6907,said premises being substantially bounded and described as follows:

DECLARATION OF RESTRICTIONS
I, Denise A. Favorat, hereby record this Declaration of Restrictions to amend my Deed which is recorded
at the Essex County Registry of Deeds in Book 1335, Page 299 pursuant to that certain Notices of
Decision Watershed Protection District Special Permit and Planning Board, issued by the Town of North
Andover Planning Department, recorded September 28 2018,at Book 15633, Page 1 and Book 15633,
Page 8,to allow for an improvement on the property.
I, Denise A. Favorat, being the Owner("Declarant") of land and improvements commonly known and
numbered as 31 Johnson Circle, Essex County, North Andover, Massachusetts consisting of Assessors
Map 97, Lot 0062 on Essex County Registry of Deeds (the "'Property")all as evidenced by Deed recorded
at Book 1335, Page 299:
Pursuant to that certain Notice of Decision issued by the Town of North Andover Planning Department
and Board of Appeals, recorded on September 28,2018 at Book 15633, Page 1 and Book 15633, Page 8,
any future use of the Property by the Owner and any of its successors or any future owners of the
property shall be subject to the following restrictions:
1. The use of fertilizers shall be limited to those which are either organic or slow-release nitrogen
and phosphorous-free;
2. The use of non-organic pesticides and herbicides is prohibited;
3. The use of coal tar-based pavement sealants is prohibited; and
4. All property owners will maintain the stone trench along the driveway,which will include
sweeping of the driveway and removal of debris,as needed.
Subject to the terms and conditions of Town of North Andover Planning Board and Zoning Board Notice
of Decisions, recorded at Essex Registry of Deeds at Book 15633, Page 1 and Book 15633, Page 8.
These restrictions shall run with the land.

recorded at the Essex North District Registry of Deeds at Book 1335, Page 299.
