The Applicant indicates states that The North Andover Planning Board issued a Watershed Special Permit in May 7, 2004 to the original owner S.J.J. Trust. That permit has since expired without the house and driveway being constructed. The Applicant also states that LRC Builders,LLC is seeking to reapply for the Watershed Special Permit on behalf of the current owner, Charles and Susan Papalia of the Five Star Investment trust in order to complete the house and driveway. I received and reviewed the following documents: 1. Special Permit - Watershed Permit Application prepared by Hayes Engineering Inc. for LRC Builders, LLC. 2. Plan to accompany Watershed Special Permit in North Andover dated Februaryl2, 2019. 11Page_ r ENGINEERING INC. 609 Winter Street Framingham.MA 01702 Phone:508-745-4077 My comments on the proposed project are outlined below: Written Certification 1. As required by the Watershed Special Permit, written certification by a Registered Professional Engineer, or other scientist educated in and possessing extensive experience in the science of hydrology and hydrogeology, stating that there will not be any significant degradation of the quality or quantity of water in or entering Lake Cochichewick,must be submitted by the Applicant. Alternate Location 2. As required by the Watershed Special Permit, proof that there is no reasonable alternative location outside the Non-Disturbance and/or Non-Discharge Buffer Zones, whichever is applicable, for any discharge, structure, or activity, associated with the proposed use to occur,must be provided by the Applicant. Design Plans 3. The Plan submitted shows the existing adjoining wetlands along with the associated buffer zones; however, the Applicant must show the required delineations i.e., Non-Disturbance and Non-Discharge Zones required for the Watershed Special Permit Application. 4. The Plan submitted states that information has been compiled from existing mapping. The Applicant should indicate if any supplemental ground survey has been completed in developing and confirming the existing conditions. 5. The Applicant should identify on the Plan any mature trees that are planned to be removed. 6. CATV and Electric Duct are proposed within the 50-foot wetland buffer. The Applicant should request a waiver or relocate these utilities outside the 50-foot buffer. 7. The proposed grading at the end of the porous paver driveway could result in channelization of runoff along the western edge of the crushed stone driveway resulting in erosion of slopes and sedimentation in the wetlands. The Applicant could revise the proposed grading and/or propose stone checkdam/level spreader to prevent concentration of runoff and intercept any sediment prior to reaching the wetlands. 2 Page A� A V ENGINEERING INC;. 609 Winter Street Framingham.MA 01702 Phone:508-745-4077 8. A construction detail for the proposed crushed stone driveway and porous pavers must be shown on the Plan. Proposed Subsurface Infiltration for Roof Runoff 9. The Applicant should state the basis for the assumed exfiltration rate of 2.41 in/hr that was used to size the subsurface infiltration system. 10. The 100-year, 24-hr design storm rainfall total of 6.5 inches used in sizing the system is low. The Applicant should use the appropriate design rainfall totals for North Andover. Down-Gradient Nitrogen Statement 11. As required by the Watershed Special Permit,evidence that all on-site operations including, but not limited to, construction, waste water disposal, fertilizer applications and septic systems will not create concentrations of Nitrogen in groundwater, greater than the Federal Limit at the down gradient of the property boundary must be prepared by a relevant professional and submitted. Stormwater Good Housekeeping 12. The Plan should identify/indicate where the snow would be plowed. 13. The Applicant may show the location of any proposed shed/sheds on the Plan and provide a written description of the intended use as it relates to storage of fuels, fertilizers and chemicals. 14. The Applicant must provide a written description of the location and refueling procedures of any oil/fuel tanks as it relates to spill prevention and protection. Erosion and Sedimentation Controls 15. The Plan shows the erosion control.A construction detail of the proposed erosion control must be shown on the Plan. 16. A construction detail for managing any potential dewatering must be shown on the Plan. This certification is based upon the following pnojeototonnwater management features: 1. Driveway will be surfaced with pervious crushed stone and paver materia|a, allowing infiltration and treatment ufotormvvaiar runoff. 2. Planting areas and other landscaping will be installed as part of the pnojoot, allowing allowing for ( stunnwater runoff infiltration and treatment. 3. Runoff from the house roof will be captured and directed into a "Stormtech" recharge chamber system. 4. Project site is located more than 40O feet from Lake Chiohevvick and the common driveway, Great Pond Road and wetlands are present between the project site and the Lake, presenting additional opportunities for utormwotarmanagement.
