Agreement nter ed into by and between Wastewater,T17eatment Seel-Vida,Inc. (herein calledand the

whIch is described below.

I Testiilg of the sludge depth "In the septic tank,
Inspection, power testing and clean/replace intake filter of the air*blower.
Inspection of the alarm,system.
Inspect overall condition of F.ASTO System,.
5 Notification to,OWNER ofray problems encountered.
6 Service othier than rouline maintenance will be billed at an hourly rite,PILIS travel and parts.
WTS shall notify the local Board of IJealth and Department of Environmental Protection in wi.Iling within 24
hours f a system faiftire or alarni event in lUdi g corrective measures that have been taken.,
OWNERwill b bulled standard W` S charges for any plartS used in repairs or maintenance. Any additiotial labor
time will be billed to the OWNER at Our current labor.rates.

OWNER Understands that(1)ANNtJAL RIATE payment is for one year only comi-riencing on,the erfect ive date set
forth above and is nion-rieftindable.;(2) CUITent Regulations require OWNER to maintain a service agreement
for the life of the AS 0 System, and (3)ANNUAL RATE is subject to change based on current WTS rates. I

Otisite testing performed twice per year will be used to dernonstrate that the sys tenis are operating at a secondary
treatment standard of 30 mg/L of BOD5 and TSS. The following will be Performed:
1) VISLial.exarninatilon of the effltient foi,color, turbidity and efflu.ent solids.
2) Effluent pl-I to deter1iiine if the waste water is between 6 and 9 standw.-d units.
3) Dissolved Oxygen,2mg/L or niore,to en.SL11'e that the system is operating.
4) Turbidity, less th,an or equal t N"T'Li. IJ
If the CfflUent does not meet efflUent quality standards, a grab sample will be coll,ected for laboratory analysis.
Results sent to state andloca]Agencies as well, as the OWNER, WNER is responsible for providing acceptable
access to effluent for field testing and/or to enable a grab sample to be taken for laboratory testing perfor-i-neel. If
such laboratory sample is reqUired, OWNER will be,responsible for charges incurred. IF RE QUIRED9 THE
COST FOR THIS ADDITIONAL TESTING WILL BE $200.00NISIT.
