HomeMy WebLinkAboutDenial Letter - Correspondence - 130 REA STREET 11/12/2019 TLED 41 ea North Andover Health Department Community and Economic Development Division November 12, 2019 Benjamin Osgood, P.E. 157 Bluff Street Salem,NH 03079 Re: 130 Rea Street(Map 98A,Lot 9) Dear Mr. Osgood: The proposed wastewater system design plan for the above site dated September 9, 2019 and revised October 30, 2019 has been reviewed. Unfortunately,the plan cannot be approved until the following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by this design follows each item where applicable. 1. In a letter submitted which is dated October 30, 2019 there are several references to a document which provides an explanation of the requested variances. Variances need to show in writing their compliance with the standards at 310 CMR 15.410. No such explanatory material was provided 2. In a letter submitted which is dated October 30, 2019 there are several references to the MassDEP's STU approval standard conditions allowing a loading rate of 0.20 to be used. That is a mis-interpretation of this document. Regardless, since the percolation test at this site was unable to be completed due to slow infiltrative capacity of the soil it is not possible to have a loading rate as defined in Title 5. A variance is needed to not have a percolation test(as indicated in the next item) 3. As indicated in note#9 in the previous review, a variance to Title 5 needs must be provided to have a design based on no acceptable percolation test results. This needs to be indicated on the design plan and on accompanying information 4. The design plan indicates a Local Upgrade Approval is being sought to reduce the size of the required 5' overdig around the soil absorption system. This cannot be authorized through a Local Upgrade Approval and must be indicated as a variance to Title 5 on the design plan and accompanying information 5. The design plan contains a proposed contour labeled 99 which does not appear correct 6. The design plan depicts a soil stockpile area located behind the soil absorption system. Using this location for a soil stockpile area would require driving over the soil absorption system 8. The design plan does identify the 5' overdig on the site plan,nor is it clear which areas are proposing to have a reduced overdig 9. The Maintenance Notes makes no references to the required services needed for the proposed secondary treatment unit(NA 3.2) 10. Provide information on the methodology to be used to prevent soil scouring associated with the 1/a"orifice proposed to be located in the lateral end elbow 11. It is generally prudent to consider using a sweep rather than an elbow for ease of maintenance of the pressure distribution laterals. You may wish to consider this adjustment 12. It appears the existing building sewer is intended to remain in place and not be replaced. Explain why this is necessary, and indicate on the design plan if there are specific items which should be examined at this pipe to assure it is watertight and properly functioning 13. Provide DEP Form 12 documenting the attempted percolation test 310 CMR 15.018 14. Provide a draft of the STU and pressure distribution system operation and maintenance agreement 15. Depict the location of a proposed standard septic tank which could be installed at this site as required by the Singulair-brand STU approval letter 16. Provide a revised Form 9A indicating the Local Upgrade Approvals and variances being sought 17. A loading rate of 0.20 gpd/sf cannot be used when a secondary treatment unit is proposed with a requested reduction in size 310 CMR 15.242(b) Please feel free to contact the office or Mill River Consulting at 978-282-0014 with any questions you may have.
