The proposed work includes the pumping,crushing and filling of the existing septic tank which is to be abandoned. 1n addition, the owner proposes to connect to sewer.The proposal includes 70' of trenching to be dug from the home to the stub tie in on Campbell Street, All work is proposed more than 25' from the Bordering Vegetated Wetland. During the July 8, 2015 public meeting, the NACC voted unanimously to approve this project. Prior to the start of construction the contractor shall place a minimum 12" silt sock (biodegradable—no plastic netting) between the proposed work and the closest wetland resource area and contact the Conservation Department for a pre-construction site visit. 2. The existing tank is to be abandoned pursuant to the regulations under Title 5 of the State. Patios less than 300 square feet, not requiring excavation or grading ,planting of new native vegetation and gardening. G. Pruning of existing vegetation flow of surface waters nor alter wildlife habitat or H. Fences and stone wails which do not alter the movement(does not include retaining walls). .......... 1. All existing structures/impervious areas to be repaired or replaced must be lawfully located and/or previously approved. F. Projects proposed under this section of the Regulations shall not be eligible for any waivers provided for under any other section of the Bylaw or Regulations including waivers of the 25-foot No-Disturbance Zone or 50-foot No-Build Zone. 4.4.4 Procedures A. The applicant shall submit to the Conservation Commission a written scope of the proposed work, including a scale drawing. The applicant shall delineate the wetland resource areas both on the drawing and in the field. B. A pre construction/pre approval meeting shall be held on site,between the applicant and a representative of the Conservation Commission. C. The Conservation Commission representative shall review the submission and prepare a written list of conditions necessary to protect any adjacent wetlands. This list shall be voted on by the Conservation Commission, and presented to the applicant within four weeks of the applicant's submission date. D. The Conservation Commission shall determine whether the proposed project qualifies as a small project and shall approve or modify the written list of conditions prepared by its representative. E. The condition list shall be presented to the applicant within four(4) weeks of the applicant's submission date. F. Upon written acceptance of the condition list,the applicant may begin work. G. Should the applicant NOT accept the condition list, a normal filing under the Bylaw shall be required. 4.4.5 Following is a Iist of conditions that may be imposed by the Conservation Commission, A. However,the North Andover Conservation Commission or its designated agent shall have the final authority to determine whether the submitted plans/drawings are adequate and include all pertinent information for review. At a minimum,all plans submitted shall clearly and accurately show the present conditions and proposed work and depict the location of all wetland resource areas within 100' of the proposed work. All plans must be signed and dated by the person who prepared thein. The scope of work is as follows. 1. 1000 gal. Septic tank and leeach filed to be pumped, I Tank is to be crashed in palce and back filled accordingly as not to create sinkage, and inspected by the Board of Health,Town of North Andover. 3. A 70' trench to be dug from the existing structure to the stub tie in. Location will be provided buy the DPW Town of North Andover after permit fee is paid. 4, A%" stone lined trench and 70' of 6" SDR 35 seiwve pipe with back flow preventer installed. 5.
