Re: Subsurface Sewage Disposal System Plan for 106 Rocky Brook Rd (Map 90A, Lot 54)
Dear Mr. Morin:
The proposed wastewater system design plan for the above site dated August 26, 2019 has been
reviewed. Unfortunately,the plan cannot be approved until the following items are corrected.
The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by
this design follows each item where applicable.
1. Provide for a notation indicating that all piping shall be Schedule 40 PVC(NA 3.2)
2. Indicate if the property is or is not within the Lake Cochichewick watershed(NA 3.2)
3. Indicate the brand and model of the proposed effluent filter(NA 3.2) and provide a copy
of the MassDEP approval letter
4. Indicate the required(minimum) annual maintenance of the effluent filter
5. Indicate that all the tanks are to be monolithic poured concrete(NA 3.2)
6. Provide a signed statement regarding the basis of the site survey(NA 3.2)
7. In the event the septic tank is found to be deficient then the building sewer pipe would be
replaced. Indicate the building sewer pipe diameter, schedule,watertightness of joints,
compact base, continuous grade and that it is to be laid in a straight line (310 CMR
15.222)
8. Provide for an access riser at finish grade over the septic tank effluent filter(310 CMR
15.227)
9. Tanks are to have soil compaction, stone bedding, 9"minimum cover specified(310
CMR 15.221 &228)
10. Provide tank buoyancy calculations (310 CMR 15.221)
11. Indicate the requirement for tanks to be watertight(310 CMR 15.221)
12. Clarify if the tank pipe penetrations are within 12" of the ESHGW(310 CMR 15.227)

13. Indicate the distribution box is to have outlet pipes at the same elevation, and they are to
be placed level for the first 2' (310 CMR 15.232)
14. Provide for the distribution box inlet tee to be at least 1"above the outlet invert(310
CMR 232)
15. Distribution box is to have soil compaction below it(310 CMR 15.221)
16. Distribution box is to be identified to be watertight(310 CMR 15.221)
17. Distribution box to have a riser to within 6"of final grade if greater than 9" of cover(310
CMR 15.221 &232)
18. Pump line is to be a minimum of 2"diameter(310 CMR 15.254)
19. Provide for soil evaluation reports to be on DEP-approved forms (NA 3.2)
20. Clarify if the C' soil layer is intended to be removed for the construction of the soil
absorption system
21. Provide for the location(s) of at least one inspection port in the soil absorption system,
show in plan view and profile (310 CMR 15.240)
22. Indicate the leach pipes are to have orifices in size between 3/8"to 5/8" (310 CMR
15.251)
23. Adjust proposed grading to meet breakout requirements on the south side of the soil
absorption system(310 CMR 15.255)
24. Indicate the size of the existing septic tank,provide for a Local Upgrade Approval
request or variance if it is not fully-sized
