As we understand it, there are some locations on the above -referenced project that required the
use of back-to-back outlet boxes in the walls due to the spacing of the studs. For those locations,
we recommend the use of a moldable putty pad to protect the boxes. The fire barrier product, as
manufactured by 3M.and distributed by Fire Retardants Inc., is an acceptable product to use in
this application, provided the boxes are completely wrapped as indicated in the installation
diagrams.

Hello Erik,
Per your request, we visited the above referenced site this past Thursday, March 2e, to perform
structural observations in accordance with the requirements set forth in the Massachusetts
Building Code 780 CMR, between the hours of 2:00 p.m. and 3:45 p.m. The temperature was
approximately 50 degrees and it was cloudy. Wally Ryan, project superintendent with Pro Con
Inc, allowed access to the site. Questions/comments were discussed with Wally while on site.
The following is a brief summary of our findings:
Progress:
Foundation:
The foundation work, consisting of concrete foundation walls, concrete piers, and concrete
column/wall footings is complete.
Slabs on grade:
Interior slabs on grade have been completed.
From 92/6/92 report: In locations at the slab on grade, cracks were viewed to be %" +/- in width in
various areas. In one location, a small amount of elevation variance on each side of the crack
was observed. As discussed with Wally on site, these cracks must be repaired. Cracks'/" or
larger in width must be grooved out and grouted as required.
Exterior slabs on grade, ramps and exterior pads have not yet been placed.
Wood Framing: The extent of the wood framing, inclusive of wood beams, joists, floor trusses,
roof trusses and wall panels had been installed at all levels at the combustible framed wing.
Structural steel framing: The majority of the structural steel beam, column and HSS x -bracing
framing had been erected at the time of the visit, only miscellaneous work remains.
Structural steel bar joists had been installed at structural steel beam support locations and CFS
wall framing on the 2nd, 3`d and 4t" floor framing levels of the non-combustible framed wing.
From 92/6/92 report:
At the "Break Room 219" location, a beam stiffener and column bolt holes are misaligned with the
column location. New stiffeners should be added to the beam, and the column attached to the
beam flange via welds (to be reviewed by the SER) or new bolts.

From 1216112 report: Various end plate moment connections were observed to have TC bolts that
have not been snapped. All bolts in these connections shall be torqued to the designed values.
From 1216112 report. The third floor beam located in the braced frame at grid line A-4 between
grid lines A -C and A-E was observed to have gaps between the top flange and the steel decking.
The steel deck should be verified to have all puddle welds in place — connecting it to the beam. If
the deck cannot be attached to the beam, shim plates may be used to create a positive
connection — notify SER for further direction.
Cold Formed Steel (CFS) Framing: CFS bearing and non-bearing exterior wall framing had
been installed at all levels of the structure for support of the structural steel joist framing. Erection
of the pre-engineered CFS roof trusses had also been completed.
In (2) locations, damaged CFS trusses had been noted, particularly in the area of the bottom
chord. These trusses require replacement or repair. If repaired, a certified repair design sketch
must be generated and forwarded to our office for review. Locations noted were above units 201
and 403. All areas must be reviewed to ensure additional locations do not exist with this
scenario.
CMU wall framing: CMU wall framing had nearly been completed at the time of the visit for stair
towers and elevator.
From 1018112 report In most locations, the L6x4 angles had been installed along the 8" CMU fire
wall as indicated on 5/S5.2. As viewed and discussed with Wally, the bond beam had been
installed a course to low at the 2nd floor level. This alternate scheme will require either of the
following to be implemented:
• Cells at 24" olc, corresponding to %" diameter expansion anchor locations must be fully
pressure grouted
• Screen anchors by Hilti or approved equal (suitable for use in ungrouted cells) must be
installed in lieu of the expansion anchors detailed on section 5/S5.
Update 12/6/12: Anchors had been installed in these locations as required on the combustible
wing side of the firewall. However, at several locations noted on the non-combustible wing side
(and few locations on the combustible), the bolts were installed at the top elevation of the vertical
slotted holes. This scenario will not allow for the designed vertical movement of the floor frame
along the CMU wall. As installed, the current condition utilizes the CMU fire wall as a bearing
wall (not permitted). These angles must be reinstalled with the bolts at the bottom of the slot to
allow the necessary vertical movement. Update 1/28/13: As an approved alternate, the washers
and nuts are to be removed and the bolts are to be cut approximately 3/" from the face of the
angle. Update 4/3/13: Along the combustible wing of the structure, the weight of the structural
steel angle, coupled with the distance from the angle to the nearest pre-engineered wood truss, is
resulting in a slight deflection of the floor sheathing. As discussed with Wally on site, an
additional wood truss is to be installed along side of the existing truss to reduce the span of the
floor sheathing and limit the deflection of the sheathing.
From 1216112 report: The 2"d floor deck support angle along the grid line A-2 stair CMU wall was
observed to have multiple bolts installed in open knockouts. These bolts (where centered in the
knockout) should be grouted solid with the placement of the stair support beams. The bolts that
are not centered in the knockouts should be re -installed a minimum of 4" from the edge (to
ensure proper grouting around the anchor).

From 12/6/12 report. The top corner of the "Stair 1203" CMU wall (at the third floor corridor) was
observed to have a crack propagating from the corner down to the interior edge of the doorjamb.
The crack was observed on both sides of the CMU wall (corridor and shaft edge). This crack
should be filled with an epoxy resin such as Sikadur 35 Hi -Mod LV (or approved equal).
Comments: As indicated by the information contained in this report, the majority of the items
remaining pertain to "A" wing. With the exception of the additional pre-engineered wood truss to
be added on the combustible side of the fire wall, there are no remaining items in "C' wing. And
with the exception of the structural steel column to be relocated along the exterior wall separating
"B" wing from the break room (item #2 below) and repairs required to the concrete slab on grade,
there are no additional items to address on "B" wing.
Items to be addressed by the Contractor:
1. From 1216112 report: Repair cracks at slab on grade as discussed above.
2. From 1216112 report: New stiffeners and a beam/column connection shall be installed at
the "Break Room219", where the as -built stiffeners are misaligned.
3. From 1216112 report. All TC bolts in end plate moment connections shall be torqued until
snapped.
4. From 1216112 report: The third floor beam located in the braced frame at grid line A4
between grid lines A -C and A-E shall have a positive connection with the steel deck.
Please verify all puddle welds are in place. If the deck cannot be attached to the beam,
please notify the SER for an alternative shim plate detail.
5. Repair or replace CFS roof trusses above units 201 and 403. If repaired, certified repair
sketches must be generated by CFS subcontractor and forwarded to our office for review.
6. From 1018112 report. At the 2" floor framing level, along the fire wall, blocks must be
grouted solid at L6x4 ledger support or screen anchors must be utilized to secure the
L6x4 continuous angles as discussed above. Update 12/6/12: Reinstall bolts at the
bottom of the slots in the angle and reinstall angles as discussed above. Update 1/28/13:
As an approved alternate, the washers and nuts are to be removed and the bolts are to
be cut approximately 3/4" from the face of the angle. Update 413113: An additional pre-
engineered wood truss is to be installed along the existing truss on the combustible wing
to reduce the span of the floor sheathing as discussed above.
7. From 1216112 report. The bolts that are not centered in the knockouts at the 2"4 floor deck
support angle along the grid line A-2 stair CMU wall should be re -installed a minimum of
4" from the edge (to ensure proper grouting around the anchor). All voids should be
grouted solid.
8. From 1216112 report: The top corner of the "Stair 1203" CMU wall should be filled with an
epoxy resin such as Sikadur 35 Hi -Mod LV (or approved equal).
9. Please continue to send special inspection reports to our office through the architect for

02/20-04 Wally Ryan discussed the library flue routing options: 1.Vertically through the building to

protection of the flue through upper floors through to the roof. Provide all required details and
revisions necessary to facilitate work. (At Pub #251, review possible routing through millwork

inadequately adhered to the felt paper. Gaps, air bubbles, and loose ends are visible. At this
instance only, apply even consistent pressure at the entire tape surface in order to get the
maximum uniform adhesion possible. At all future locations, use of this product is prohibited.
Submit to architect for approval all substitute materials to be used before any similar future
work commences. WR
04/18-02 In some instances, open mortar joint between the thin brick soldier course and the thin brick
running bond course exceeds the allowable 3/8" joint depth. Thin brick contractor to submit
via RFI how this joint is to be treated in order to look uniform with the other thin brick work
and fully conceal all substrate material behind the thin brick. WR
04/18-03 The installed trim between the thin brick pilaster and the PVC pilaster is acceptable and
approved to be replicated at all trim conditions between brick and PVC pilasters. WR
04/18-04 Provide pipe insulation for all piping above Entry Hall #131 and Stair #3 and elsewhere as
required by the drawings, specifications and good practice. Pipe insulation is to safeguard
against condensation and noise. WR
04/18-05 The ceiling at Sun Room #301 is to be raised in order to accommodate adequate window and
window trim clearance. Architect to submit via ASI a revised reflected ceiling plan for this
change. The cove lighting will be deleted in its entirety and replaced with strip light fixtures
and automatic dimming to facilitate "sun -downing" on the east elevation. WR/CB
04/18-06 The installed vented vinyl soffit material above the fourth floor balconies is acceptable as
installed. The previous requirement for additional 1x3 PVC trim on top of the vinyl (to match

WORK IN PROGRESS: (appears to be perjob requirements unless noted.)
• IL unit trim and balcony guardrail mock-up completed and ready for review.
• GWB installation in progress in `5A' (building part `C') common areas.
• Installation of exterior windows in `2A' (building part `A') half of building in progress.
• Framing at `2A' (building part `A') east half of building in progress and almost complete.
• Porte Cochere steel framing complete, roof framing not yet started.
• Siding and trim in progress on south elevation of building.
• Construction of IL Patio roof canopy set to start.

09/18-08 At stair No. 4 exterior masonry opening for window, grind smooth the cmu outside corners to
view to match the bullnose jamb profiles above. (03/07/13) Item OPEN. WR
10/17-01 WR reported that due to MEP -FP conflicts, the 3`d floor corridor ceilings must be lowered 2" to
8-4" a.f.f. EA noted this is acceptable only if required. All MEP -FP items are to be held tight to
structure above. (03107/13) Ceiling framing installed as tight as possible to underside of
sprinkler piping. Ceiling height to be 8'-2" +/-. CLOSED
11/14-01 Cracks in 1s` floor slab were noted. Cracks to be filled and ground smooth prior to installation
of floor finishes. (03/07/13) Item OPEN. WR
12/19-02 At West end of building, foundation wall protrudes into recess, beyond extent of exterior walls
above. Roof over exit door will be extended to cover protruding foundation wall. (01/23/13)

12/19-03 In IL -05c one bedroom units with dens: the sprinkler head at the center of the ceiling appears
to present a conflict with the surface mounted light scheduled for that location. (01/23/13)

02/20-04 Wally Ryan discussed the library flue routing options: 1.Vertically through the building to
above the roof. 2. Horizontal venting out the side of the building onto the patio. Option #2 is
not acceptable due to beam clearance and patio use. Architect to review vertical routing and
protection of the flue through upper floors through to the roof. Provide all required details and
revisions necessary to facilitate work. (At Pub #251, review possible routing through millwork

puddles and mud. WR or LS to inspect materials prior to installation. All soiled materials to be
thoroughly cleaned and dried. All deformed or damaged materials will not be allowed to be
used on this project at the contractor's expense. WR
03/07-02 In AZ Dining Room #331, the revised light fixture quantities, locations, and GWB soffits have
not yet been revised in the field. WR to coordinate with Florence Electric to ensure the work
being done on-site is per the revised drawings. Refer to RFI #63 response and sketch #-70
for clarification. Owner to verify all light switching controls, typical. WR
03/07-03 Not used.
03/07-04 Architect to further review balcony guardrail mock-up at PCIA office. WR/LS to verify with
guardrail contractor if top rail and bottom stile can be offset to better attach to column without

Unit #338 entrance door conflicts with fish tank location. Relocate entrance door 24" from

Lobby bridge framing and concrete floor complete. Architect to provide detail for typical
guardrail post connection, blocking, and trim. (Pour stop in alignment with face of beam

02/20-01
At the open shelving in IL unit living rooms provide front apron to support edge of shelf and
conceal ledger wall support beyond. Refer to attached photo for further clarification and

At all unit bathroom open shelving above bathroom vanities. Shelf depth is 18" +/-. Provide
typical shelf depth of 12". Infill installed shelves with GWB in order to maintain 12" uniform
shelf depth for all unit bathrooms. See attached photo for further clarification. (03/07/13) Item

At first floor corridors, there are several locations where plumbing risers along the concrete
retaining wall do not align within framing for the typical corridor arch feature. At these
locations only, double the pilaster width to cover the riser. Also, provide a soffit and matching
pilaster at the adjacent arch. Refer to attached sketch/photo for clarification. (03/07/13) Item

03/07-05 Architect to have fence enclosure option at the kitchen exhaust fans, adjacent to the AZ
Patio, reviewed by acoustic consultant. The fence may possibly conceal the vertical black
membrane roof at the higher roof above the Kitchen. Fence type, location, and extent are
dependent upon the final review by the consultant. CB
03/07-06 Shift in operable partition location requires revised plan detail at column enclosure at
opposite end of partition storage closet. Architect to review and provide sketch. CB
03/07-07 Modified beam at IL Unit #433 complete. Architect to provide detail for beam enclosure and
revised ceiling plan. (See attached photo.) CB
