The Health Department has received your request of November 19, 2010. This would be the
second type of extension in this matter. The first was allowed until October 24, 2010.
After several attempts to set up a time with the tenant, due to a change in phone contact numbers,
a re -inspection of the premises was not conducted until November 17, 2010. At that inspection
the majority of violations had been addressed. Please see the attached copy of the Order Letter
with the notations regarding re -inspection status.
You have requested to have compliance to the final items delayed until December 15th, as this
should allow the time for the tenant to vacate and you to finish the corrective action. As I
personally observed that the tenant is almost complete in vacating the premise, there is no
imminent need to complete these items nor is there a need to set a specific time. Please contact
me when all items are complete. Be advised that renting any unit with known violations is a
violation to the code. You must contact the Health Office and have the inspection verify
corrective action is completed.

on July 28, 2010 at which violations of 105 CMR 410.550 Chapter II of the State
Sanitary Code, Minimum Standards of Fitness for Human Habitation were found.

The Health Department has received your request of September 24, 2010. In response to your
questions a second onsite inspection was completed on Friday, October 1, 2010. Please note the
following:
1) Extension - As requested, you are granted a 30 day extension from the date of receipt,
September 24, 2010.
2) The walls of the over sized bathroom — please note that all portions of the wall that are
covered by the painted wainscoting will be considered in compliance as long as the paint
stays in good condition and the walls remains non porous. All other areas must install
absorbent covering ie. tile or other, up to 48 inches throughout the entire bathroom.
Around the tub/shower area, per code, it must be to a height of 6 feet with a non porous
covering.
In addition, please note that during the inspection a few plumbing issues, not previously seen in
August, were observed. The most important concern was that the water in the tub was running
approximately a gallon a minute with no ability to turn off the faucet. I contacted you personally
and informed you of the issues and you indicated that you would send a plumber over that day.
Thank you for your attention to the matter.
If you have not done so already please contact the fire department to confirm the existence or
non existence of issues with the current fire detection system.

above on September 14, 2010, it appears that this document was provided to you at the Lawrence
District Housing Court on Thursday, September 9, 2010.
I left Mr. Robinson a phone message today and requested that you contact the Health Department
to discuss this issue. If you have done so and an agreement of extension has been granted before
receiving this letter, please disregard the following.
If you have not done so, please be advised that this Order Letter is now considered legally
delivered and the Health Department will be submitting a complaint to the Lawrence District
Housing Court in the near future.
Failure to comply within the specified time period may result in further action by the North
