Re: 257 Boxford Street (Map 106A, Lot 3)
Dear Mr. Smith:
The proposed wastewater system design plan for the above site dated June 2, 201$ and received
on June 12, 201$ has been reviewed. Unfortunately, the plan cannot be approved until the
following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North
Andover regulation that is not met by this design follows each item where applicable.
I. On sheet 1 of 2, the test pits and the percolation test should be graphically depicted to
accurately reflect the orientation they were dug during the soil evaluation. At least one
test pit is required to be within the proposed soil absorption system.
2. A Local Upgrade Approval will be required if only one test pit is within the proposed soil
absorption system(3 10 CMR 15.405(l)(1c)). If a Local Upgrade Approval is requested
then please note this on the revised design plan and submit the Form 9A.
3. On sheet 1 of 2, it appears buoyancy calculations are not needed for the septic tank with a
bottom elevation of approximately 94.2 and the FSHWT in TP-2 at$9.0.
4. On sheet 2 of 2, the scaled profile does not provide a minimum of 9" of cover above the
septic tank (3 10 CMR. 15.22$(1)).
5. On sheet 2 of 2, the top of the inspection port is not depicted within 3" of finish grade as
stated in System Specification note#14.
6. Since a percolation test was performed in the C horizon in TP-3 the design plan should
clearly indicate that the A and B horizons shall be removed beneath the leach field.
