Attached please find our review letter for the plan that was submitted at this address.
Like the last plan submitted by Sullivan, they used the wrong approval letter for the black plastic chambers they selected
and therefore have some additional information to gather and demonstrate. They also are proposing to re-use the
existing pump chamber, pump and floats but there is no information provided as to whether it complies with Title 5 or not.

Septic Design Plan
Health Department:
Enclosed are the revised Septic Design Plans(3 sets) for the above referenced property for your review and
approval. In response to the review letter dated May 21, 2019 the following changes and/or revisions have
been made:
1) The DEP approval for the infiltrator system requires proof that the designer has satisfactorily
completed any required training by the company. In a phone call to Infiltrator systems on 5/29/19 by
this office there is NO training and/or certification required for designers.
2) The owner has been provided a copy of the Title 5 I/A technology approval by MassDEP. There is no
owners manual or O&M manual for the infiltrator chamber.
3) The owner agrees to fulfill his responsibilities to provide written notification of the Approval to any
new Owner,as required by 310 CMR 15.287(5).
4) This system is designed WITHOUT the use of a garbage grinder and the restriction of not allowing a
garage grinder with this design is understood and accepted.
5) The System owner understands the requirement to repair,modify, or take any other action as required
by the Department or the LAA(Local Approving Authority), if the Department or LAA determines the
system to be failing to protect public health and safety and the environment,as defined in 310 CMR
15.303.
6) A best feasible upgrade plan is shown on the design plan. A conventional septic system leaching field
(pipe/stone) is shown in trench configuration and sizing calculations are provided as well.
7) The maximum area available for a conventional system is shown(see detail on Sheet 1 in upper right
hand corner)
8) The lot area and lot dimensions have been provided on Sheet 1
9) A dimensional table has been provided on Sheet 2 for the 2 inch PVC tee inlet to the D-Box
10)The existing pump chamber(to remain)has been detailed on Sheet 1 in the lower left corner to show
invert elevations(in/out) and pump float elevations. Additionally, an emergency storage calculation
has been provided on Sheet 1 showing the chamber exceeds the minimum 440 GPD storage. On Sheet
2 a pump performance curve is shown for the existing Barnes SE51 0.5 hp pump and it clearly shows
this pump can handle the needed lift and distance for the new system.

local regulations.
