The property consists of 1.6+/- acres. The total area of construction disturbances is 1.5 +/--acres. The maximum area to be disturbed at any one time is less than 1.5 +/- acres. No federally -listed threatened or endangered species or their designated critical habitat(s) are likely to occur in your site's "action area" as defined in Appendix A of this permit. Criterion B. The construction site's discharges and discharge -related activities were already addressed in another operator's valid certification of eligibility for your action area under eligibility Criterion A, C, D, E, or F and there is no reason to believe that federally - listed species or federally -designated critical habitat not considered in the prior certification may be present or located in the "action area". To certify your eligibility under this Criterion, there must be no lapse of NPDES permit coverage in the other operator's certification. By. certifying eligibility under this Criterion, you agree to comply ith any effluent limitations or conditions upon which the other operator's certification as based. You must include in your NOI the tracking number from the other operator's notification of authorization under this permit. If your certification is based on another operator's certification under Criterion C, you must provide EPA with the relevant supporting information required of existing dischargers in Criterion C in your NOI form. Criterion C. This determination may include consideration of any stormwater controls and/or management practices you will adopt to ensure that our discharges and discharge -related activities are not likely to adversely affect listed species and critical habitat. To make this certification, you must include the following in our NOI: 1) any federally listed species and/or designated habitat located in your "action area"; and 2) the distance between your site and the listed species or designated critical habitat (in miles). You must also include a copy of your site map with our NOL Criterion D. Coordination between you and the Services has been concluded. The coordination must have addressed the effects of your site's discharges and discharge -related activities on federally -listed threatened or endangered species and federally - designated critical habitat, and must have resulted in a written concurrence from the relevant Service(s) that your site's discharges and discharge -related activities are not likely to adversely affect listed species or critical habitat. You must include copies of the correspondence between yourself and the Services in your SWPPP and your NOI. Criterion E. Consultation between a Federal Agency and the U.S. Fish and Wildlife Service and/or he National Marine Fisheries Service under section 7 of the ESA has been concluded. The consultation must have addressed the effects of the construction site's discharges and discharge -related activities on federally -listed threatened or endangered species and federally -designated critical habitat. You must include copies of the correspondence between yourself and the Services in our SWPPP and your NOL Criterion F. Your construction activities are authorized through the issuance of a permit under section 10 of the ESA, and this authorization addresses the effects of the site's discharges and discharge -related activities on federally -listed species and federally -designated critical habitat. Inspections shall be performed either (1) every (7) calendar days; or (2) a site visit inspection once every (14) days and within (24) hours of the occurrence of a storm event of 0.25 inches or Greater. This will ensure the erosion controls are operating and functioninaproperly. Within 24 hours of completing each inspection, the inspector is required to complete an inspection report. 4.1 Natural Buffers or Equivalent Sediment Controls Buffer Compliance Alternatives Are there any surface waters within 50 feet of your project's earth disturbances? ❑ YES ® NO Check the compliance alternative that you have chosen: ❑ I will provide and maintain a 50 -foot undisturbed natural buffer. ❑ I will provide and maintain an undisturbed natural buffer that is less than 50 feet and is supplemented by additional erosion and sediment controls, which in combination achieves the sediment load reduction equivalent to a 50 -foot undisturbed natural buffer. ❑ It is infeasible to provide and maintain an undisturbed natural buffer of any size, therefore I will implement erosion and sediment controls that achieve the sediment load reduction equivalent to. a 50 -foot undisturbed natural buffer. ❑ 1 qualify for one of the exceptions in Part 2.1.2.1.e. For Alternative 1 (see Appendix G, Part G.2.3.2.a): For Alternative 2 (see Appendix G, Part G.2.3.2.b): ❑ Buffer disturbances are authorized under a CWA Section 404 permit. 16 Stormwater Pollution Prevention Plan (SWPPP) North Andover School Department Administration Building ❑ Buffer disturbances will occur for the construction of a water -dependent structure or water access area (e.g., pier, boat ramp, and trail). 4.2 Perimeter Controls General Sediment controls include the incorporation of silt fence and hay bales as shown on the plan. Specific Perimeter Controls Perimeter Control # 1 Hay bales and/or silt sacs are to be placed around each of the catch basins to prevent sediment travel into the storm drainage system. The forebay has the effect of collecting and holding runoff from the street drainage system, and allowing it to be treated and controlled, before being infiltrated into the around or discharged. The overland outlet from the system will be loamed and seeded and rip -rapped. The disturbed portions of the entire site will be hydro -seeded if construction including loaming and seeding has not been completed prior to October 1st of a season. This will provide approximately three (3) or four (4) weeks growing season for the grass, which will serve to stabilize the site to accommodate spring runoff periods. Installation The silt fence and hay bales will be installed on site prior to the start of any construction. Maintenance Requirements The hay bales and silt fence are considered temporary measures to be employed during construction. These devices will remain in place until the earth disturbing activity is stabilized with loam and seed and grass is growing. If a significant amount of soil is tracked onto the roadway from lot construction, then, hay bales or silt sacs will be reinstalled in the catch basins. Silt fence and hay bales shall be checked weekly and after rainstorms, repaired and replaced as necessary due to sediment buildup behind it or on the device. After large storm events, the cleaning of sediment may be necessary and properly disposed of. If storm events occur after the slopes and other areas, which are stabilized, have been planted but prior to seed germination, then replanting and re- aradina of those areas may be necessary. 4.3 Sediment Track -Out General Minimize the tract -out sediment by complying with the requirements. Specific Track -Out Controls Track -Out Control # I A stabilized construction entrance has been provided to help reduce vehicle tracking of sediments. The paved street adjacent to the site entrance will be swept as necessary to remove any excess mud, dirt or rock tracked from the site. A stabilized pad of aggregate underlain with filter cloth located at any point 17 Stormwater Pollution Prevention Plan (SWPPP) North Andover School Department Administration Building where traffic will be entering or leaving a construction site to or from a public right-of-way, street, alley, sidewalk or parking area shall be provided. Track -Out Control Description ■ Restrict vehicle use to properly designated exit points. • Use appropriate stabilization techniques at all points that exit onto paved roads so that the sediment removal occurs prior to vehicle exit (aaaregate stone with underlying geotextile, filter fab(c, or turf mats). ■ Use additional controls to remove sediment from vehicle tires prior to exit (ex. wheel washing). Maintenance Requirements The entrance shall be maintained in a condition which will prevent tracking of sediment onto public rights-of-way or streets All sediment spilled dropped or washed onto public rights-of-way must be removed immediately. When necessary, wheels must be cleaned to remove sediment prior to entrance onto public rights-of-way. When washing is required it shall be done on an area stabilized with aaaregate which drains into an approved sediment trapping device All sediment shall be prevented from entering storm drains, ditches, or watercourses. If sediment has been tracked -out remove the deposited sediment by the end of the same work day by sweeping shoveling or vacuuming the surfaces or by using other similarly effective means of sediment removal You are prohibited from hosing or sweeping tracked -out sediment into any stormwater conveyance (unless it is connected to a sediment basin sediment trap, or similarly effective control), storm drain inlet, or surface water. 4.4 Stockpiled Sediment or Soil General All runoff will be controlled by erosion controls; hay bale s and silt fence to reduce and divert the flows from stockpiled sediment or soil retain or detain flows or otherwise limit exposure and discharge of pollutants. Minimize the generation of dust to avoid pollutants from being discharged into surface waters through the application of water or other dust suppression techniques. Better vegetative stabilization reduces erosion rates for the underlying soil and also increases the infiltrative capacity of the soil, reducing the amount of sediment transported to downslope sediment and perimeter controls. Specific Topsoil Controls Topsoil Control # 1 Topsoil Control Description ■ The stockpile will be in an area that will not interfere with construction. ■ The slopes of the stockpile will not exceed 2:1 to prevent erosion. ■ Stockoile the native topsoil on the site • Limit disturbance and removal of the topsoil and associated vegetation Installation ■ Topsoil stockpiles will be established during grading activities. Maintenance Reauirements: ■ The area will be inspected weeklv. Maintain the soil structure on construction site and provide a growing medium for vegetative stabilization measures. Limit clearing and grading to only those areas where necessary. 4.8 Soil Compaction General ■ Reduce/avoid soil compaction which reduces total pore space of a soil. It reduces the amount of large pore space, restricts air & water movement into and through the soil. Minimize soil compaction to allow for infiltration and retention of stormwater reducing stormwater discharge volume and velocity. Specific Soil Compaction Controls Soil Compaction Control # 1 Soil Compaction Control Description ■ Restrict vehicle and equipment use where final vegetative stabilization will occur or infiltration installed. ■ Use techniques that condition the soils to support vegetative growth. ■ Control/manage traffic ■ Limit vehicle load ■ Avoidance on wet soils Maintenance Requirements 20 Stormwater Pollution Prevention Plan (SWPPP) North Andover School Department Administration Building ■ Reduce stormwater discharges which reduces erosion and therefore reduces the amount of sediment and other pollutants discharged from the site. ■ Soil conditioning techniques may include deep -ripping and decompaction or sub -soiling. 4.9 Storm Drain Inlets General ■ Storm drain inlet protection methods prevent sediment from entering storm drainage systems prior to permanent stabilization of the disturbed area. This reduces the rate at which sediment laden water may enter an inlet thereby causing ponding and settlincLof sediment. Minimize sediment discharges from the site by reguiring_stormwater inlets to be protected with sediment controls. Specific Storm Drain Inlet Controls Storm Drain Inlet Control # 1 Storm Drain Inlet Control Description ■ Inlet protection will prevent cloaaina of the storm sewer caused by heavy sediment loads. ■ Hay bales and/or silt sacs can be constructed around the drain inlet and installed according to the specifications contained in BMP. ■ Install inlet protection measures that remove sediment prior to the entry into the storm drain inlet. ■ Inlet protection measures include fabric filter, sandbags, hay bales and gravel barriers. Installation • Inlet protection should be Dlaced immediately after the storm sewer inlets are installed ■ If removed during certain phases of construction, inlet protection should be replaced • In low areas a pond may form around inlets. Ponding is necessary for removing sediment from runoff and should be encouraged in conjunction with inlet protection. ■ Inlet protection should be left in place until the drainage area is stabilized with establish vegetation or pavement. Maintenance Requirements ■ Clean or remove and replace, the protection measures as sediment accumulates, the filter becomes clogged and/or performance is compromised. Where there is evidence of sediment accumulation adjacent to the inlet protection measure, you must remove the deposited sediment by the end of the same work day in which it is found or by the end of the following work day if removal by the same work day is not feasible. ■ Inspect inlets frequently and after each rainfall ■ Remove accumulated sediment ■ Replace or repair if damaged 4.10 Constructed Stormwater Conveyance Channels General ■ Channels must be desianed to avoid unstabilized areas on the site and to reduce erosion. Design stormwater conveyance channels to avoid unstabilized areas on site and to reduce erosion, unless infeasible. Minimize erosion of channels and their embankments, outlets, adjacent stream banks, slopes, and downstream waters during discharge conditions through the use of velocity dissipation devices (e.g. check dams, sediment traps, riprap or grouted riprap at outlets) within 21 Stormwater Pollution Prevention Plan (SWPPP) North Andover School Department Administration Building and along the length of any constructed stormwater conveyance channel and at any outlet to provide a non-erosive flow velocity. 4.11 Sediment Basins General ■ N/A Specific Sediment Basin Controls Sediment Basin Control # I Sediment Basin Control Description ■ Provide storage for either the calculated volume of runoff from a 2 -year. 24-hour storm or 3,600 cubic feet per acre drained; ■ Utilize outlet structures that withdraw water from the surface in order to minimize the discharge of pollutants; • Prevent erosion of the sediment basin using stabilization controls and the inlet and outlet using erosion controls and velocity dissipation devices; ■ Sediment basins must be situated outside of surface waters and any natural buffers and must be designed to avoid collecting water from wetlands. Maintenance Reauirements • Keep in effective operating condition and remove accumulated sediment to maintain at least 1/2 of the design capacity of the sediment basin at all times. 4.12 Chemical Treatment Soil Types List all the soil types (including soil types expected to be found in fill material) that are expected to be exposed during construction and that will be discharged to locations where chemicals will be applied: The Web Soil Survey Essex County, maps the site as a Woodbridge, Paxton and Udorthent Soil. Woodbridge series consist of nearly level to steep, deep (5+ ft.), moderately well drained soils on drumlins. They formed in compact glacial till. Woodbridge soils have friable fine sandy loam or sandy loam surface soil and subsoil with moderate permeability over a firm, fine sandy loam or sandy loam substratum at 15 to 38 inches which has a slow or very slow permeability. Woodbridge soils have a very stony or extremely stony surface except where stones have been removed and have stones below the surface. They have a perched, seasonal high water table at 18 to 24 inches. Paxton series consists of gently sloping to very steeg, deep (5+ ft.) well drained soils on drumlins. They formed in compact glacial till. Paxton soils have friable fine sandy loam surface soil and subsoil with moderate permeability over a firm or very firm fine sandy loam substratum (hardpan) at 15 to 38 inches which has slow or very slow permeability. Paxton soils have a very stony or extremely stony surface, except where stones have been removed, and have stones below the surface. Udorthents consists of areas from which soil has been excavated and/or deposited due to construction operations They occur on uplands glacial outwash glacial lake and coastal plains and urban land These areas have been disturbed to the extent that the natural lavers of soil are no longer recognizable and are no longer a maior factor in determining_ limitations or capability of the land. 22 Stormwater Pollution Prevention Plan (SWPPP) North Andover School Department Administration Building Chemicals must be selected that are appropriately suited to the types of soils likely to be exposed during construction and discharged to locations where chemicals will be applied and to the expected turbidity, pH and flow rate of stormwater flowing into the chemical treatment system or area. Treatment Chemicals List all treatment chemicals that will be used at the site and explain why these chemicals are suited to the soil characteristics: No chemicals will be used on site. Describe the dosage of all treatment chemicals you will use at the site or the methodology you will use to determine dosage: N/A Provide information from any applicable Material Safety Data Sheets (MSDS): Describe how each of the chemicals will stored: If chemicals are on site store all treatment chemicals in leak proof containers that are kept under storm -resistant cover and surrounded by secondary containment structures designed and maintained to minimize the potential discharge of treatment chemicals in stormwater. (example storing chemicals in covered area or having a spill kit available on site . Include references to applicable state or local requirements affecting the use of treatment chemicals, and copies of applicable manufacturer's specifications regarding the use of your specific treatment chemicals and/or chemical treatment systems: Comglly with relevant state and local requirements affecting the use of treatment chemicals. Special Controls for Cationic Treatment Chemicals (if applicable) If you have been authorized by your applicable Regional Office to use cationic treatment chemicals, include the official EPA authorization letter or other communication, and identify the specific controls and implementation procedures you are required to implement to ensure that your use of cationic treatment chemicals will not lead to a violation of water quality standards: Schematic Drawings of Stormwater Controls/Chemical Treatment Systems Provide schematic drawings of any chemically -enhanced stormwater controls or chemical treatment systems to be used for application of treatment chemicals: Training Describe the training that personnel who handle and apply chemicals have received prior to permit coverage, or will receive prior to the use of treatment chemicals: Ensure all persons who handle and use treatment chemicals at the construction site are provided with appropriate, product -specific training. Training must cover proper dosing requirements. 4.13 Dewatering Practices General Prohibit the discharge of groundwater or accumulated stormwater that is removed from excavations, trenches foundations vaults or other similar points of accumulation, unless such waters are first treated by an appropriate control (sediment traps sediment basins, dewatering tanks, tub settlers, weir tanks, or filtration systems that are designed to remove sediment). 23 Stormwater Pollution Prevention Plan (SWPPP) North Andover School Department Administration Building Specific Dewatering Practices Dewatering Practice # I Dewatering Practice Description ■ Do not discharge floating solids or foam. ■ Use an oil -water separator or suitable filtration device that is designed to remove oil, grease, or other products . ■ Utilize vegetated, upland areas of the site to infiltrate dewatering before discharge. ■ Comply with the velocity dissipation requirements Maintenance Requirements ■ With backwash water, either haul it away for disposal or return it to the beginning of the treatment process; and replace and clean the filter media used in dewatering devices when the pressure differential equals or exceeds the manufacturer's specifications. 4.14 Other Stormwater Controls General ■ N/A Specific Stormwater Control Practices Stormwater Control Practice # I Description Maintenance Requirements 4.15 Site Stabilization After completion of vour construction Droiect you must take steps to permanently and finally stabilize it. After the project has been fully stabilized remove temporary sediment and erosion control devices. Final stabilization is when a uniform, evenly distributed perennial vegetative with a density of 70 percent of the native background cover has been established on all unpaved areas and areas not covered by permanent structures. Once construction activity has been completed and disturbed areas are stabilized, you are required to submit a Notice of Termination (NOT) to end your coverage under the Construction General Permit CGP. Comply with specific pollution prevention standards for the following pollutant -generating activities that may result in pollutant discharges. Construction site pollutants shall be stored indoors or under cover whenever possible or in areas with secondary containment. Secondary containments prevent a spill and ensure the protection of groundwater. Designate staging areas for activities such as fueling vehicles, mixing paints, plaster, mortar, etc. will help monitor the use of material and clean up any spills. The following are practices that will be used to reduce the risk of spills or their accidental exposure of materials and substances to stormwater runoff: • An effort will be made to store only enough products required to do the job. • All materials stored onsite will be stored in a net, orderly manner in their appropriate containers and, if possible, under a roof or other enclosure. • Products will be kept in their original containers with the original manufacturer's label. • Substances will not be mixed with one another unless recommended by the manufacturer. • When possible, all of a product will be used up before disposing of the container. Proper used and disposal will be followed. • The site supervisor will inspect daily to ensure proper use and disposal of material onsite. • Manufacturer's recommended methods for spill cleanup will be clearly posted and site personnel will be made aware of the procedures and the location of the information and cleanup supplies. • Material and equipment necessary for spill cleanup will be kept in the materials storage area onsite. • All spills will be cleaned up immediately after discovery. • The spill area will be kept well ventilated and personnel will wear appropriate protective clothing to prevent injury from contact with a hazardous substance. • Swills of toxic or hazardous materials will be reported to the appropriate State or local government agency. • Spill prevention plan will be adjusted to include measures to prevent this type of spill from reoccurring and how to clean un the spill. 5.3 Fueling and Maintenance of Equipment or Vehicles General Provide an effective means of eliminatina the discharae of spilled or leaked chemicals, includina fuel from the area where activities will take place. Prohibit toxic or hazardous substances from a spill or other release. Performing equipment/vehicle fueling and maintenance at an off-site facility is preferred over performing these activities on site however this is not always possible. Must inspect all pollutant -generating activities and pollution prevention controls in accordance with your inspection frequency requirements to avoid situations that may result in leaks, spills, and other releases. ■ The on-site fueling area should have a spill kit and staff should know how to use it. ■ Use drip pans, drip cloths or absorbent pads when replacing spent fluids. ■ Collect all spent fluids, store in appropriate labeled containers. 5.4 Washing of Equipment and Vehicles General Minimize the discharge of pollutants from equipment and vehicle washing, wheel wash water, and other wash waters. ■ Designate location of eauipment/vehicle washing and all washing must occur in this area only. Educate on proper washingprocedures. Contain wash water and treat and infiltrate it whenever possible. Use high-pressure water spray at vehicle washing facilities without any detergents. Specific Pollution Prevention Practices Pollution Prevention Practice # 1 Description ■ Provide cover to prevent products from coming into contact with rainwater or prevent the discharge of pollutants from these areas. • Establish proper building material staging areas ■ Designate paint and concrete washout areas ■ Provide for waste management Maintenance Reauirements • Minimize the exposure to stormwater of any of the products, materials, or wastes that are present at the site by complying with the requirements. ■ Desianate hazardous waste -collection areas on-site ■ Place all hazardous and toxic materials wastes in secondary containment ■ Hazardous waste containers shall be inspected to ensure that all containers are labeled properly and that no leaks are present ■ Reuse and recycle construction materials when possible 5.5.2 Pesticides, Herbicides, Insecticides, Fertilizers, and Landscape Materials General Prevent chemicals from coming into contact with rainwater, to prevent the discharge of pollutants. ■ Fertilizers used will be applied only in the minimum amounts recommended by the manufacturer. Once applied, fertilizer will be worked into the soil to limit exposure to stormwater. Storage will be in a covered area. The contents of any partially used bags of fertilizer will be transferred to a sealable plastic bin to avoid spills. If infeasible to complete the installation or repair within 7 calendar days, you must document why it is infeasible and document your schedule for completing the corrective action as soon as practicable. Following are the most important points you need to know about. Although the binder you have been handed may seem overwhelming, we encourage you to read through it all. Following are some questions and answers, which may help your understanding of this process. WHY MUST I FILE? The Environmental Protection Agency (EPA) has instituted regulations to prevent pollution from stormwater and runoff into wetlands and streams. This is the focus of the National Pollutant Discharge Elimination System (NPDES) permit for regulated construction activities. Polluted stormwater runoff is a leading cause of impairment to the nearly 40% of surveyed U.S. water bodies which do not meet water quality standards. WHO MUST FILE? Operators of construction sites one acre or larger (including smaller sites that are part of a larger common plan of development) must obtain authorization to discharge stormwater under an NPDES construction stormwater permit. HOW DO I FILE? A) You or your representative (Norse) must prepare a Stormwater Pollution Prevention Plan (your binder). B) You or your agent must file a Notice of Intent (National Pollutant Discharge Elimination System NOI) with the EPA. This is a (2) page form mailed to the EPA, and a copy is found in your SWPPP under "Appendix D EPA NOI". WHEN MAY I START CONSTRUCTION? You are authorized to discharge stormwater from construction activities under the terms and conditions of this permit seven (7) calendar days after acknowledgement of receipt of your electronic NOI is posted on EPA's database. The NOI may be reviewed NPDES website: http://cfpub.epa. o�v/npdes/stormwater/noi/noisearch.cfm WHAT IF THERE ARE CHANGES IN THE PROJECT? The SWPPP must be amended when there is a change in design, operation or maintenance at the construction site, which could have an impact on the discharge of stormwater. It must also be modified as necessary if there were problems identified which needed correction. Z WHAT IS THE PURPOSE OF THE SWPPP? The SWPPP is a tool to ensure that construction site operators and their employees are aware of all sources of potential pollution, describe practices to be used to reduce pollutants, and assure compliance with the terms and conditions of the permit. HOW DO I TERMINATE -COVERAGE? A Notice of Termination (NOT) must be filed with the EPA either electronically or by completing the paper NOT form. This form will be completed after the disturbed soils at the site have finally been stabilized and temporary erosion and sediment control measures have been removed; stormwater discharges have been eliminated; the residence has been transferred to the homeowner (for residential construction only); or the permittee is no longer an operator of the site. The NOT must be filed within 30 days after cessation of construction activities and final stabilization has been met. HOW LONG MUST I KEEP THE SWPPP AND DATA? Copies of the SWPPP and all documentation required by this permit, including records of all data used to complete the NOI to be covered by this permit, must be retained for at least (3) years from the date that permit coverage expires or is terminated. This period may be extended by request of EPA at anytime. WHAT IS FINAL STABILIZATION? Final stabilization is defined as meaning that all soil disturbing activities at the site have been completed, and that a uniform perennial vegetative cover with a density of 70% of the cover for unpaved areas not covered by permanent structures has been established, or equivalent permanent stabilization measures (such as the use of riprap, gabions, or geotextiles) have been employed. HOW LONG IS THE PERMIT GOOD FOR? The permit is effective until May 15, 2018. WHERE MUST I KEEP A COPY OF THE SWPPP? A copy of the SWPPP, NOI and EPA acknowledgement letter must be retained at a central location on the construction site (or other location easily accessible during normal business hours to EPA, local agencies, or US Fish and Wildlife Service). If an on-site location is unavailable to store the SWPPP when no personnel are present, notice of the plan's location must be posted near the main entrance at the construction site. WHAT ABOUT THE SIGN? Norse will provide you with a sign that meets regulations. You just "fill in the blanks". Once the EPA has received your NOI and "run it by" US Fish and Wildlife, they will issue you a permit number; this number will need to be put on your sign. A notice about the permit and SWPPP must be posted conspicuously near the main entrance of the site. The permit does not require that the general public have access to the construction site nor does it require that copies of the plan be available or mailed to the members of the public. 2 WHAT ABOUT INSPECTIONS? Inspections must be conducted by a person knowledgeable in the principles and practice of erosion and sediment controls who possesses the skills to assess conditions at the construction site that could impact stormwater quality. The site must be inspected at least once every (7) calendar days or once every (14) calendar days and within 24 hours of a storm event of 0.25 inches or greater.
