Re: Design Plan for 340 Forest Street (Map 106A, Lot 215)
Dear Mr. Sullivan:
The proposed wastewater system design plan for the above site dated November 19, 2019 has
been reviewed. Unfortunately, the plan cannot be approved until the following items are
corrected. The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is
not met by this design follows each item where applicable.
1. On Sheet 2 of 2, note that a gas baffle shall be installed on the outlet of the septic tank
(227(4)).
2. An effluent filter is to be installed on the outlet of the septic tank. Include the brand and
model approved by the DEP, filter type/name, riser with manhole cover at grade above
the filter, and the DEP approval letter(227(7)) (NA 3.2).
3. Depict the outlet tee centered beneath the access cover(227(1)).
4. Provide a note that the septic tank shall have 1 childproof 20"riser/manhole to grade over
the outlet(221 &228(2)).
5. On Sheet 2 of 2, add to the note beneath the septic tank that the 6" crushed stone
compacted base must be<=1'/2" stone (221(2) &228(1)).
6. Provide a note that the septic tank shall be watertight(221(1)).
7. Provide a note that there shall be a minimum of 9" of cover over the septic tank (228(1)).
8. Provide a note that the top of the septic tank and the top of the pump chamber shall be<_
36"below grade (221(7)).
9. On Sheet 2 of 2, add to the note beneath the distribution box that the 6" crushed stone
compacted base must be <=1%2" stone (221(2)).

10. Since there is greater than 9"of cover over the distribution box, a riser to within 6"of
final grade is necessary(221(13),232(3)).
11. On Sheet 2 of 2, the pump chamber is noted to have"20"Dia. Access Covers",provide a
note that the pump chamber shall have 1 childproof 24"riser/manhole at final grade
(231(5)).
12. On Sheet 2 of 2,add to the note beneath pump chamber that the 6" crushed stone
compacted base must be <=1%z"stone(221(2) &228(1)).
13. Provide a note that the pump chamber shall be watertight(221(1)).
14. Clarify Dosing Chamber Note 7 regarding a"lazy drain".
15. Provide dimensions of the desired inlet tee inside the distribution box
16. The pump curve provided appears to show an operating flow rate in excess of 40 gpm,
while Dosing Chamber note 11 indicates to use a pump with less than 40 gpm flow rate.
Select a different pump or else provide greater clarity on how this design requirement is
to be achieved.
17.Note the soil class used as the basis for design on the plan.
18. The town's assessor's information indicates the dwelling currently contains 3 bedrooms,
while the design plan is completed for 4 bedrooms. Clarify this discrepancy, or provide
for a design plan which meets the New Construction standards in Title 5.
19. Since the Infiltrator Chamber system is proposed as an alternative soil absorption system
the"Standard Conditions for Alternative Soil Absorption Systems with General Use
Certification and/or Approved for Remedial Use"will apply. Please provide the
following as required by the approval conditions.
Section II(2):
- In accordance with 310 CMR 15.240(6), absorption trenches should be used
whenever possible. Accordingly, approved Disposal-Only and Treatment with
Disposal Alt. SAS Systems shall be used in trench configuration whenever
possible, unless a different configuration is allowed by the Approvals)
Special Conditions.
Section II(7)•
c) Prior to Local Approval of the System, the Designer shall show on the plans
the maximum available area for a conventional system (without reserve)
designed in accordance with the standards of 310 CMR 15.100 through
15.255.
e) The record drawings, approved by the LAA, must clearly indicate an area for
the best feasible replacement system that could be installed in the event that
the proposed Alternative Soil Absorption System fails or it is determined that
it is not capable ofproviding equivalent environmental protection.

Section II08):
c) certification by the Designer that the design conforms 10 the Approval, ally
Company Design Guidance, and 310 CMR 15.000; and
d) a certification, signed by the Owner ofrecord for the property to be served by
the TechnoloD} staling that the properly Oivner:
i) has been provided a copy of the Title 5 IIA technology Approval,
the Owner's Manual, and the Operation and Maintenance Manual,
and the Oii,ner agrees to co)nply i4th all terms and conditions;
ii) for Sysle)ns installed under a Ren)edial Use Approval, the owner
agrees to f tUiill his responsibilities to provide written notification
of the Approval to any neii, Otiimer, cis required by 310 CMR
15.287(5);
iii) if the design does not provide for the use of garbage grinders, the
restriction is underslood and accepted; and
h) iwhethe)-or not covered by a warranty, the System Oiimer
understands the require)nent to repair, replace, modem or take any
other action as required by the Department or the LAA, if the
Department or the LAA determines the System to be failing to
protect public health and safety and the environment, cis defined in
310 CMR 15.303.
Additionally, while not a reason for disapproval you may wish to consider the following:
It is reconmiended that the pump be submersed at all times. You may wish to raise the
height of the float elevations accordingly.
If sheet flow from the road onto property occurs, you may wish to divert flow around the
proposed SAS.
Provide an exterior elevation such as the bottom of siding rather than an internal
dimension such as the top of foundation as indicated as the current benchmark
