In order for me to sign off on your building permit application, I need for you to fill out and bring back the enclosed form giving me permission to enter your property and ensure that erosion control (hay bales) has been placed along the perimeter of your construction to ensure that no runoff will occur or cause detrimental effects to the Lake.
