Plan for 1429 Osgood Street (Map 34 Lot 20) To Whom It May Concern: The proposed wastewater system design plan for the above site dated July 1.0, 2018 with a final revision date of November 5, 2018 and received on November 6, 2018 has been approved. The design has been approved for use in the construction of a repaired on-site septic system for a commercial building renovation. The renovated commercial space would include 1,900 S.F. of office space and 19,370 S.F. of warehouse space with five (5) employees, utilizing a 1500 gallon septic tank, 500 gallon pump chamber and a leaching trenches with a design flow of 275GPD. This design plan approval is valid until November 11, 2021. During this time, a licensed septic system installer must obtain a permit and complete this work, and a Certifieate of Compliance be endorsed by the installer, designer and the Town of North Andover. This approval. is also subject to the following conditions: f I. If site conditions are found in the field to be different from those indicated on the design plan and/or soil evaluation, the originally issued Disposal System. Construction Permit is void, installation shall stop, and the applicant shall reapply for a new Disposal Systems Construction.Permit (310 CMR 1.5.020(1)). i 2. These may include review by the Conservation Commission, Zoning Board, Planning Board, Building Inspector, Plumbing Inspector and/or Electrical Inspector. The issuance of a Disposal System Construction Permit shall not construe and/or imply compliance with any of the aforementioned requirements.
