Conservation Conditions of Approval,NACC #218
Pursuant to section 4.4.2 (D) of the North Andover Wetlands.Protection Regulations, applicant
Mike Melvin... of Brother's Home Services, filed for a small project for work proposed at The Family
Coop Preschool located at 48 Caroline Way/565 Osgood Street,North Andover. The proposed
work includes trenching approximately 100' from the school to an existing sewer stub in the
unpaved driveway and installing sewer piping (trench up to 7.5' deep). In addition the existing septic
tank will pumped, crushed and filled. All work is outside of the 25' No-Disturbance Zone to a.
Bordering"Vegetated Wetland associated with an intermittent stream/swale.
During the August 22, 2018 public meeting, the NACC voted unanimously to approve this project.
All work shall conform to the following:

The following conditions are hereby mandated:
CONDI 11ONS:
1. Prior to the start of construction the applicant shall ensure that the site contractor has reviewed
the small project permit and is aware of the wetland.resource area and the limits of the proposed r
work. I
2. Erosion control (12" bark mulch or compost filter sock) shall be installed between the driveway
swale and the limit of work.
3. Once installed the applicant shall contact the Conservation Department to inspect the installed
erosion controls.

4. Excess construction material shall be properly disposed of offsite and accepted engineering and
construction standards and procedures shall be followed in the completion of the project, 'F here
shall be no stockpiling of material within 100' of wetland resource areas.
5. Upon completion of the approved project and site stabilization, please contact the Conservation
Department for. a final inspection.
6. This .perinit shaft ex-pire six nionths from the date of issuance.
