Dear Owner:
Please note that upon speaking with your septic system technician and based upon a septic pumping
record dated 3/1.2/19 and received on 3/18/19,your septic system was reported as being in very poor

public health and the environment. This is a violation of 310 CMR 15.000 and needs to be brought into
compliance as soon as possible by either connecting to sewer or replacing the septic system.
Due to the information mentioned above,we have developed a set of requirements to help manage your
septic system until connection to town sewer is completed. The following timeline must be adhered to
until then;
1. You must submit a contract with a Septic Hauler agreeing to a Monthly pumping of your Septic
System within one week of receiving this letter.
2. You must obtain a Sewer tie-in permit from the Town of North Andover's Department of Public
Works within two weeks of receiving this letter.
3. You must have submitted a sewer tie in application to MassDOT within 30 days of receiving
this letter.
