Re: Pest Control Issues — BEDBUGS - Building 41, Royal Crest Drive North Andover, MA 01845
Dear Residents:
The North Andover Health Department is aware of the Bedbug issue in Building 41 at Royal
Crest Estates. AIMCO (Apartment Investment & Management Co.), the parent company for
Royal Crest Estates, has engaged F&W Pest Control, Inc. to implement an assault on this
infestation. Although there are numerous methods that have shown to be successful in the
eradication of a bedbug infestation, F&W will be utilizing federally approved pesticides.
AIMCO will be distributing information regarding the proper methods for residents to follow in
an attempt to avoid the re -infestation of bedbugs and reach a successful extermination of the
Bedbugs.
Please follow the protocol as instructed by AIMCO's pest control company, F&W. The Health
Department understands that this is a great inconvenience to you all; however the pest control
process requires all residents' cooperation. Thank you for the anticipated cooperation.

Hi Michele,
I just took a call from Karl Lammi, the Service Manager for F&zW Pest Control out of Wrentham. He relayed
the following:
F&rW was contracted by AIMCO/Royal Crest to treat Building 41- Unit 1 for bedbugs.
Evidence of bedbugs was found in the following areas: Evidence of droppings and eggs - couch;
recliner; main bedroom mattress and boxspring, as well as bedroom ceiling - under molding between
ceiling and wall - evidence of bugs.
The tenants in Bldg. 41, Unit 1 evidently had vacuumed up all of the "live" bugs, and F&rW only saw
evidence (droppings) and eggs from the actual bugs, and treated accordingly with their standard
method as outlined on the invoice.
Also, when they do inspections, it is strictly visual - using their eyes and a flashlight - they remove matress
pads and sheets, fold and put on floor, examine mattress, underneath mattress, top of boxspring and all around
the bedfram and boards. They are looking for visible droppings, eggs or live bugs. F&rW also did visual
inspections of units 2,3, and 5 within the same building, and found no additional evidence of bed bugs.
Karl mentioned that Nathan of AIMCO/RC wanted F&rW to send a letter outlining all this, but Karl just
wanted to run it by you first. If you would like an official letter, he can do that. If any more questions, please

As we discussed earlier, I am attaching the invoice and inspection forms for the affected and neighboring units. 41-1 was
treated for bedbugs and all surrounding units were inspected — the inspections were negative.
To be proactive, we will be reinspecting the neighboring units along with all other units in the building that we have not yet
been to. This inspection will take place on Friday afternoon.

Good Afternoon Michelle and Susan,
Attached you will find the F&W bed bug inspection for'all 11 apartments in building 41. There was no activity found;
however, as Michelle and I discussed today we will be treating apartments 2-4 and #5 next week. I have left a message
for Jim Merrill to set up a meeting Monday or Tuesday so I can get the date of re -treatment for 41-001. In addition, I let
him know that I need for him to come out and perform an inspection and give recommendations for 41-003 in terms of
treatment for the mice and identifying the areas to seal. Finally, the residents in 37-004 are in a hotel until next Saturday,
august 13th. Based on what I know so far it will take approximately 10 days to complete this so we should be on target
with getting them back in the unit. They refused a transfer, but eventually accepted the hotel stay. I left a message for
Brittany just a little while ago to touch base and see if there was anything she needed. I will update as we go through
this. Please let me know if I missed anything. Thanks.

Tenant was called and left a message on Friday, August 12, 2011 in the morning, stating the Health Dept. needed to do
an inspection to confirm the corrective action.
Bldg 41, common areas and exterior areas were observed. All entry holes were filled that were possible to do so using
various methods; covering, foam and steel wool. Entire building to be visited and baited with weekly monitoring.
Bldg 37, #4 — was observed mid -construction. Removal of sheetrock was done throughout in all areas up to approx. 4
feet. Ceilings as needed. Proper cleaning and sanitizing process was used on beams behind the sheetrock. Anticipated to
be complete and ready for final inspection on 8/19/11. Tenant being relocated.
Agreed to hold final letters owed to the Health Department to 8/19/11 after follow up visit to check for pest and
bedbug activity..
Also, we received a distress complaint needing immediate attention on 8/10/11 Tenant moving to Bldg 35, #9 from Bldg
49, #2. Tenant distressed about mold that is on her mattress, which has been discovered in the move. Mark Johnson
personally handled the problem within 1 hour of the Health Dept receiving the complaint. On 8/11/11, 1 personally went
to unit being vacated. The stench was intense. It was not only mold; but was mixed with other foul odors urine, dirt etc.
Walls had food stains. Entire unit obviously not been cleaned in months. Rugs filthy and dust level very high on heaters
and window sills. This unit will need serious attention prior to renting. Observed the mold on the mattress in addition to
the numerous old urine and other wet stains on the mattress.
Also informed you of another inspection that was conducted on Thursday morning. This inspection did not require an
inspection report or Order Letter, but the claims were mold related. No chronic water found. Mark was notified via
email, so is aware they may have a hidden problem in the apartment. This was in Bldg 25.

An inspection for bed bugs was performed on August 5, 2011 at Royal Crest
Estates in North Andover, building 41, this excluded unit #1 (which was treated
on August 2nd and is scheduled for a follow-up visit on August 19th) and units 2
and 10 who denied us access. No evidence of bed bugs was found at this time.
Our recommendation is to continue with the scheduled follow-up visit as
scheduled; and, at the request of the Board of Health, treat the unit directly
above and unit #2 which is adjacent to the infested unit

F&W'S MONEY -BACK GUARANTEE
if, during the regularly scheduled visits, the Customer Is not satisfied with the
results, F&W will return to treat the area again at no extra charge. Such service
visits shall be made promptly when requested by the Customer. Should the pro-
lem persist, F&W will continue to work to solve the problem to the Customers
satisfaction or F&W will refund the Customer's last monthly payment.
CUSTOMER COOPERATION
The Customer agrees to cooperate fully with F&W to insure effective results
from the Past Control service. In order to qualify for the Money -Back Guarantee,
the Customer agrees to maintain its premises as recommended by F&W in
the Housekeeping and Maintenance Survey Report originally provided to
Customer and as recommended by F&W in its Pest Control Service Reports.
The Customer agrees that F&W may withhold any and all refunds until its
recommendations have been accomplished.
PEST DAMAGE
The Customer agrees that F&W is not responsible for insect or rodent damage
or disruption to its operation, premises or its contents, and the Customer
specifically releases F&W from liability for any such claims.
DOCUMENTATION
Upon request, F&W shall fumish to the Customer a certificate of insurance cov-
erage in effect and any Material Safety Data Sheets (MSDS) or Labels request-
ed.
MATERIALS
The materials used to control pests in and around Customers premises shall
conform to Federal, State, and local laws and regulations, and shall be used
with all due precautions to avoid the possibility, of accident to humans, domestic
animals and pets.
TERM OF CONTRACT
This contract shall be effective for an original period of ' 3 'r` , Thereafter,

1. REDucE CLUTTER (i.e. Throw out shoeboxes,
newspapers, etc.)
2. Remove all bedding from all beds (sheets, blankets,
comforters, pillows) and place them DIREC into a
plastic bag (do not throw them on the floor, then put them
into a bag, as this will only spread them further) seal bag
tightly.
3. Remove all items from under bed, these items should
be inspected for bed bugs, cleaned and placed in sealed
plastic trash bags.
4. Vacuum bed frames, box springs and mattresses
(pay special attention to the edges, buttons and tufts).
Also vacuum the floors, rugs and around baseboards. All
shoes must be vacuumed as well.
5. Empty all bedside tables, closets, dressers,
bookshelves, wall units, hutches, etc, throughout the
residence, including platform bed drawers (if any). All
items that have been removed from these areas should
be inspected, cleaned and put into plastic bags then
sealed tightly. All of the above mentioned areas must be
vacuumed once they have been emptied to help eliminate
any live bedbugs or eggs that may be present.
6. Go through all items within ten (10) feet of bed. After
inspection, place items in a tightly sealed trash bag.
7. Remove all curtains from windows, place these
directly into a plastic and seal tightly.
8. Vacuum floors.
9. Once all vacuuming is done, place vacuum bag in
a sealed plastic bag and discard outside for garbage
removal.
10. All linens, curtains and clothing must be machine
washed in HOT WATER and dried on high heat in the
clothes dryer.
11. After washing, place in new plastic bags (seal them
tightly). Discard old plastic bags outside for garbage
removal, do not re -use.
12. Leave all clothing in bags until the area is determined
to be pest free. We recommend this until after second
scheduled treatment.
13. ALL BAGGED ITEMS SHOULD BE REMOVED FROM ROOMS To
BE TREATED.
14. You must keep all areas such as bedside tables,
closets, dressers, bookshelves, wall units, hutches,
etc. Empty in preparation for the follow-up treatment
approximately 30 days after the original treatment.
15. Pregnant or nursing women must consult their
physician regarding this service, prior to the application.
We recommend being out of the home for a minimum of
twenty four (24) hours.
16. Toddlers under the age of one (1) year old must be
out of the home for a minimum of twenty four (24) hours.
Anyone with an asthmatic condition must stay out of the
home for a minimum of forty eight (48) hours.
17. Remove any children's toys that are out in the open.
18. The home must be vacant for a minimum of four
(4) hours following the treatment or until application is
completely dry.
19. All pets must be out of the home for at least six (6)
hours. Birds must be out of the home for twenty four (24)
hours. Fish tanks and air pumps must be covered tightly
with plastic and the filter must be turned off.
20. Remove any pet food that is out in the open
21. After bedbug treatment has been provided, we
strongly recommend that vinyl mattress and box spring
covers be used on all mattresses and box springs to
avoid contact with the products used in the application

As we discussed earlier, I am attaching the invoice and inspection forms for the affected and neighboring units. 41-1 was
treated for bedbugs and all surrounding units were inspected — the inspections were negative.
To be proactive, we will be reinspecting the neighboring units along with all other units in the building that we have not yet
been to. This inspection will take place on Friday afternoon.

Thank you Michelle. I have advised Deana Susko and she will personally get involved and relay information and the plan
of action back to me. She is also contacting the pest control company to get copies of the written documentation on the
apts they inspected, treated, etc. She will have all apts and common areas in the building inspected as a matter of
precaution and identifying if the problem is bigger than originally believed.

More thoughts.
As a follow up to our phone call moments ago regarding Building 41, Apt. 1. As I have not been able to contact
the renters. I wanted to relay other concerns. There are 4 people living there. Husband-Damion, Wife —Jamie and 2
small children. This family has not been asked to throw anything away. All furniture is still in the apartment etc.
The other renters have been told that they can use the washers and dryers without risk.
