Murphy August 21, 2019. 60 Lyons Way North Andover, MA 01845 60 Lyons Way, North Andover Small Project Permit Deck Replacement Conservation Conditions of Approval,NACC ##239 Pursuant to section.4.4.2 (B) of the North Andover Wetlands Protection Regulations,jai-lies Murphy, applicant/homeowner, filed for a small project for work proposed at 60 Lyons Way,North Andover. The proposed work includes an in-bind replacement of an existing 12' by 28' deck and associated stairs and tailings. The existing deck,which was built along with the house in 2000, is approximately 85' from a Bordering Vegetated Wetland (BVW) on the property. The work does not involve any relocation or replacement of existing footings, posts or joists—however see Condition ##2 for allowances as permitted by the Conservation Comn-lission. ]::luring the August 14, 2019. public meeting, the North Andover Conservation Conntnission voted unanimously to approve this project. The project is approved only as it relates to the Conservation Connnnission and any and all other approvals should be obtained by the applicant/home owner. l-.he applicant is responsible for deternaitiilg property boundaries.All work shall conform to the following: RECORD DOCUMENTS: Small Project piling Including: Application Checklist, Project Narrative, Site Plan,Aerial Imagery, Site Photographs Received August 1, 2019 The following conditions are hereby mandated: CONDITIONS: 1. Prior to the start of construction the applicant shall ensure that the site contractor has reviewed the small project permit and is aware of the wetland resource area and the limits of the proposed work. The applicant is allowed to replace all elements of the deck should the need be determined due to tot or other zoning code requirements —Conservation Department staff shall be notified of this prior to iinpletnentation. 3. Should additional footings need to be dug and poured, the applicant shall contact the Conservation Department for staff review and approval prior to implementation—the plan shall be updated to reflect any such change regarding footing and ground disturbance 4. No hea-%,y equipment shall operate in the buffer zone to accomplish the project. 5. Dumpster (if needed) and other construction materials shall be stored in the driveway. 6. Upon completion of the approved project and site stabilization,please contact the Conservation Department for-a final inspection. 7. This permit shalt expire one year ftom the date of issuance.
