair leak as needed NOTE: Basement drain hoses end close to building. May be related to water problems. Move farther out from foundation. b. Copper water pipe in corner by wash area has a leak if valve is turned on. - Owner must maintain all plumbing infrastructure 10) Tenant states the unit breakers constantly tripping due to possible electrical overload 410.351 - Owner must maintain electrical infrastructure in accordance with state Standards. Owner must hire a licensed electrician to ensure that the system circuits are capable of handling the electrical needs of the tenants. Electrician must submit findings to the Health Office or proof of corrections completed. Concerns passed on to other departments 11) CO detectors not found, contacted NA Fire 410.482 Dept. for follow up -owners shall provide, install , and maintain in operable condition smoke detectors and carbon monoxide alarms in accordance with the MA Gen. Laws etc. Comply with North Andover Fire Dept. direction 12) Also on July 20, upon concern the Health Dept contacted NAFD regarding hot water tank issues reported by the tenant. NAFD responded and did find the hot water heater leaking from the top. NAFD de -energized it [electrical] by shutting off the breaker and also shut off the cold water feed to the unit. It was noted that the discharge for the relief is facing up towards the ceiling and an electrical junction box. #7 Elm Court 7/21/10 Comply with Building codes as needed. NAFD responded and did find the hot water heater leaking from the top. NAFD de -energized it [electrical] by shutting off the breaker and also shut off the cold water feed to the unit. What was discovered that the tenant had plugged the air conditioner into the general purpose living room circuit rather than the dedicated air conditioner outlet. The tenant's air conditioner was not compatible with the dedicated air conditioner outlet. We replaced the air conditioner outlet so that the air conditioner would be powered by its separate circuit. On July 26, 2010, we investigated a complaint that the bathroom ventilation was not working. We discovered the first floor bath fan was not working, we ordered a new motor and blower wheel and replaced these items on July 27. Both the first floor and second floor bath fans exhaust at a rate of 50 CFM The first floor bath's dimensions are 5 feet wide, 6 feet deep and 8 feet high. The cubic footage of the first floor bath is therefore 240 cubic feet. At a rate of 50 CFM the air changes per hour is 12.5. The second floor bath's dimensions are 5 feet wide, 10 feet deep and 8 feet high. The cubic footage of the second floor bath is 400 cubic feet. Dependent on tenant completing their obligations in the basement a. Wet floor — mold evident, insulation in ceiling wet, Piles of wet clothing tenant states was from past flooding during storm Conditions confirmed by observations listed above. This is a chronic problem - Owner must maintain basement free from chronic wetness Water source must be investigated by an expert in basement water problems. A report identifying the problems must be submitted to the health office and the infiltration problem must be addressed completely. Owner must hire a licensed electrician to ensure that the system circuits are capable of handling the electrical needs of the tenants. Not a health code issue contacted NAFD regarding hot water tank issues reported by the tenant. NAFD responded and did find the hot water heater leaking from the top. NAFD de -energized it [electrical] by shutting off the breaker and also shut off the cold water feed to the unit. It was noted that the discharge for the relief is facing up towards the ceiling and an electrical junction box. Comply with Building codes as needed.
