Installation of a Natural Gas Line
Conservation Conditions of Approval,NACC #166
Pursuant to section 4.4.2 (L) of the North.Andover Wetlands protection Regulations,Tyson and
Rebecca Moore (owners/applicants) and Columbia Gas of MA (co-applicant), filed for a small
project for work proposed at 1055 Salem Street,North Andover. The proposed work includes the
installation of a gas line to connect the home to natural gas from the existing line in Salem Street.
The closest point of the proposed work is approximately 85 feet from the edge of bordering
Vegetated Wetland (13 as shown on the herein referenced plan.
During the December 16,20115 public meeting, the NACC voted unanimously to approve thus
project. All work shall conform to the following:

CONDITIONS:
1
1. Prior to the start of construction the site contractor shall review the small project permit and be
aware of the wetland resource area and tlae limits of the proposed work.

2. Prior to the start of construction a silt sock (or other erosion controls as approved by the
Conservation Department) shall be placed between the work area and the wetlands and the
Conservation Department shall be contacted for a pre-construction site inspection.
3. Excess construction material shall be properly disposed of offsite and accepted engineering and
construction standards and procedures shall be followed in the completion of die project.
4. Upon completion of the approved project and site stabilization, please contact the Conservation
Department for a final inspection.
5. This .permit shall exPire six months from the date of issuance.

of stairs and ramps,which are to be attached to an existing home,where soil disruption during
construction is limited to less than 50 square feet, and where no permanent soil alterations are
planned under the footprint of the deck
B. In kind replacement of existing decks,regardless of size.
❑ C. Sheds or other outbuildings of less than 7.00 square feet,which are to be placed oil footings,
not permanent foundations.
❑ D. Sewer system connections as required by the Town of North Andover.
E. Unpaved pedestrian walkways (less than four(4)feet wide) for private use or:for public
access on conservation property.
❑ F. Patios less than 300 square feet,not requiring excavation or grading.

the Conservation Commission. Date of Meeting:
❑ The delineated resource area boundaries have been confirmed by the Conservation Department as
accurate.

Certified and/or professionally prepared plans are not required. However,the North Andover Conservation
Commission or its designated agent shall have the final authority to determine whether the submitted
plans/drawings are adequate and include all pertinent information for review.
At a minimum,all plans submitted shall clearly and accurately show the present conditions and proposed
work and depict the location of all wetlax-Ld resource areas within 100' of the proposed work. All plans must
be signed and dated by the person who prepared there.
The Commission may also require the following information on the proposed plans in addition to the
aforementioned criteria where applicable:
❑ Location of any subsurface sewage disposal systems.
j ❑ Locations of any drainage structures.
El Locations of any easements,including but not limited to, drainage and utility easements.
El Locations of any open space boundaries if(e.g. if the lot is affected by the 50' open space boundary as
required in a Planned Residential Development(PRD)).
❑ Location and nature of all proposed alterations to the Buffer Zone Resource Area.

Conservation Department has prepared a written list of conditions necessaay to protect any adjacent
wetlands for approval by the Conservation Commission.
This condition list shall be filed with the North Andover Conservation Department as NACC#V"
❑ INCOMPLETE APPLICATION-The following sections of the Wetland Protection Bylaw &/or
Regulations have not been complied with and as a result this Department is rejecting this incomplete
application w/in two (2)business days from the date of receipt (Section 178.4 North Andover Wetland
Protection Bylaw). You may pick up this application and supporting plans at the Conservation Office and
re-file with the required supporting documents and information.
F1 NOTA SMALL PROJECT-The North Andover Conservation Commission has determined that the
proposed project as submitted does not meet the small project criteria and hereby denies the application.
As such,a Request for Determination of Applicability(RDA) or a Notice of Intent(NOl)must be filed.

We would like to connect our house to the natural gas line that is already on Salem Street. We are
currently using propane for heat/cooking. Enclosed is the plot of our property with the proposed gas
line drawn. We have contacted Columbia Gas about this project, and they have indicated the need to be
named in this proposal for authorization to complete the work. Thank you for considering this proposal.
