This submittal is regarding a release of non-polychlorinated biphenyl mineral oil dielectric fluid(MODF) from a pad-mounted electrical transformer formerly located at the above-referenced location. A copy of the above-referenced report will be available for viewing after January 16,2019,at the Massachusetts Department of Environmental Protection Northeast Regional Office(MassDEP-NERD)in Wilmington, Massachusetts,and may also be obtained by contacting the undersigned in writing. A copy of the Phase I Initial Site Investigation summary of findings and conclusions is appended to this notice. A legal notice regarding this release will be published in Ybe Eagle-Tribune on or about January 12,2019. A copy of this notice is included with this letter. A copy of the Disposal. Site Plan is also attached for reference. Additional Public Involvement Activities may be requested pursuant to 310 CMR 40.1403(9). Disposal Site Plan cc: MassDEP-NERD MECO 4 First Street, Bridgewater,MA 02324 (508)fig 7-3191 (800)548-3355 cO.com Summary of Findings and Conclusions of Phase I Initial Site Investigation Non-PCB Mineral Oil Dielectric Fluid Release Pad-Mounted Electrical Transformer 435 Andover Street North Andover,Massachusetts Release Tracking Number 3-34729 Conditions of the area impacted by a historical release of non-polychlorinated biphenyl (non-PCB) mineral oil dielectric fluid(MODF) from a 150-kVA pad-mounted electrical transformer in use at the commercial property identified as 435 Andover Street in North Andover,Massachusetts were evaluated in a maimer consistent with applicable guidelines for a Phase I- Initial Site Investigation (ISI)as presented in 310 CMR 40.0480 of the Massachusetts Contingency Plan (MCP). Response actions performed in association with release tracking number(RTN) 3-34729 included a review of pertinent Disposal Site history,the performance of an Limited Removal Action(LRA) which included the removal of approximately 2 cubic yards of impacted media, the advancement of four(4) Geoprobe'test borings,the installation of four(4)monitoring well, and the collection and laboratory analysis of soil and groundwater samples. Based on the information and observations described herein, the following is a summary of the investigation findings: 0 On September 18, 2017, a historical release of non-PCB MODF fi-om a 150-kVA pad- mounted electrical transformer was identified at the Site by Massachusetts Electric Company d/b/a National Grid (MECO), who then contracted Coneco Engineers & Scientists, Incorporated(Coneco)to oversee response actions. No indication of a sudden release in excess of the applicable reportable quantity(RQ) for untested MODF(i.e., greater than 10 gallons within a consecutive 24-hour time period)was observed by MECO or Coneco personnel. Based on observations made at the time of release identification by MECO and Coneco,no 2-hour or 72-hour reportable conditions, as described in the notification requirements presented in 310 CMR 40.0311 through 40.0314,were identified in relation to the release of MODF at the Disposal Site. Although the volume of impacted soil was not known, it was conservatively assumed to be greater than 2 contiguous cubic yards. Therefore,based on observed release conditions,the release was conservatively considered to represent a Massachusetts Department of Environmental Protection(MassDEP) 120-day reportable condition, as defined in 310 CMR 40.0315(2), 0 Original manufacturer's nameplate information present on the 150-kVA pad-mounted electrical transformer indicated that the MODF contained "mineral insulating oil less than 5 ppin PCB at the time of manufacture," Pursuant to 40 CFR 761.2(a)(2), in-use mineral oil- filled electrical equipment manufactured after July 2, 1979, is considered non-PCB (i.e., less than 50 ppm, PCBs). In addition, pursuant to 40 CFR 761.2(b)(2)(i), the manufacturer's anufactuier's nameplate provides a permanent label indicating that there was"less than 5 ppm PCB at time of manufacture." As such,the MODF within the transformer was characterized as"non- PCB" in accordance with the definition set forth at 40 CFR 761.3. I • To verify the specific PCB-content of the released MODE, a MODF sample collected from the transformer on September 19, 2017,was submitted for laboratory analysis of PCBs by EPA Method 8082. No PCBs were detected in excess of the laboratory quantification limits of 0.98 mg/kg. Based on these certified laboratory analytical results, the MODF associated with the transformer was confirmed to be"non-PCB"pursuant to 40 CFR 761.3 and 310 CMR 40.0000, • In order to eliminate the source of the release and stabilize environmental conditions at the Site,MECO personnel de-energized and removed the leaking transformer on September 18, j 2017. Prior to removal, Clean Harbors Environmental Services, Inc. (CHESI)personnel drained the remaining non-PCB MODF from the transformer using a vacuum tank truck. Based on the capacity of the transformer and the volume of non-PCB MODF recovered, Coneco personnel conservatively estimated that approximately 10 gallons of non-PCB MODF had been released at the Disposal Site. • On September 18 and 19, 2017, Coneco oversaw the remediation of impacted solid surfaces and excavation of impacted media within the release area as part of LRA activities. Due to customer electrical demands, excavation activities were discontinued prior to achieving background conditions in relation to the release of non-PCB MODF. j i • Upon completion of initial LRA activities,Coneco personnel composited discrete soil. samples collected from within the conduit cutouts of the concrete transformer pad into one (1) confirmatory soil sample, designated CS-01,which was submitted for laboratory analysis of EPH by the appropriate MassDEP Method. Results of laboratory analysis indicated that concentrations of extractable petroleum hydrocarbons (EPH)carbon fraction ranges in excess of the applicable RCS-I reportable concentrations (RCs) and the applicable Method 1 Risk Characterization standards remained in soil beneath the concrete transformer pad. As such, a 120-day reportable condition was confirmed to be present at the Disposal Site. Accordingly on behalf of MECO, Coneco submitted a Release Notification to the MassDEP on January 16, 2018, and RTN 3-34729 was subsequently assigned to the Disposal Site. • On December 12 and 1.3, 2018, Coneco provided oversight for the advancement of four(4) Geoprobee test borings, designated SB-01 through SB-04, to delineate the extent of potential impacts associated with the release of non-PCB MODF at the Disposal Site. Groundwater was observed at approximately 4 to 5 feet below grade. Bedrock was not encountered during subsurface investigation activities. • Select soil samples collected from test borings SB-01 through. SB-05 on December 13,2018, were submitted for laboratory analysis of EPH carbon fraction ranges by the appropriate MassDEP Method. Results of laboratory analysis identified no EPH carbon fraction ranges in excess of the applicable Method 1 Risk Characterization Standards in any of the submitted soil samples. • The release occurred from a point source and impacted a concrete transformer pad asphalt, trap rock, and soil. Potentially-impacted soil at the Disposal Site is located beneath the concrete transformer pad. Therefore, the potential routes for exposure are limited to inhalation, ingestion, and/or dermal contact with soil during non-routine activities, such as construction or utility work, conducted within the limits of the Disposal Site. o No current or potential off-Site migration pathways have been identified at the Disposal Site. Based on Disposal Site conditions,potential human exposure is considered unlikely. Based on current Disposal Site conditions, no conditions warranting an Immediate Response Action(IRA) are present; therefore,an IRA condition is not present at the Site. Future remedial actions will be conducted under a Release Abatement Measure (RAM) and/or as part of Comprehensive Response Actions. i 1 NOTICE OF PHASE I INITIAL SITE INVESTIGATION AND TIER II CLASSIFICATION 1 Mineral Oil Dielectric Fluid Release i Pad-Mounted Electrical Transformer 435 Andover Street North Andover, Massachusetts Release Tracking Number 3-34729 A release of oil and/or hazardous materials has occurred at this location, which is a disposal site as defined by M.G.L. c. 21 E, § 2 and the Massachusetts Contingency Plan, 310 CMR 40.0000. To evaluate the release, a Phase I Initial Site Investigation was performed pursuant to 310 CMR 40.0480. As a result of this investigation, the site has been classified as Tier II pursuant to 310 CMR 40.0500. On January 16, 2019, the Massachusetts Electric Company filed a Tier Classification Submittal with the Department of Environmental Protection (MassDEP).
