Re: 312 Boxford.Street(Map 104B, Lot 59)
Dear Mr. Sullivan:
The proposed septic system design plan for the above site dated June 26, 2018 and received on
July 5, 2018 has been reviewed. Unfortunately,the plan cannot be approved until the following
items are corrected. The specific section in Title 5: 310 CMR 15.000, or North Andover
regulation that is not met by this design follows each item where applicable.
1. On Sheet 1 of 2, the soil logs for DTII-4 and DTH-5 have the Bw layer depicted incorrect
as a Sandy Loam instead of a Loamy Sand(Class 1). Also please revise the soil
evaluation forms accordingly.
2. On Sheet 1 of 2, the names of the abutters are not shown (NA 3.2).
3. On Sheet 1 of 2,please indicate the existing leach pits shall be abandoned with the same
requirements as a septic tank in accordance with 310 CMR 15.354(3).
4. On Sheet 1 of 2,please provide proposed spot grades or contours to depict the finished
grades as shown on the profile view on sheet 2 oft above the effluent line from the septic
tank to the distribution box.
5. On Sheet 1 of 2, the finish grading on the southwestern site of the leach field appears to
have a 2:1 slope between the proposed 100 contour and the existing 98 contour(3 10
CMR 15.255).
6. A cleanout should be provided in the building sewer line for the directional change
proposed between the inlet and outlet in the location of the existing septic tank (3 10
CMR 15.222(8).
7, On Sheet 2 of 2, the profile view depicts less than 9"of cover material above the septic
tank ((3 10 CMR 1.5.228(1)).

8. On Sheet 2 of 2., it appears a riser is proposed within 6" of finish grade above the
distribution box. Please provide a note on the plan to ensure the installer is aware of this
requirement.
9. Since the Infiltrator Chamber system is proposed as an alternative soil absorption system
the "Standard Conditions for Alternative Soil.Absorption Systems with General Use
Certification and/or Approved for Remedial Use"will apply. Please provide the
following as required by the approval conditions Section II(l 8):
a certification, signed by the Owner of record fbr the property to he served by
the Technology, stating that the property Owner:
L has been provided a copy of the Title 5 11A technology Approval,
the Owners Manual, and the Operation and Maintenance Manual,
and the Owner agrees to comply with all terms and conditions,-
2. jbrSystems installed under a Remedial Use Approval, the owner
agrees to fulfill his responsibilities to provide a Deed Notice as
required by 310 CA41? 15.287(10) and the Approval;
3. fir Systems installed under a Remedial Use Approval, the owner
agrees tofillflll his responsibilities to provide written notification
of the Approval to any new Owner, as required by 310 CMR
15.287(5),-
4. if the design does notprovidefin-the use of'garbage grinders, the
restriction is understood and accepted,- and
5. whether or not covered by a warranty, the System Owner
understands the requirement to repair, replace, modify or take any
other action as required by the.Department or the LAA, zf the
Department or the LAA determines the System to he.failing to
protect public health and safety and the environment, as defined in
310 CMR 15303.
