This Letter ofAgreement is between the the above named Independent Monftor and the North Andover Planning
Board for monitoring services to be provided at the above mentioned Pro' c dd s and in a ce with
the conditions and restrictions stated by the Planning Board in it's Dei / .a
copy of that Decision has been provided to the Independent Monitor. nitia| of|'depmndont
Monitor) By initialing, the Independent Monitor acknowledges receip ecision and an understanding of
the conditions/restrictions set by the North Andover Planning Board.
The Independent Monitor also acknowledges and agrees that he/she is providing monitoring services for the
benefit of the Town of North Andover and is not the project owner or applicant, and does not have a financial or
personal interest in the project. Further, the Independent Monitor understands that she/he may not act as the
project owners agent, representative, partner, employee or joint -venturer on this project.
The Independent Monitor agrees to immediately report to the North Andover Planning Board, via the Town
Planner, any violations of the terms of the Decision listed above, as soon as observed or otherwise become
known to the Independent Monitor. This report is in addition to regularly scheduled monitoring reports.
The Independent Monitor understands that the failure to perfomi under this Letter of Agreement, to the
reasonable satisfaction of the North Andover Planning Board, will be grounds for said Independent Monitor to be
excluded in this role for any future projects which are within the jurisdiction of the North Andover Planning Board.

I have had a conversation with Peter Devlin and I have approved temporarily suspending the monitoring through the
winter months. Peter has committed to having his contractor install straw mulch at the site be the end of this week.

Date of Inspection: September 12, 2017 (Week of September 11th)
74 degrees/clear
- Erosion control in place (silt sock); no sign of erosion
- A few trees cut at edge of work area right at limit of work (outside 100 foot wetland buffer)
Date of Inspection: September 19, 2017 (Week of September 18th)
65 degrees/raining
Erosion control is in good condition; no signs of silt buildup
- Foundation hole dug and footing is in place
- Drywell has been installed according to the approved plan
Date of Inspection: September 26, 2017 (Week of September 25th)
84 degrees/clear
- Erosion control in good shape (dry week)
- No signs of silt build-up along barriers
- Foundation walls have been formed and poured
Next week forms to be stripped and foundation backfilled

Site has been rough graded around garage
Date of Inspection: October 9, 2017 (Week of October 9th)
74 degrees/rain
Erosion control in good condition; 110 signs of erosion or silt migration
Inspection took place during 0.37" rain event
Framing of garage is complete
Date of Inspection: October 16, 2017 (Week of October 16th)
54 degrees/clear
Erosion control in good condition; no signs of erosion or silt migration
- Site has been roughed graded
Date of Inspection: October 25, 2017 (Week of October 23rd)
Inspection took place during 0.88" rain event
Erosion control in good condition; no signs of site erosion
- Owner is going to try an loam/seed disturbed areas early next week
Date of Inspection: November 2, 2017 (Week of October 30th)
- 0.91" rain event on 10/29 0.34" rain event on 10/30
- Erosion control is in good condition
- Spoke with contractor and loam/seed to take place next week
