The work includes installing a new gas service connection via the open trenched method within the existing paved roadway and maintained lawn/landscaped areas. A portion of the service connection is within the 100'buffer zone of a Bordering Vegetated Wetland (BV along the southeastern property line. Erosion controls (12" minit-num compost or bark rnulch sock) shall be installed between the area of work and the wetland resource area, following the updated site plan from October 8,2019 (sock running diagonally from the back of the house and straightening out along the tree line;no EC off of the property). Silt sacks shall also be installed in any downstream catch basins within the work area (if any),. 3. Prior to the start of work, tlic applicant shall obtain written permission from the Massachusetts Department of Transportation for street opening and provide a copy to the Cofftn-n scion for its files. 4. Once erosion controls are in place and prior to the start of work, the applicant shall arrange a preconstruction meeting at the site with the Conservation Department. 5. Upon completion of the approved project,please contact the Consetvation Departi-nent for a final inspection to approve erosion control removal. Work may then proceed at the owner's risk only upon notice to the Department and to the Conservation Commission. Appeals The applicant, owner, any person aggrieved by this Determination, any owner of land abutting the land upon which the proposed work is to be done, or any ten residents of the city or town in which such land is, located, are hereby notified of their right to request the appropriate Department of Environmental Protection Regional Office(see htto://www.mass,00v/eea/aaencies/iniassdeD/abouUcontactst) to issue a Superseding Determination of Applicability. The request must be made by certified mail or hand delivery to the Department, with the appropriate filing fee and Fee Transmittal Form (see Request for Departmental Action Fee Transmittal Form) as provided in 310 CIVIR 10.03(7)within ten business days from the date of issuance of this Determination. A copy of the request shall at the same time be sent by certified mail or hand delivery to the Conservation Commission and to the applicant if he/she is not the appellant. The request shall state clearly and concisely the objections to the Determination which is being appealed.
