Re: 1429 Osgood Street (Map 34, Lot 20)
Dear Mr. Keefner:
The proposed wastewater system design plan for the above site dated July 10, 2018 and received
on July 10, 2018 has been reviewed. Unfortunately, the plan cannot be approved until the
following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North
Andover regulation that is not met by this design follows each item where applicable.
General
1. The location and log of all test pits and percolation tests were not depicted on the design
plan (310 CMR 15.220(4)(h -i)).
2. A benchmark is required within 50 to 75 feet of the system components is required (310
CMR 15.220(4)(q)).
3. A locus map was not depicted on the design plan (310 CMR 15.220(4)(t)).
4. The map and lot were not depicted on the design plan (310 CMR 15.220(4)(u)).
System components - General
5. A note should be added to the design plan to indicate that the septic tank, pump chamber
and distribution box shall be made watertight in accordance with 310 CMR 15.221(1).
6. A 6" stone base is required beneath the septic tank, pump chamber and distribution box
(310 CMR 15.221(2)).
7. Buoyancy calculations were not submitted for the septic tank and pump chamber (310
CMR 15.221(8)).
8. Based on the ESHWT it appears the inlet and outlet inverts of the septic tank and pump
chamber will not meet the requirement of being 12" above the ESHWT in accordance
with 310 CMR 15.227(5)). Therefore, a Local Upgrade Approval. (LUA) request is

required. The LUA form 9A should be submitted with the revised design plan and noted
on the design plan.
Septic tank
9. Since the building sewer line is proposed to cross the water service line the requirements
from note #18 on sheet C-101 will apply and should be noted clearly in the site plan view
and profile view.
10. On sheet C-102, the septic tank detail should depict the required depths of the inlet and
outlet tees (310 CMR 15.227(6)).
Pump chamber
11. On sheet C-102, the manufacturer and model number were not depicted for the pump
chamber (NA 3.2).
12. Indicate the proposed vehicle loading of the pump chamber (H-10 or H-20) (310 CMR
15.231(5)).
13. The design plan shall indicate that the pumps and alarm shall be on. separate circuits (310
CMR 15.231(9)).
14. An effluent filter is required prior to the pump chamber (310 CMR 15.231(10)). The
DEP approved brand, model number and required annual maintenance for the effluent
filter should be noted on the design plan as well.
15. Pump specifications were not provided (310 CMR 15.220(4)(r)).
16. Please confirm that mercury float switches are allowed to be used in the pump chamber
and the junction box is allowed to be installed inside the pump chamber.
Distribution box
17. An inlet tee is required in the distribution box (310 CMR 15.232(3)(a)).
18. On sheet C-102, the profile appears to depict approximately 3" of cover material above
the distribution box.. It is recommended that 9" of cover material be provided.
Soil absorption system
19. A finished grade slope of 0.02 feet per foot is required above the soil absorption system
(310 CMR 15.240(10)). Proposed spot grade elevations should be shown to depict the
minimum cover material and slope.
20. An inspection port is required in the soil absorption system (310 CMR 15.240(13)).
21. On sheet C-101, it appears the removal and sand fill for the leach trenches extends
westerly approximately 20 additional feet beyond the required overdig. Please explain
the reason for this additional sand overdig.
22. On sheet C-101, crushed stone appears to be proposed to the north of the leach trenches
between the proposed concrete retaining wall and the existing retaining wall. If this is
correct it should be labeled accordingly and shown in the profile cross section view.
23. A concrete retaining wall detail should be depicted on the design plan.
24. On sheet C-101, the storm water basin to the west of the leach trenches should be labeled
accordingly.
.Although not a reason for disapproval, you may wish to consider the following:
25. A reserve area is not required for a septic system replacement with no additional flow.
The proposed leach trenches could be closer together than proposed in order to reduce the
amount of sand fill and concrete retaining wall needed.

26. A leach field design could potentially reduce the height of the proposed concrete
retaining wall compared to the leach trench design.
