Pursuant to section 4.4.2 (H) of the North .Andover Wetlands Protection regulations,Jennifer
Costantino, applicant/home owner, filed for a small project for work proposed at 265 Granville
Lane, North.Andover. The proposed work includes installation of an approximately 6-foot high
white vinyl fence outside of the 50-Foot No-Disturbance Zone to F_?pherneral Pool Habitat as
depicted on the herein reference plan.
During the August 12, 2015 public.tweeting, the North.Andover Conservation Commission voted
unanimously to approve this project. The project is approved only as it relates to the Conservation
Commission and any and all other approvals should be obtained by the applicant/home owner. .All
work shall conform to the folloNving:

The following conditions are hereby mandated:
CONDITIONS:
1. Prior to the start of construction the applicant shall ensure that the site contractor has reviewed
the small project permit and is aware of the wetland resource area and the limits of the proposed
work.

2. Excess construction material (fencing, soil) shall be properly disposed of offsite and accepted
construction standards and procedures shall be followed in the completion of the project.
3. Wetland markers shall be installed at the 5U' Talo-Disturbance Gone, on the fence or on existing
trees as determined by the Conservation Department staff. Wetland markers are available at the
Conservation office ($2 round/$3 square). `
. Upon completion of the approved project and site stabilization, please contact the Conservation
Department for a final inspection.
5. This petmit shall ex ate six months from the date of issuance. 1

of stairs and ramps,which are to be attached to an existing home,where soil disruption during
construction is limited to less than 50 square feet,and where no permanent soil alterations are
planned under the footprint of the deck
❑ B.In bind replacement of existing decks,regardless of size.
❑ C.Sheds or other outbuildings of less than.100 square feet,which are to be placed on footings,
not permanent foundations.
D.Seger system connections as required by the Town.of North Andover.
❑ E.Unpaved pedestrian walkways (less than four(4)feet wide) for private use or for public
access on conservation property.
❑ F.Patios less than 300 square feet,not requiring excavation or grading.
El G Pruning of existing vegetation,planting of new native vegetation and gardening.

�7H. Fences and stone walls which do not alter the flow of surface waters nor alter wildlife habitat
or movement(does not include retaining walls).

Two (2) copies of a 1" =20` scale (or other as deemed appropriate by the Conservation Commission or
agent thereof) drawing showing existing and proposed conditions and the delineated wetland resource
areas (see plan requirements below for additional detail).

Certified and/ox professionally prepared plans are not required. However,the North Andover Conservation
Commission or its designated agent shall have the final authority to determine whether the submitted
plans/drawings are adequate and include all pertinent information for review.
At a minimum, all plans submitted shall clearly and accurately show the pLesent conditions and p1qposed
work and depict the location of all wetland resource areas within 100' of the proposed work. All plans must
be signed and dated by the person who prepared them.
The Commission may also xecluixe the following information on the proposed plans in addition to the
aforementioned criteria where a Iicable:
❑ Location of any subsurface sewage disposal systems.
❑ Locations of any drainage structures.
❑ Locations of any easements,including but not limited to,drainage and utility easements.
❑ Locations of any open space boundaries if(e.g.if the lot is affected by the 50' open space boundary as
required in a Planned Residential Development(PRD)).
❑ Location and nature of all proposed alterations to the Buffer Zone Resource Area.

Conservation Department has prepared a written list of conditions necessary fo protect any adjacent
wetlands for approval by the Conservation Commission.
This condition list shall be filed with the North Andover Conservation Department as NACC ##1, 3
❑ INCOMPLETE APPLICATION-The following sections of the Wetland Protection Bylaw&/or
Regulations have not been complied with and as a result this Department is rejecting this incomplete
application w/in two (2)business days from the date of receipt (Section 178.4 North Andover Wetland
Protection Bylaw). You may pick up this application and supporting plans at the Conservation Office and
re-file with the required supporting documents and information.
❑ NOTA SMALL PROJECT-The North Andover Conservation Commission has determined that the
proposed project as submitted does not meet the small project criteria and hereby denies the application.
As such,a Request for Determination of Applicability(RDA) or a Notice of Intent(NOI) must be filed.
