Pursuant to section 4.4.2 (<) of the North.Andover Wetlands Protection Regulations, Dave Fanuele
on behalf of the Annie L. ;Sargent School, filed for a small project for work proposed at 300 Abbott
Street, North Andover. The proposed work includes the installation of a 5'wide paved pathway to
provide access to the swings for mobility challenged students. The pathway is proposed to be
approximately 96 feet from the edge of Bordering Vegetated Wetland (BVV%) as shown on the
herein referenced plan.
During the July 22, 2015 public meeting, the NACC voted unanimously to approve this project.All
work shall conform to the following:

Filing received: 7/15/15.
The following conditions are hereby mandated:
CONDITIONS:
1. Prior to the start of construction the applicant shall ensure that the site contractor has reviewed
the small project permit and is aware of the wetland resource area and the limits of the proposed
work.
. Erosion controls which are already in place and will be relocated in the area of the proposed
pathway to be between the pathway and the wetlands.

4. Upon completion of the approved project and site stabilization, please contact the Conservation
Department for a final inspection.
5. This permit shall ex ire six months from the date of issuance.

of stairs and ramps,which are to be attached to an existing home,where soil disruption during
construction is limited to less than 50 square feet,and where no permanent soil alterations are
planned under the footprint of the deck
B.In kind replacement of existing decks,regardless of size.

not permanent foundations.
D.Sewer system connections as required by the Town of North Andover,

access on conservation property.

El G.Pruning of existing vegetation,planting of new native vegetation and gardening.

A written narrative description/scope of work of the proposed project and potential impacts to all wetland
resource areas has been included.
The applicant has identified,and clearly marked all wetland resource area boundaries in the field,

agent thereof) drawing showing existing and proposed conditions and the delineated wetland resource
areas (see plan requirements below for additional detail).

A pre-construction/pre-approval meeting was held on site between the applicant anda representative of

The delineated resource area boundaries have been confirmed by the Conservation Department as
accurate.
Plan REquirements(Section 4.4.3 B):
Certified and/or professionally prepared plans are not required. However,the North Andover Conservation
Commission or its designated agent shall have the final authority to determine whether the submitted
plans/drawings are adequate and include all pertinent information for review.
At a minimum,all plans submitted shall clearly and accurately show the present conditions and proposed
work and depict the location of all wetland resource areas within 100' of the proposed work. All plans must
be signed and dated by the person who prepared them.
The Commission MAalso re wire,the following information on the proposed plans ja addition to the
aforementioned criteria where Applicable.
El Location of any subsurface sewage disposal systems.
El Locations of any drainage structures.
F] Locations of any easements,including but not limited to, drainage and utility easements,
